Funding pledges from the donors have doubled resource commitments between 2002 and 2004 to over $6 billion. That surge in funding belies the volatile nature of contributions to HIV/AIDS initiatives at the country level. The paper analyzes the impacts of abrupt HIV/AIDS funding on macroeconomic stability, fiscal health and the development of health institutions. The macroeconomic effects are ambiguous, but depend on the overall level of aid flows, as well as those for HIV/AIDS, the management of foreign exchange inflows, and effective spending policies. The fiscal ramifications revolve around the jump in external funding that reached around 1000% in Lesotho and Swaziland, and 650% in Zambia between 2002 and 2004, and the required rapid scale up if resources are to be used productively. At the same time, the new HIV/AIDS monies are swamping public health budgets in some cases exceeding 150% of the government's total allocations for health. The vertical HIV/AIDS programs and the set aside funding threaten to undermine the very institutions that will need to carry forward the long term HIV/AIDS prevention and treatment agenda for each country. Health systems are already fragile, and governance problems and uneven productivity compound the challenges. Health institutions require funding and attention to strengthen them in the fight against HIV/AIDS. While the committed funds are desperately needed, solutions to the dilemma will require creative options to ensure the flow of funds, manage the economic implications and ensure effective service delivery. These are explored in the concluding section.

This study was led and financed by the World Bank and conducted in collaboration with the Government of Jamaica and United Nations Programme on HIV/AIDS (UNAIDS). This study examines the impact of federal funding allocated to U.S. cities through the Ryan White CARE Act, which is the largest program for combating HIV/AIDS in the United States. The empirical approach identifies the impact of the funding by studying funding variation that comes from Ryan White policy features that resulted in large funding differences among cities that were originally on parallel HIV/AIDS trajectories and finds that Ryan White's city-level funding has improved HIV/AIDS outcomes in the cities receiving the funds. The estimates indicate that one HIV/AIDS death has been avoided for every $314,000 allocated through the program and that the program has saved approximately 60,000 lives through 2018. The estimates also indicate that funding differences across cities have contributed to the uneven progress in combating HIV/AIDS across the United States.

Book excerpt: Rationale: New global funding mechanisms for HIV/AIDS have arisen in the wake of a concerted global effort to mobilize support for the fight against HIV/AIDS - namely, the Global Fund for AIDS, Tuberculosis and Malaria (GFATM) in 2001 and the Presidents Emergency Plan for AIDS Relief (PEPFAR) in 2003. In 2003, WHO initiated "3 by 5", a campaign to place 3 million individuals, or roughly 50% of those in need, on ART by December 2005. Although the targets set by 3 by 5 have not been achieved, the demonstrated efficacy of ART in rich countries and in resource-poor setting provides a strong rationale for continued emphasis on expanding coverage. External funding to support the rapid scale-up of ART has signifcantly increased. However, questions remain regarding the actual increase in treatment coverage of the population in need that has been achieved, and the possible overestimation of the numbe of people receiving treatment, claimed by different programs. Objective: In this analysis, we will analyze the impact of spending on HIV/AIDS on targets set by global health organizations. Specifically, we will assess the relative contributions and overall impact of Global Fund and PEPFAR spending on observed trends in ART coverage. Methods: To quantify the impact of Global Fund and PEPFAR spending on scaling up ART coverage, we use ART coverage - the ratio of treatment utilization to estimated need for ART - as the outcome variable in analysis. We obtain these variables separately from UNAIDS country-specific epidemiological fact sheets and from online databases available from the WHO. The consistency of estimates across sources is investigated. Disbursement information for HIV/AIDS was obtained from the Global Fund and PEPFAR websites. Additional covariates - GDP per capita and indicators of government effectiveness - were included in the model to investigate the influence of other country-level determinants. Results: Our preliminary findings suggest that countries receiving concurrent assistance from the Global Fund and PEPFAR are experiencing increases in rates of ART coverage. Conclusion: We find that Global Fund and PEPFAR spending are having a positive impact on the expansion of ART coverage. These results must be interpreted with caution due to data quality issues such as cross-country discrepancies in how coverage data was collected and reported. Because the organizations function differently in ways that might influence spending outcomes, future work to disentangle the relative contributions of Global Fund and PEPFAR to these outcomes is especially important.

This volume highlights work conducted under the umbrella of a World Bank work program on “The Fiscal Dimension of HIV/AIDS,” including country studies on Botswana, South Africa, Swaziland, and Uganda. It covers four aspects of the fiscal dimensions of HIV/AIDS: First, it aims for a comprehensive analysis of the fiscal costs of HIV/AIDS, with a wider scope than a costing analysis focusing on only the policy response to HIV/AIDS. Second, it embeds the analysis of HIV/AIDS costs in a discussion of the fiscal context, and interprets these costs as a quasi-liability, not a debt de jure, but a political and fiscal commitment that binds fiscal resources in the future and cannot easily be changed, and very similar to a pension obligation or certain social grants or services. Third, it develops tools to assess the (fiscal dimensions of) trade-offs between HIV/AIDS policies and measures that take into account the persistence of these spending commitments. Fourth, most of the fiscal costs of HIV/AIDS are ultimately caused by new infections, and this study estimates the fiscal resources committed (or saved) by an additional (or prevented) HIV infection. Building on these estimates, the analysis here is able to assess the evolving fiscal burden of HIV/AIDS over time.
