The volume also examines the specific segments of the WIC population at risk for each criterion, identifies gaps in the scientific knowledge base, formulates recommendations regarding appropriate criteria, and where applicable, recommends values for determining who is at risk for each criterion. Recommendations for program action and research are made to strengthen the validity of nutrition risk criteria used in the WIC program.

Book excerpt: The mission of the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is to safeguard the health of low-income women, infants, and children through age 4 who are at nutritional risk. WIC provides nutritious foods to supplement diets, nutrition education, and referrals to health care and other social services. Almost half of all infants and about a quarter of all children ages 1-4 in the U.S. participate in the program. WIC accounts for 10% of total Federal spending on food and nutrition assistance. This report describes the WIC program ¿ how it works, its history, program trends, and the characteristics of the population it serves. It also examines current issues facing WIC, focusing mainly on those with important economic implications.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) began 40 years ago as a pilot program and has since grown to serve over 8 million pregnant women, and mothers of and their infants and young children. Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. In this third report, the committee provides its final analyses, recommendations, and the supporting rationale.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (the WIC program) has promoted the health of low-income families for more than 30 years by providing nutrition education, supplemental food, and other valuable services. The program reaches millions of families every year, is one of the largest nutrition programs in the United States, and is an important investment in the nation's health. The U.S. Department of Agriculture charged the Institute of Medicine with creating a committee to evaluate the WIC food packages (the list of specific foods WIC participants obtain each month). The goal of the study was to improve the quality of the diet of WIC participants while also promoting a healthy body weight that will reduce the risk of chronic diseases. The committee concluded that it is time for a change in the WIC food packages and the book provides details on the proposed new food packages, summarizes how the proposed packages differ from current packages, and discusses the rationale for the proposed packages.

Book excerpt: Started in 1974, the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) was designed to meet the special nutritional needs of low-income pregnant, breastfeeding, or postpartum women; infants; and children up to 5 years of age who have at least one nutritional risk factor. The WIC Program provides three main benefits: supplemental foods, nutrition education, and referrals to health and social services. Since the inception of the WIC program, substantial changes in size and demographics of the population, food supply and dietary patterns, and health concerns have made it necessary to review the WIC food packages. Proposed Criteria for Selecting the WIC Food Packages proposes priority nutrients and general nutrition recommendations for the WIC program, and recommends specific changes to the WIC packages.

Book excerpt: Dietary Risk Assessment in the WIC Program reviews methods used to determine dietary risk based on failure to meet Dietary Guidelines for applicants to the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC). Applicants to the WIC program must be at nutritional risk to be eligible for program benefits. Although "dietary risk" is only one of five nutrition risk categories, it is the category most commonly reported among WIC applicants. This book documents that nearly all low-income women in the childbearing years and children 2 years and over are at risk because their diets fail to meet the recommended numbers of servings of the food guide pyramid. The committee recommends that all women and children (ages 2-4 years) who meet the eligibility requirements based on income, categorical and residency status also be presumed to meet the requirement of nutrition risk. By presuming that all who meet the categorical and income eligibility requirements are at dietary risk, WIC retains its potential for preventing and correcting nutrition-related problems while avoiding serious misclassification errors that could lead to denial of services for eligible individuals.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) provides nutritious foods and assistance to low-income pregnant and postpartum women, infants, and young children. Research has shown that WIC helps to improve birth and dietary outcomes and contain health care costs. The U.S. Dept. of Agriculture’s (USDA’s) Food and Nutrition Service (FNS) oversees the program, which is administered by state and local agencies. While federal regulations define criteria that must be used to determine applicants’ income eligibility for WIC, state and local agencies are also given some discretion. In addition, federal law allows families who participate in other assistance programs, such as Medicaid, to be automatically income-eligible for WIC. This report assessed: (1) How do state and local criteria for determining WIC income eligibility vary? (2) To what extent are individuals who would otherwise be ineligible for WIC deemed eligible due to their participation in other programs? (3) How does USDA assist and monitor state determination of WIC income eligibility? Tables and figure. This is a print on demand report.

Book excerpt: Abstract: This GAO report estimates the number of additional participants who might be added to the WIC program by utilizing the savings that might be achieved by states purchasing infant formula at less than retail cost. The question of whether or not states should be allowed to apply savings to cover increases in administrative costs instead of requiring states to apply the money to purchase additional food for clients is also discussed.

Book excerpt: The National School Lunch Program and the School Breakfast Program, administered by the Food and Nutrition Service of the U.S. Department of Agriculture (USDA), are key components of the nation's food security safety net, providing free or low-cost meals to millions of school-age children each day. Under the most commonly adopted provisions, USDA reimburses districts for meals served on the basis of data collected in a "base year," during which applications are taken. After 3 or 4 years, applications must be taken again to establish new base-year data, unless the district provides evidence that local conditions have not changed. A special provision that does not require applications to be taken every few years would reduce burden, be more attractive to school districts, and potentially increase student participation by expanding access to free meals. To support the development of such a provision, the Food and Nutrition Service asked the National Academies to study the technical and operational issues that arise in using data from the American Community Survey (ACS)-a new continuous survey replacing the long-form survey of the decennial census-to obtain estimates of students who are eligible for free and reduced-price meals for schools and school districts. Such estimates would be used to develop "claiming percentages" that, if sufficiently accurate, would determine federal reimbursements to districts for the schools that provide free meals to all students under a new special provision that eliminates the base-year requirements of current provisions.

Book excerpt: Federal agencies that administer means-tested programs are responsible for both ensuring that people have appropriate access to assistance & ensuring the integrity of the programs they oversee. Knowing the proportion of the population that qualifies for these programs relative to the numbers who actually participate can help ensure that agencies can monitor & communicate key info. on program access. This report provides info. on: the proportion of those eligible who are participating in 12 selected low-income programs; factors that influence participation in those programs; & strategies used by fed., state, & local admin. to improve both access & integrity & whether agencies monitor access by measuring participation rates.
