Book excerpt: Advances made in microsurgical techniques during the last three decades have dramatically improved the treatment of complex limb injuries, such as a closed fracture covered by healthy tissue and a complex open fracture of the tibia. Microvascular transfers from the foot have revolutionized the treatment of digital amputations, and vascularized bone transfers are used in various indications. The microvascular techniques are still evolving. Many imaginative new flaps are being proposed to meet precisely the functional requirements of the various defects. Alternative anastomotic techniques are being investigated and the no-flow phenomenon has been extensively studied. Allotransplantation may be feasible in the future. This unique book is a result of the exchange of ideas among clinicians of various specialties noted for their innovations and basic researchers involved with the problems of microvascular reconstruction of upper and lower extremities. While most of the chapters are invited contributions, some are based on presentations made during an international symposium held in Brussels, Belgium, in 1997.

Available in PDF, EPUB and Kindle. Book excerpt: This book focuses on the internal fixation of long bones by using intramedullary locked nails in a closed technique. Intramedullary fixation fulfils the biological requirements for fracture healing and minimises surgical trauma. The text illustrates the use and relevance of this technique in orthopaedic and trauma surgery including reconstructive surgery, covering the basic scientific principles of reaming and locking as well as basic and advanced surgical techniques. Prevention of complications and complication management are also discussed in detail, making it an ideal text for those with an interest in the proper use the techniques described.

This can have a severe psychological impact on the patients, and represents a multitude of challenges for orthopedic surgeons. This book discusses currently available tools for diagnosing long bone nonunions, illustrates means of prevention, and specifies the indications for management using compression-distraction techniques. Based on hundreds of cases treated personally by the authors.

Available in PDF, EPUB and Kindle. Book excerpt: Professor B.G. WEBER has once again and in a very timely fashion produced a superb book on an orthopaedic subject of great importance. "The Extemal Fixator" is the most comprehensive text on the subject in orthopaedic literature to date. Professor WEBER thoroughly discusses extemal fixation with c1arity, organization, profuse illustrations and roentgenograms. Professor WEBER acknowledges that the use of extemal fixation in orthopaedic surgery is not newand traces its history over the years. He points out c1early the fact that though at various times the "method" has experienced periods of disrepute, modem sophistication, improved technology and a better understanding of its philosophy have given the system a newand perhaps permanent place in the armamentarium of the orthopaedic surgeon. All methods of treatment have critics as well as supporters, and not infrequently the strongest criticisms are the result of poor understanding of the philosophy proposed and its proper implementation. Professor WEBER, in his carefully detailed and well illustrated book, has made it abundantly c1ear that the use of extemal fixators in the treatment of fractures must be c1early understood by the orthopaedic surgeon in order to obtain satisfactory clinical results. His discussion of its philosophy, pathomechanics and technology are most comprehensive and leave "no stone untumed" and because of this the book represents a most comprehensive text on the subject.

Available in PDF, EPUB and Kindle. Book excerpt: This volume deals with the transosseous external fixation techniques that I have been developing over the course of the past 40 years. During this time, our research in medicine, biology and engineering has led to the evolution of more than 800 unique, highly effective methods of treatment that extend beyond the realm of traumatology and orthopedics. The book features a comprehensive theoretical and clinical description of the biologic laws governing the depen dence of the shape-forming processes of bones and joints upon the adequacy of blood supply, as well as a delineation of the effect of tension-stress upon the genesis and growth of tissues. I have in cluded our latest data on tissue growth and regeneration during transosseous osteosyntheses. The book summarizes the biomechanical principles of applica tion of my apparatus; clinical cases selected from more than 25000 patients illustrate the management of some of the most complex disorders of the locomotor system. New solutions to many therapeutic problems are described. In particular, severe limb trauma with large defects of bone, vessels, nerves and skin can be managed without resort to transplantation. Radical debridement surgery can be followed by a one-step restora tion of the missing tissue, thus decreasing the likelihood of a serious wound infection or an amputation.

Book excerpt: The papers presented at the Fourth 'Meeting of the European Society of Biomechanics, held in collaboration with the European Society for Biomaterials in late September 1984 in Davos, Switzerland, are published herewith. The main idea of the meeting was to gather together the many disciplines of researchers and clinicians active and interested in promoting biomechanical knowledge in one interdisciplinary society: the European Society of Biomechanics. We feel that the dialog across the disciplines is one of the important goals of the society, a goal which can be furthered by meetings like the one in Davos. A surgeon, whether a general, trauma or orthopaedic surgeon, is normally brought up without relevant exposure to spe cific technical problems. It therefore is not surprising that he speaks a different language with respect to mechanical problems than an engineer. Although a surgeon often has a feeling for what the solution to a particular problem might be, a fruitful inter disciplinary collaboration is made difficult by this scientific language barrier. On the other hand, a physicist, chemist, engi neer and metallurgist, to name a few, would do well with a realistic perception of the possibilities and limitations of surgery and of the relevance of a solution found to the initial question. Similar problems exist in other areas, e. g. in the field of sports biomechanics in the dialogue betweeen coach and researcher. Interdisciplinary misunderstandings have led to quite some unaeces£ary frustration in the past.

The chapters discuss the anatomy of bone vascularization, the physiology of vascular regulation, the histopathology of microcirculation and osteonecrosis, experimental studies on bone-blood flow, experimental surgery, methods of exploration, vascular studies of grafts and bone transfer, and surgical and conservative treatment. New developments are given on blood-bone barrier, effect of PGE2 on blood-bone flow, laser Doppler flowmetry, microcirculation and demineralization, vascular repair in osteotomy and fracture, bone arteriography, angioscintigraphy, Ilizarov's technique, and therapeutic aspects of lipid-clearing agents.

Book excerpt: External fixation is now being used widely to maintain fractures, osteo tomies, and arthrodeses in a desired position during consolidation. Whereas external fixation has been readily accepted in European countries, its use has weathered a rather stormy course in North America, especially in the treatment of fractures. Only recently has external fixation found its rightful place on this continent as well. Many different models are on the market today, and the practitioner is faced with a difficult decision in selecting a model. Should he buy a system where the fracture has to be reduced first, or should he work with a device permitting a reduction after insertion of the pins? To enable surgeons to study the different systems, to discuss their advantages and disadvantages, and to permit them to put their hands on these devices and inspect them personally, the Division of Orthopedic Surgery, University of Ottawa organized an applied basic science course in May 1981, External Fixation of Fractures. During this course, all major systems were pre sented to the participants. As happened during the course "Internal Fixation of Fractures" held two years ago, the rigidity of internal fixation was frequently and intensively debated. Whereas the rigidity of internal fixation cannot be altered during the course of healing, the rigidity of external fixation can be changed. In fact, with progression of union, rods of increasing elasticity can be used.
