Book excerpt: La fréquentation des services d'urgences en France est en constante augmentation et ce sont 15 millions de citoyens qui pourraient avoir recours à ces services en 2010. À partir de 1995 une nouvelle discipline médicale voit le jour : la médecine polyvalente d'urgence. Mais qui sont ces médecins "Urgentistes" qui l'exercent ? Afin de dresser le profil du médecin Urgentiste, nous avons réalisé une étude à l'échelon régional. En premier lieu chaque urgentiste recensé a été sollicité pour répondre à un questionnaire envoyé par voie postale. Type et mode d'exercice, statuts, formation initiale, conditions d'exercice, reconnaissance de la profession sont les thèmes que nous avons abordés. Dans un deuxième temps, nous avons interrogé les chefs de service des spécialités les plus fréquemment sollicitées par les services d'urgences pour la prise en charge conjointe des patients (réanimateurs, cardiologues, pédiatres et obstétriciens) afin de déterminer leur perception de la profession d'Urgentiste. Enfin, nous proposons, dans un but de documentation, une présentation de deux modèles de médecine d'urgence étrangers : la médecine d'urgence allemande et la médecine d'urgence américaine. Notre étude a permis de mettre en exergue les caractéristiques suivantes : formation de base jugée insuffisante, statuts précaires ne permettant pas d'envisager l'exercice de la médecine d'urgence à long terme, hétérogénéité des conditions de travail, manque de reconnaissance. À l'heure où les Tutelles consentent un effort en terme de nombre de postes proposés, les Urgentistes réclament la professionnalisation de leur exercice avec la mise en place d'une véritable formation, de plans de carrière précis ainsi qu'une réflexion transversale pluridisciplinaire permettant une gestion cohérente des flux de population vers les services d'urgences.

Le plan blanc est le dispositif mis en place à l'hôpital pour répondre à ces situations de crise. Notre étude consiste à évaluer la connaissance du personnel médical et des cadres de santé des urgences de Gironde sur le plan blanc. Méthodes : nous avons proposé une étude épidémiologique transversale, multicentrique. Les centres concernés étaient les centres hospitaliers de Gironde disposant d'un service d'urgence adultes. Tous les médecins et cadres de santé travaillant dans ces services étaient ciblés (hors SAMU/SMUR exclusifs). Un questionnaire leur a été distribué, comportant des questions sur l'organisation du plan blanc, les formations reçues, leur sentiment d'aptitude et leur désir de formations complémentaires. Le critère de jugement principal était le minimum de connaissance requis pour faire face à un plan blanc : savoir qui appeler et où trouver le document. Résultats : 88 sujets ont été inclus dans 11 centres. 42% des sujets interrogés détiennent le minimum de connaissances. L'objectif principal de cette étude était d'évaluer la prévalence des professionnels souffrant d'un équivalent de Syndrome post commotionnel défini par les critères de la CIM-10. L'objectif secondaire était d'évaluer la prévalence du Burn-out et du Trouble de stress post traumatique. Matériel et Méthode : il s'agit d'une étude observationnelle transversale monocentrique réalisée aux urgences adultes de l'hôpital Pellegrin du Centre Hospitalier Universitaire de Bordeaux sur la période du 9 au 20 septembre 2017. L'enquête a été réalisée avec des auto-questionnaires anonymes remis à l'ensemble du personnels travaillant aux urgences (médecins, infirmier(e)s diplômés d'état, aide-soignant(e)s, les brancardier(e)s et agents administratifs). Leurs participations étaient libres et bénévoles. Résultats : 129 personnes ont participé à notre enquête. L'âge moyen était de 34 ans. La prévalence des symptômes proche du SPC était de 39,1% chez nos professionnels de santé. Il y avait également 6% de Trouble de stress post traumatique et 54% de Burn-out. Une relation statistiquement significative entre le stress au travail, le niveau de bonheur, le TSPT, le Burn-out et les symptômes du syndrome post commotionnel est mis en évidence. Conclusion : le travail en médecine d'urgence semble avoir des répercussions sur l'ensemble du personnel. En effet, une si grande proportion de symptômes équivalents au syndrome post commotionnel doit nous alerter. Ces résultats suggèrent donc la nécessité d'une intervention au plus près du personnel afin de les prévenir.
