Materials and Methods: Metal spheres were placed in partially edentulous casts. CbCT scans were taken and used to localize the centre of the spheres. The experimental trace and conventional fiducial registration protocols were performed to localize the sphere centres, and the results compared to the findings from the CbCT scans. Each protocol was completed three times for each sphere, for a total of 756 data points. The average difference found between the registration methods was compared. Results: The trace and fiducial registration protocols were comparable, localizing the centre of each sphere to an average difference of 0.33 mm, though trace registration measurements showed greater variability. Conclusions: With appropriate case selection, trace registration may be a practical and comparably accurate alternative to fiducial registration with the Navident system.

Many authors reported good results in terms of implant placement accuracy using different navigation systems. Augmented reality (AR) is employed in different surgical fields, although only few dentistry studies are available to date. Aim: to present clinical cases of implant placement using a dynamic navigation system associated with an augmented reality device and to report its accuracy .Material and Methods: Two patients, both needing implant rehabilitation in the upper premolar area, were treated with flapless surgery. Each patient received one implant (WinSix, Ancona, Italy; Straumann, Switzerland). In both cases, the use of AR glasses (Fifthingenium, Milan, Italy) displaying a dynamic navigation system (ImplaNav, BresMedical, Sydney, Australia) allowed the deployment of a computer-aided procedure. Accuracy using this procedure was evaluated measuring on CBCT the deviation between real and planned positions of the implants.Results: The deviations for the first implant were 0.53 mm at the entry point and 0.50 mm at the apical point; for the second implant were 0.46 mm at the entry point and 0.48 mm at the apical point. The angular deviations were respectively 3.05u00b0 and 2.19u00b0. The use of AR glasses allowed the surgeon to have at the same time both the view of the surgical field and of the navigation system monitor displaying implant planning and virtual burs.Conclusion and Clinical implications: For both surgeries, it was possible to proceed using the AR technology as planned. The use of augmented reality applied to dynamic navigation led to accurate values of implant placement. The association of these two technologies seems to be promising in dental implantology. More studies are needed to confirm these results.

Some of these are an accurate and prosthetic-based implant placement, the surgical time reduction and safe use of a less invasive surgical procedure, like the flapless technique. This technique allows a higher patients comfort but is more challenging and is usually associated with less accurate placement of the fixture with respect to an open-flap method.Aim: to investigate the difference in terms of accuracy of implant placement between an open-flap and a flapless technique using a dynamic navigation system.Material and Methods: 12 patients were consecutively recruited and allocated to the flapless or open-flap group depending to soft tissue parameters. A total of 20 implants were placed: 11 with a flapless technique and 9 with the conventional open-flap method. The surgery was performed with the navigation system according to the virtual planning. The deviation between the real implant position obtained from the post-operative cone-beam computed tomography and the planned one was measured. Data were statistically analysed using a Mann-Whitney U test.Results: The deviation of implants placed using a flapless approach was 0.88 uf0b1 0.50 mm (range 0.46-1.84 mm) at the insertion point and 1.09 uf0b1 0.62 mm (range 0.48-2.28 mm) at the apical point. The depth error was 0.37 uf0b1 0.29 mm (range 0.03-0.86 mm) and the angular deviation was 4.78 uf0b1 2.58uf0b0 (range 1-9.7uf0b0). Using a conventional open-flap technique, the errors were 1.06uf0b1 0. 48 mm (range 0.45-2.21 mm), 1.40 uf0b1 0.58 mm (range 0.59-2.21 mm), 0.45 uf0b1 0.37 mm (range 0.18-1.41 mm), and 7.13 uf0b1 4.76uf0b0 (range 1.4-15.9uf0b0), respectively. The values of the two groups were not significantly different (p = 0.43, p = 0.27, p = 0.59,p=0.20).Conclusion and Clinical implications: no difference was found between the conventional open-flap and flapless approaches in terms of the accuracy of implant placement. Therefore, the use of a navigation system can overcome the drawback of a u201cblindu201d technique and ensure a high level of accuracy.

After cone-beam CT acquisition, DICOM files were imported and merge with predetermined crown STL data, planning restorative driven implant placement. 11 implants placed in healed ridges, 5 implants in immediate placement. Post operation assessment use the EvaluNav software to evaluate the deviations between the planned and the postsugery position of the implants. Estimate the deviations of entrance point, apical point (3d), apical point (v) and angle deviations.Results: The EvaluNav software estimate the mean deviation of entrance point is 0.9 3 u00b1 0.18 mm, the apical deviation (3d) is 1.61 u00b1 0.87 mm, the apical deviation(v) is 0.90 u00b1 0.92 mm and the angle deviation is 3.43 u00b1 0.99 degree. Used the navigation guided surgery was easily to determine the inferior border of the sinus walls, more efficiency for antrostomy of proper sized sinus lateral window and avoid the complications during sinus floor elevation procedures.Conclusions: Based on the limitations of this presentation, it can be concluded that the accuracy of computer-aid dynamic guided implant surgery is within clinical acceptance, and decrease human free hand errors. The safety range of at least 2 mm is need respected. Navigation guided surgery of antrostomy for lateral sinus elevation augmentation is clinic efficiency and save time for open lateral sinus window elevation procedures.

More research is needed to evaluate the outcome of implant-supported surgical procedures assisted by dynamic navigation.Aim/HypothesisTo describe the workflow of a maxillary full-arch rehabilitation through the All-on-4u00ae concept assisted by dynamic navigation. Material and MethodsA female healthy patient with 57 years of age presented with need of maxillary full-arch rehabilitation. Orthopantomography and Cone Beam Computerized Tomography (CBCT) scan were taken and the DTX Studio Implant Software was used to evaluate the baseline clinical situation.The surgery was performed under local anesthesia. After incision, flap raising and bone reduction 5 tri-screws were fixated in the maxilla. A CBCT scan was made and in the DTX Studio Implant Software the position and size of implants were selected. After the fixation of the X-Guideu00ae receptor the navigation surgery was initiated. The drills used to prepare the implant bed were calibrated before the osteotomy. Four implants (All-on-4u00ae Maxilla MALOCLINIC protocol; Nobel Parallel CC implants) were inserted achieving 35 N/cm of primary stability; Multi-Unit Plus abutments were attached and a provisional pre-made prosthesis was connected achieving immediate function.ResultsThe X-Guide Navigationu00ae provided the precise position and orientation of the drills and implants in every step of the surgery. This tool enabled the placement of the implants in the most favorable prosthetic and surgical positions, avoiding important anatomic structures such as the maxillary sinus and the nasal cavities. All the implants were placed with an insertion torque of 35 N/cm which allowed the connection of the prosthesis on the same day of surgery, achieving immediate function. The main advantages of the dynamic navigation assisted surgery include its accuracy, time- and cost-effectiveness, and the ability to change the implant size, system and location during the surgical procedure. Nevertheless, the clinician must understand that a learning curve is required to gain proficiency using a dynamic navigation system. This system could benefit from the following improvements. For total edentulous cases, simpler fiducial markers could make the surgery faster and less complicated once the tri-screws fixation costs time and could limit the implants position.Conclusion and Clinical implicationsThe surgical procedure assisted by dynamic navigation for the full-arch rehabilitation of the maxilla was safe and predictable, allowing an accurate insertion of the implants in depth, angulation and crestal position. The system could benefit from improvements in order to make the management of full-arch cases simpler.

The collected data were used to analyze the possible differences of the implant fixture body placement between preoperative plan and postoperative actually placed implant, evaluating the angulation and distance errors.Results: The mean angulation errors between the preoperative planned and postoperative placed implant was 5.84u00b0 u00b1 1.87u00b0; the mean distance errors between the planned and actually placed implants were 0.67 u00b1 0.38 mm horizontally and 0.74 u00b1 0.48 mm vertically at the implant neck and 0.82 u00b1 0.51 mm horizontally and 0.74 u00b1 0.39 mm vertically at the implant apex for all 57 implants.Conclusions: Computer-guided implant planning and insertion with 3Shape software, seems to provide sufficient accuracy, although more clinical studies should be conducted. Deviations are mainly related to CBCT accuracy and the implementation of the digital radiographic data to the 3D model through the software planning system.

Nevertheless, the evaluation of the surgical anatomy and the simulation of the procedures still represent a problem, due to the difficult combination of a virtual image with the real structure. ImplaNavTM (BresMedical, Sydney, 2015), an advanced system manufactured for oral and maxillofacial surgery navigation , builds a three-dimensional preview of the patientu2019s anatomy, giving the surgeon the tools to plan the procedures with accuracy before operating and guiding his hands while placing implants as previously planned. Aim:The aim of this in-vivo study is to evaluate the accuracy of the ImplaNavTM system.Materials and Methods:Eight patients and ten implants were evaluated in these Clinical Trials approved by the Ethical Committee. Six patients were partially and two were totally edentulous. According to the navigation system protocol a teeth-supported or implant supported intra-oral reference tool was positioned before a Cone Beam CT scan. The ImplaNavTM software was used for the diagnosis and the virtual planning of maxillary and mandibular fixtures. The surgery was tracked by way of following the planning, in real-time, until the final implant position. A post-operative Cone Beam CT scan was undertaken within three months of the surgery. The planned implants position and the real position of the implants obtained from the post-operative scan were aligned using a dedicated software for spatial registration. The deviation between the real position of the implants and the planned one was measured. Results:A maximum 3D spatial deviation between the inserted fixtures and the virtual ones of 0.77 mm was measured at the entry point reporting a standard deviation of 0.28 mm. The deviation recorded at the apex was 0.94 mm with a standard deviation of 0.09 mm. The mean angular deviation between the axes resulted in a 3.98 degrees angle. Conclusions and Clinical Application:The ImplaNavTM system seems to be useful for the clinical practice. The accuracy reported in this study results comparable with the literature data found for dynamic navigation and drill guides systems. The simplified procedure does not result in additional costs for each treated patient and the possibility to work with a direct view of the operative field could introduce some significant advantages. The use of this system could be particularly suitable for advanced implantology like tilted or zygomatic implants. More clinical trials with a larger sample size are required to compare this system with others in the different clinical situations.

Minimal risk during surgical and healing phases is also of high importance. One of the most promising technique hasbeen introduced recently. This technique is dedicated to minimazing invasiveness associated with surgical interventions duringimplant placement. Aim/Hypothesis: The purpose of this study was clinical and radiological assessment of the healing process in augmented sites of the patients treated with dental implants inserted with the use of dynamic implant navigation system. Materials and Methods: The group of 4 healthy patients with bone augmented sites were estimated by the CBCT examination. The workflow required the useof optical markers attached to the patientu2019s arch during CBCT examination. The DICOM datas was loaded into the navigation systemcomputer. The implants were inserted according to the pre surgical planning under traditional local anesthesia. The flapless surgerywith the use of punch tissue technique was performed. After the period of 3 to 6 months the implants were uncovered and the prosthetic treatment was introduced. In all cases CBCT was performed after implantation as well as implant stability measured by (OSSTELL). Results: 1. In all cases the assessment revealed very good effectiveness of the treatment according to the pre surgical treatment planning.2. The healing process was uneventful in each patients due to the minimal invasiveness of the surgery.3. Implant placement was performed after complete bone regeneration and confirmed clinical efficacy of this bone reconstructiontechnique and implant stability achieved after insertion and secondary stability as well. Conclusions: The use of dynamic navigation and performance of flapless implant surgery reduced harm to patients leading to reduced patient discomfort, risk of infection and faster recovery.

Book excerpt: Aim: To compare early implant failure, template-related complications, and virtual planning accuracy of computer-assisted template-based implant placement using conventional impression and scan model or digital impression using conventional impression and scan model or digital impression to rehabilitate partial edentulous patients using flapless or mini-flap procedures and immediate loading.u2028Materials and methods: Any partially edentulous patients requiring at least 1 implant to be plan on three-dimensional (3D) cone beam computed tomography (CBCT) scans using a dedicated software were enrolled in the trial. Patients were randomised according to a parallel group design into two arms: intraoral digital impression (fully digital group) versus conventional impression and scan model (control group). Implants were to be placed flapless and loaded immediately, if inserted with a torque over 35 Ncm, with reinforced provisional prostheses. Three deviation parameters (horizontal, vertical, and angular) were defined and calculated between the planned and placed implant positions and analyzed statistically. Results were compared using a mixed-model repeated-measures analysis of variance (u03b1=0.05).Results: Twelve patients were randomised to the fully digital group (6 patients with 17 implants) and control group (6 patients with 20 implants). The mean error in angle was 2.56u00b11.52u00b0 (range 0.3u20135.0u00b0) in the fully digital group and 2.18u00b11.41u00b0 (range 0.3u20135.8u00b0) in the control group (P=0.519); in the horizontal plan (mesio-distal), the mean error was 0.57u00b10.32 mm (range 0.1u20131.1 mm) in the fully digital group and 0.43u00b10.26 mm (range 0.1u20130.9 mm) in the control group (P=0.249); in the vertical plan (apico-coronal), the mean error was 0.67u00b10.51 mm (range 0.0u20131.6 mm) in the fully digital group and 0.43u00b10.32 mm (range 0.0u20131.2 mm) in the control group (P=0.180).Conclusions: With the limitation of the present randomized controlled trial, intraoral digital impression may be a viable option to conventional impression and scan model for the rehabilitation of partial edentulous patients by using computer-guided template-assisted implant placement.
