Book excerpt: Since the development of the first laser generator by Maiman in 1960, laser energy has gained increased interest and has subsequently been used in various fields of applica tion. Due to its high energy density, combined with a sharply defined irradiation field, laser has aroused great expectations, especially since laser beams have been used very successfully in various fields especially technology and medicine. Medical and surgical applications of the laser were among the first and have been of undisputed benefit in, for example, ophthalmologic interventions such as coagulation of the retina, or for endoscopic tumorectomy in the bronchial, gastrointestinal and urogenital tracts. Consequently, considerable research effort was made to also apply laser techniques in interventional cardiology for the treatment of atherosclerosis, aimed at removing plaque material from stenosed tissue or occluded ateries and recanalizing tough and calcified segments unsuitable for the well-extablished technique of balloon dilatation. Ablating atheroma rather than pushing it aside was thought to achieve better results. However, it was found that the laser may create rough-edged cuts partly with substantial thermal or mechanical damage to the surrounding tissue, consequently provoking preconditions for unwanted restenosis. Different types of laser generators and probes have therefore been investigated in arterial vessels, mostly focusing on indirect laser effects. Intravasal probes with laser heated metal tips were used, as well as balloon catheters with an incorporated laser fiber designed to heat the surrounding tissue outside the balloon and to, ineffect, "iron out the plaque".

The purpose of the study, as formulated by the STG, was to analyze future health care technology. Part of the task was to develop an 'early warning system' for health care technology. The primary goal of the project was to develop a list or description of a number of possible and probable future health care technologies, as well as information on their importance. Within the limits of time and money, this has been done. However, given the vast number of possible future health care technologies, complete information on the importance of each area could not be developed in any depth for all technology. Therefore, four specific technologies were chosen and were prospectively assessed. These future technologies were examined in more depth, looking particularly at their future health and policy implications. Subsequently, the project was extended to September 1986, and two additional technologies will be assessed.

Book excerpt: Since the introduction of balloon angioplasty for the relief of coronary artery stenoses and of anginal symptoms in patients with coronary artery disease by Andreas Griintzig in 1977, the field of interventional technology and treatment strategies has grown enormously. For the reduction of hemodynamically significant coronary artery stenoses balloon angioplasty is the standard and reference method with a high primary success and low complication rate. Because of the relatively high recurrence rates of 30-40% of balloon angioplasty a whole family of different angioplasty techniques has been developed since then. Among those are atherectomy devices, laser angioplasty, radiofrequency angioplasty, high and low speed rotational angioplasty and stenting of stenosed vessels. Balloon angioplasty has been extended to aortic and mitral valve stenoses, and supraventricular and ventricular tachycardias can now be treated by catherter ablation techniques. In 1989 an International Symposium on standard and newer inter ventional techniques has been held at the University of Ulm. This volume contains the essential parts and presentations of the international faculty of experts in the field. In four chapters the principles, advantages, pitfalls and future developments of coronary angiopiasty, angioplasty of peripheral arteries, balloon valvuloplasty and of catheter ablation of supraventricular and ventricular tachycardias are described in detail. Improved lasing techniques in the cardiac catheterization suites and operation rooms have been implemented for treatment of ischemic coronary syndromes, peripheral arterial occlusive disease and other atherosclerotic thrombotic conditions. Moreover, during this 20 year time frame, several multicenter and single center clinical studies have been published focusing on the role and utilization of lasers in coronary and peripheral revascularization. Book excerpt: Percutaneous Transluminal Coronary Angioplasty is presently the widest used non-surgi cal method for the treatment of stenotic lesions in coronary artery disease. Continuous development of the procedure and equipment has made complex and multi-vessel-inter ventions possible and has led to other techniques of intravascular angioplasty. In reviewing current results, this volume pays particular attention to causes, incidence, circumstances, recognition, treatment and outcome of complications of PTCA; it also considers restenosis, which still occurs at an unsatisfactory high rate after primarily successful procedures. To deal with these topics, experts on PTCA met to discuss their experiences and insights into peculiar aspects of complications in PTCA. The discussion was subdivided into: Recognition of complications; Complications in new interventional techniques; Management of complications; and Restenosis after angioplasty. The question of whether angiographic and pathological characteristics of sclerotic lesions allow the identification of complication-prone lesions in advance is discussed in the first chapter. One paper deals with the state of the art of digital cardiac imaging, which enables the operator to have an early knowledge of and adequate reaction to possible complications occurring during angioplasty. Special attention is directed to the so-called "high-risk"-PTCA, detailing the clinical variables, in order to have better knowledge about patients with a higher probability of unsuccessful or complicated procedures.
