Book excerpt: Section 723 of the National Defense Authorization Act for fiscal year 2006 directed the Secretary of Defense to "establish within the Department of Defense a task force to examine matters relating to mental health and the Armed Forces" and produce "a report containing an assessment of, and recommendations for improving, the efficacy of mental health services provided to members of the Armed Forces by the Department of Defense." Towards that end, the Department of Defense Task Force on Mental Health (Task Force) was established, comprising seven military and seven civilian professionals with mental health expertise. Maintaining the psychological health, enhancing the resilience, and ensuring the recovery of service members and their families are essential to maintaining a ready and fully capable military force. Towards that end, the Task Force's vision for a transformed military system requires the fulfillment of four interconnected goals: A culture of support for psychological health, service members and their families will receive a full continuum of excellent care in both peacetime and wartime, sufficient and appropriate resources will be allocated to prevention, early intervention, and treatment, at all levels, visible and empowered leaders will advocate, monitor, plan, coordinate and integrate prevention, early intervention, and treatment. Findings: In general, the Task Force found that current efforts fall significantly short of achieving each of the goals enumerated above. The Task Force arrived at a single finding underpinning all others: The Military Health System lacks the fiscal resources and the fully-trained personnel to fulfill its mission to support psychological health in peacetime or fulfill the enhanced requirements imposed during times of conflict.

Shortly after troops started returning from their deployments, some active-duty service members and veterans began experiencing mental health problems. Given the stressors associated with war, it is not surprising that some service members developed such mental health conditions as posttraumatic stress disorder, depression, and substance use disorder. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

In August 2012, the President identified mental health care for servicemembers as a high priority. The National Defense Authorization Act for Fiscal Year 2015 included a provision for GAO to assess the perception of the impact of mental health care stigma. This report examines (1) military servicemembers' and deployed civilians' reported perceptions about mental health care stigma; and the extent to which DOD (2) has policies and related efforts to address mental health care stigma and (3) is positioned to measure the progress of its efforts to reduce mental health care stigma. GAO analyzed program documentation and DOD-wide and service-specific survey results; conducted 23 non-generalizable focus groups with servicemembers and 3 with deployed civilians at six locations representing distinct and dispersed populations; and interviewed DOD and service officials.
