Book excerpt: This concise guide to the often overlooked association between erectile dysfunction and hypertension/cardiovascular disease covers a wide range of aspects of importance to the clinician. It examines the impact of antihypertensive drug therapy on erectile function and explains how the management of erectile dysfunction in hypertensive patients depends on a variety of factors. Different treatment approaches are described, including lifestyle modification, PDE-5 inhibitors and other novel agents and behavioral therapy and helpful therapeutic algorithms are presented. A further focus of the book is the potential role of erectile dysfunction as an early diagnostic indicator of asymptomatic coronary artery disease and a prognostic marker for cardiovascular events. In addition, key background information is supplied on epidemiology and pathophysiology, and the significance of erectile dysfunction in different patient groups, such as the elderly and those with chronic kidney disease, is examined. Erectile dysfunction is a major public health problem affecting more than ten percent of the general male population and is now considered to be predominantly of vascular origin. This book will be informative and of practical value for all practitioners responsible for caring for the very many patients who experience erectile dysfunction in the setting of hypertension and cardiovascular disease.

Ideal treatments are also covered, including Viagra, Levitra, Cialis, penile injections and implants, testosterone gels, intraurethral medications, vacuum pumps, and constriction rings.

The first focuses on the pathway from symptom to diagnosis and covers the full range of symptoms commonly encountered by the urologist. Any particular symptom can be located within a matter of seconds and in each case differential diagnosis is explained with the aid of a flow chart. The second part of the book is devoted to the pathway from diagnosis to therapy, with each short chapter devoted to a specific diagnosis. Basic information is provided on the disease in question, along with an outline of state of the art treatment options and possible complications. Again, flow charts are used to help the reader to reach the correct therapeutic decision in different circumstances. Numerous high-definition photographs and illustrations are also included. This book will be a great asset for urologists and practitioners in related disciplines.

This book was released on 2010 with total page 728 pages. Available in PDF, EPUB and Kindle. Book excerpt: This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Book excerpt: 510 males with ischemic heart disease were asked to fill out the abridged five-item version of the International Index of Erectile Function (IIEF-5. 90.4% (461) of the respondents reported some degree of erectile dysfunction(IIEF-5 score >22). 24.5% have mild ED; 26.7% have mild to moderate ED; 5.5% have moderate ED and 33.7% have severe ED. Age above 60 years old and diuretic are the most significant risk factors for ED by logistic regression when adjusted for all confounding factors. Other important risk factors include diabetes and hypertension. Erectile dysfunction is common among patients with established coronary heart disease. In this study, the prevalence is much higher than that of the normal population. The prevalence and severity of ED increased significantly in men above the age of 50 years old. Diabetes, hypertension and diuretic use are important risk factors for erectile dysfunction. Understanding the magnitude of the problem and taking remedial steps may significantly improve the quality of life of these affected individuals.
