Book excerpt: Objectifs : Le but de notre étude était d'observer les patients se présentant de façon spontanée au sein de notre SU, c'est-à-dire sans y avoir été adressé par un médecin. L'objectif principal était d'évaluer le recours au SAU de ces patients afin de voir si leur motif de consultation était gérable en médecine de ville ou si le recours au SAU était justifié. Les objectifs secondaires étaient d'observer les heures de consultation, les motifs et le devenir de ces patients. Matériel et Méthodes : Il s'agit d'une étude épidémiologique, observationnelle, prospective et mono centrique s'étant déroulée sur toute l'année 2017 au Service d'Accueil des Urgences du CHR de Thionville. Chaque patient consultant de façon spontanée au SAU était inclus via le logiciel Résurgence selon les renseignant de l'IAO. Le médecin ayant pris en charge le patient confirmait ensuite l'inclusion et évaluait le recours. Résultats : 11798 patients ont pu être inclus dans cette étude soit 31% des patients ayant consulté au SU de Thionville en 2017. On y retrouvait un sexe ratio Homme-Femme à 1.18. La moyenne d'âge était de 41.6 ans avec un écart type de 12 ans. La majorité des patients (55.9%) consultaient pour un motif médical, 41.6% d'entre eux consultaient pour un motif traumatologique et 2.5% consultaient pour motif psychiatrique. Parmi les patients de notre effectif, 50.2 % consultaient pour des motifs estimés comme gérables en médecine de ville. Enfin, 11% de notre effectif a été hospitalisé. Conclusion : Les patients s'étant présentés de façon spontanée au service d'accueil des urgences de Thionville auraient pu, dans la majorité des cas, avoir eu recours à leur médecin traitant ou à la permanence des soins. Cependant, nombre de ces patients ne relevaient pas d'une simple consultation et nécessitaient des soins parfois spécialisés ainsi qu'une hospitalisation pour un nombre non négligeable d'entre eux. Il est donc nécessaire de porter attention à chaque patient quel que soit son motif de consultation. Par ailleurs, une amélioration du système de permanence des soins de santé ambulatoire permettrait de réduire le nombre de consultations au sein des SU et de réduire les temps de passage.

Le service des urgences du CHD de La Roche-sur-Yon n'est pas épargné par ce constat, dans un département dont la démographie médicale est en recul. Malgré la présence d'une permanence des soins ambulatoires sur le territoire vendéen, nous constatons une hausse du nombre de patients consultant sans avis médical préalable. Nous avons voulu identifier leurs motivations et analyser la pertinence de ces consultations. Matériel et Méthode : étude prospective monocentrique durant une semaine basée sur un questionnaire standardisé distribué aux patients majeurs consultant, sans avis médical préalable, le service d'accueil des urgences du CHD de La Roche-sur-Yon. Un questionnaire à l'attention des médecins prenant en charge le patient est également distribué. Résultats : l'étude repose sur l'analyse de 248 questionnaires. Les consultations étaient appropriées dans 45,2 % des cas avec seulement 13,7 % de patients hospitalisés. Près d'un quart des patients n'ont pas eu d'examen complémentaire. Les motifs de consultation relevaient majoritairement de la prise en charge de symptômes douloureux et du besoin supposé d'examens. Les patients ont déclaré être suivi par un médecin généraliste dans 82,7 % des cas mais 17,3 % d'entre eux n'ont pas réussi à le joindre. Discussion : la faible proportion d'hospitalisations, le nombre de patients n'ayant pas eu d'examen complémentaire et la part d'examens prescrits pouvant être réalisés en ville rendent les consultations spontanées aux urgences inappropriées. La motivation des patients à consulter aux urgences peut s'expliquer par la nécessité supposée d'examens et la présence d'un plateau technique facile d'accès. De plus, du fait de sa démographie actuelle, la médecine générale est indisponible pour la prise en charge des soins non programmés qui sont alors traités aux urgences, souvent après plus de 12h d'évolution. Les délais de prise en charge des patients, ne nécessitant pas d'examen complémentaire, sont plus importants aux urgences comparés à examen en médecine de ville. Conclusion : la promotion de la permanence des soins ambulatoires auprès de la population et le développement de structures pouvant prendre en charge les soins non programmés en journée nous semble pertinent pour délester les services d'urgences.

L'objectif de cette étude était de décrire le parcours de soins et le devenir des patients pris en charge aux urgences afin de proposer des solutions pour que perdure notre système de soins. Méthode : Nous avons réalisé une étude observationnelle, prospective, monocentrique aux urgences du centre hospitalier de Brive la Gaillarde entre le 1 et le 7 octobre 2021. Résultats : Les données ont été analysées pour 71% des personnes ayant consulté. La classe d'âge la plus représentée concernait les moins de 30 ans. La majorité des patients sont venus par leurs propres moyens, sans avis médical préalable pour une urgence ressentie. 80% étaient suivis par un médecin traitant. Moins de 15% des consultations spontanées faisaient suite à l'indisponibilité du médecin généraliste. Les patients ont consulté plutôt pendant les horaires d'ouverture des cabinets médicaux entre 8h et 20h. 70% ont bénéficié d'examens complémentaires et 30% ont été hospitalises. 80% des hospitalisés ont été adressés par le 15 ou leur médecin traitant. Conclusion : Le renforcement de l'éducation des patients concernant le parcours de soins coordonnés et la mise en place d'une organisation spécifique dédiée à la prise en charge des soins non programmés font partie des solutions à envisager.
