Le score ISAR est un des outils préconisé. Objectif : Déterminer si les personnes âgées de plus de 75 ans fragiles selon le score ISAR sont évaluées par l’équipe mobile de gériatrie au service d’accueil des urgences du centre hospitaliser de Roubaix. Méthode : Il s’agit d’une étude prospective observationnelle descriptive et analytique monocentrique. Une analyse descriptive de la population a été réalisée. La probabilité conditionnelle d’avoir un avis auprès de l’EMG avec un score ISAR supérieur ou égal à 2 a été calculée. La concordance entre le nombre d’avis EMG et le score ISAR a été évaluée grâce au coefficient kappa. Les facteurs prédictifs d’avoir ou de ne pas avoir d’avis auprès de l’EMG ont été recherchés en construisant un arbre de classification et de régression. Résultats : 100 patients ont été inclus. 33% des personnes âgées de plus de 75 ans étaient évaluées par l’EMG aux urgences de Roubaix alors que 86% des personnes âgées étaient fragiles selon le score ISAR. La probabilité conditionnelle d’avoir un avis EMG aux urgences pour un score ISAR supérieur ou égal à 2 était de 38,5%. La concordance entre le nombre d’avis auprès de l’EMG et le score ISAR était au mieux médiocre pour un score ISAR supérieur ou égal à 4. Les facteurs prédictifs d’avoir un avis auprès de l’EMG étaient un temps passé aux urgences supérieur à 11,15h et un âge supérieur à 83,5 ans. Conclusion : Pour optimiser la prise en charge des personnes âgées fragiles aux urgences de Roubaix, le score ISAR pourrait être réalisé pour chaque patient âgé de plus de 75 ans. Le but de la réalisation de ce score serait une sensibilisation des urgentistes à la fragilité du sujet âgé afin de solliciter plus souvent l’EMG.

Nous avons évalué si la prise en charge que nous proposions aux patients précaires était globale : médicale et sociale (conformément à la législation) ainsi que le ressenti des soignants. Méthode : Il s’agit d’une étude prospective descriptive monocentrique à base de questionnaire menée sur 5 mois. L’inclusion était réalisée par l’infirmier(e) d’accueil et d’orientation sur la base de critères préétablis. Le questionnaire était complété par le médecin +/- l’assistante sociale. Les résultats étaient comparés aux indicateurs de la population générale et à un travail antérieur sur le même lieu pour juger de l’évolution de certains paramètres. Un test de χ2 était réalisé. Un second questionnaire évaluait le ressenti des soignants face aux patients en situation de précarité. Résultats : 184 patients ayant réalisé 472 passages ont été inclus. Les patients précaires sont majoritairement des hommes (66%) d’âge moyen de 48,2 ans, avec un faible niveau scolaire, sans emploi, bénéficiant des minima sociaux, avec une problématique de logement (49%), présentant une dépendance à l’alcool (71,7%) et une pathologie psychiatrique (50%). Book excerpt: Contexte : L'échographie clinique en médecine d'urgence pré-hospitalière est une pratique récente encore peu étudiée. Le service des urgences de l'hôpital de Roubaix possède un appareil d'échographie mais son SMUR n'en est pas équipé. Notre étude a pour objectif d'évaluer les besoins des médecins urgentistes roubaisiens en échographie pré-hospitalière. Méthode : Nous avons réalisé une étude observationnelle, prospective, monocentrique au centre hospitalier de Roubaix du 09/03/2016 au 20/06/2016. Les médecins urgentistes répondaient à un questionnaire après chaque intervention du SMUR sur l'utilité potentielle de l'échographie au cours de celle-ci. Les patients pour lesquels l'échographie semblait utile étaient suivis pour évaluer l'influence que cette pratique pourrait avoir sur leur prise en charge. Résultats : 264 patients ont été inclus. Le taux de réponse au questionnaire était de 40,5%. Dans 25% des cas, le médecin jugeait la pratique d'une échographie utile au cours de son intervention : à visée diagnostique dans 90,77% des cas, thérapeutique dans 40% des cas et pour une meilleure orientation dans 20% des cas. Les motifs éligibles à cette pratique étaient cardio-respiratoires dans 71% des cas et traumatologiques dans 12% des cas. Le temps de prise en charge intra-hospitalier aurait pu être diminué grâce à l'échographie dans 43% des cas selon le médecin. Parmi les 18 médecins ayant participé à l'étude, 55,55% avaient une formation universitaire en échographie clinique (DIU ou formation intégrée au DESC de médecine d'urgence). Nous avons remarqué que le niveau de formation n'influençait pas leur réponse au questionnaire (p=0,8679). Conclusion : La nécessité de l'échographie en pré-hospitalier est avérée dans un quart des cas selon les médecins urgentistes de Roubaix. La pratique échographique pourrait, lorsqu'elle est jugée utile, augmenter la certitude diagnostique dans plus de 90% des cas.

Book excerpt: Contexte : La question de l’intensité des soins à délivrer occupe de plus en plus le quotidien des médecins, y compris aux urgences. Afin d’encadrer les pratiques de limitation des soins, la SFMU met à jour en 2018 des recommandations. L’objectif de notre travail est d’établir un état des lieux de ces pratiques aux urgences de Roubaix en vue de leurs améliorations. Méthode : Il s’agit d’une étude rétrospective sur 12 mois, monocentrique, observationnelle, descriptive des patients décédés aux urgences ou orientés dans les services des soins intensifs ou palliatifs, avec recueil des données concernant la qualité de constitution des procédures de limitation comparativement aux recommandations de la SFMU. Les caractéristiques démographiques et médicales ont été relevés. Le ressenti des urgentistes autour de ces prises en charge a été évalué avec un auto-questionnaire. Résultats : 1485 dossiers ont été analysés, 119 font mention d’une discussion autour d’une limitation des soins (8%). Nous avons relevé qu’une argumentation est présente dans 93,3% des cas. Cependant, la recherche de la volonté n’est faite que dans 60,5% des cas et cette recherche n’est directe que 18,7% du temps. La collégialité est respectée dans 63,5% mais n’inclue le médecin traitant que dans 1,2% des cas. Seulement 10,1% des procédures sont réalisées avec un protocole. L’âge moyen des patients concernés par une limitation des soins est de 78 ans. Ils présentent plus fréquemment des antécédents de maladie grave notamment néphrologique, neurologique et oncologique. Les urgentistes interrogés concernant ces prises en charge mettent en avant des difficultés à informer le patient sur la procédure et souhaiterait la mise en place d’un protocole non contraignant. Conclusion : Si la qualité des procédures de limitation est globalement respectée, des axes d’amélioration sont relevables : optimiser la recherche de la volonté du malade, renforcer les liens entre les urgentistes et les médecins traitants ou spécialistes, familiariser les urgentistes à l’utilisation d’un protocole. Utiliser de façon plus systématiques les outils informatiques à notre disposition comme le DMP afin d’optimiser le recueil des données pourrait être une solution envisageable.
