It is our vision that all of these enhancements and improvements will advance en route care across the services and the Department of Defense. The CCFP Program Office facilitates appropriate training and medical education to the CCFP providers. The CCFP program of instruction ensures the appropriate skills and knowledge required for CCFPs to apply these medical guidelines during aeromedical evacuation. Unit medical trainers and medical directors should evaluate CCFPs ability to follow and execute the medical instructions herein. These medical guidelines are intended to guide Critical Care Flight Paramedics (CCFP) and prehospital professionals in the response and management of emergencies and the care and treatment of patients in both garrison and theater of war environments. Unit medical providers are not expected to employ these guidelines blindly. Unit medical providers are expected to manipulate and adjust these guidelines to their unit’s mission and medical air crew training / experience. Medical directors or designated supervising physicians should endorse these guidelines as a baseline, appropriately adjust components as needed, and responsibly manage individual unit medical missions within the scope of practice of their Critical Care Flight Paramedics, Enroute Critical Care Nurses, and advanced practice aeromedical providers.

In 1970, Emergency Medical Service (EMS) systems were a new and much needed development in the national health care delivery system. A systems approach to field casualty care has been progressively improved during each successive military conflict since the Civil War. These improvements were ini tiated after the rnedil:al care and evacuation disaster experienced by the Union Army of the Potomac at Bull Run on July 21, 1861. During the Civil War, major changes in administration, professional personnel, transportation, hospitals, sanitation, and medical records established patterns that have been continually refined and improved. Stimulated by the pressing demands of war surgery and coupled with parallel advances in medical care over the last century, an almost unbelievable level of performance was realized in Vietnam. Advances in field resuscitation, efficiency of aeromedical transportation, and energetic treatment of military casualties have proved to be major fac tors in the decrease in death rates of battle casualties reaching facilities: from 8% in World War I to 4. 5% in World War II to 2. 5% in Korea and to less than 2% in Vietnam.

Aeromedical evacuation is an important capability that the Joint Force Commander must have in order to successfully conduct combat operations. This paper deals with the intratheater tactical transportation of wounded personnel via rotary-wing aircraft. The paper summarizes current joint patient movement doctrine from Joint Pub 4-02.2. It also explains the differences in the U.S. Military services' patient movement capability. The paper details three problems with current aeromedical evacuation doctrine. First, attrition of aircraft, due to combat, is not factored in doctrine. Second, because of their medically trained crews and onboard medical suites, dedicated air ambulance capabilities exceed air casualty evacuation in providing lifesaving transportation. Third, although the Army is assigned the responsibility for transfer of patients to afloat medical treatment facilities, this mission is often accomplished by Navy or Marine Corps rotary-wing casualty evacuation assets. The paper proposes a solution to these problems by establishing a standing Joint Intra-Theater Aeromedical Evacuation Task Force (JIAETF) to provide dedicated aeromedical evacuation to the Joint Force Commander.
