Previous cross-sectional studies have estimated a significant negative effect of spousal coverage on wives' labor supply. However, these estimates potentially suffer from bias due to the simultaneity of wives' labor supply and the health insurance status of their husbands. This paper attempts to obtain consistent estimates by using several panel data methods. In particular, the likely correlation between unobserved personal characteristics of husbands and wives-such as preferences for work-and potential joint job choice decisions can be controlled by using panel data on intact marriages. The findings, using data from the National Longitudinal Survey of Youth and the Current Population Survey, suggest that the negative effect of spousal coverage on labor supply found in cross-sections results mainly from spousal sorting and selection. Once unobserved heterogeneity is controlled for, a relatively smaller estimated effect of spousal coverage on wives' labor supply remains.

In particular, I investigate how husbands' health insurance coverage offers affect wives' decisions to enter the labor force and work full-time and how this has changed over time. I endeavor to correct for potential biases of these effects by 1) using an instrumental variables model to deal with endogeneity and 2) estimating and netting out likely unobserved heterogeneity biases, such as assortative mating or income effects. Using Current Population Survey data from 1995 to 2005, I find that husband's employer provided health insurance coverage has a negative effect on wife's labor supply that has increased (become more negative) over time.

First, we develop a theoretical model that examines the effects of employment-contingent health insurance on the labor supply response to a health shock, to clarify under what conditions employment-contingent health insurance is likely to dampen the labor supply response. Second, we empirically evaluate this relationship using primary data. The results from our analysis find that -- as the model suggests is likely -- health shocks decrease labor supply to a greater extent among women insured by their spouse's policy than among women with health insurance through their own employer. Employment-contingent health insurance appears to create incentives to remain working and to work at a greater intensity when faced with a serious illness.

Book excerpt: Employment-contingent health insurance creates incentives for ill workers to remain employed at a sufficient level (usually full-time) to maintain access to health insurance coverage. We study employed married women, newly diagnosed with breast cancer, comparing labor supply responses to breast cancer diagnoses between women dependent on their own employment for health insurance and women with access to health insurance through their spouse's employer. We find evidence that women more dependent on their own job for health insurance reduce their labor supply by less after a diagnosis of breast cancer - the estimate difference is about 5.5 to 7 percent. Women's subjective responses to questions about working more to maintain health insurance are consistent with the conclusions from observed behavior.

The availability of health insurance appears to provide distinct labor force exit and entry incentives for women; it can induce women with higher incomes to leave the labor force entirely, while encouraging those with low incomes to enter. Because different women appear to make very different decisions related to work based on variables such as income, a detailed study of these decisions is necessary for appropriate and effective government policy. Using data from the March 2001 Current Population Survey, this study finds that women of both low- and higher-incomes tend to participate in the labor force more when private insurance is available, but participate less when government health coverage is provided. This reduction in labor supply is likely due to government regulations concerning income allowances and benefits. Effects of health insurance availability on hours worked is inconclusive for both groups of women. As such, policy makers could expand both government and private health insurance coverage without adversely affecting employment.

Insurance is portable, but the cost may change when an individual changes jobs, again leading to the potential for job-lock. This paper assesses the empirical magnitude of health insurance-related impediments to job mobility in the United States and Germany, The results show little evidence that health insurance provision interferes with job mobility in either the United States or Germany, thus suggesting that these employer-based systems for providing the health insurance portion of the social safety net do not alter this aspect of labor market efficiency.

While most health insurance is privately provided to workers (by employers) in the United States, Spanish workers obtain health insurance coverage from the public social security system. The Spanish system is financed by a payroll (social security) tax shared between employers and employees. There is clear evidence, however, of widespread non-compliance with the social security tax. This paper empirically compares the incidence of health insurance coverage among U.S. workers to the pattern of compliance with the social security tax among Spanish workers. The main finding of this paper is that these two patterns are very similar. They both depend on the same supply and demand factors, which is consistent with basic economic models of private provision of benefits and of tax compliance. However, one important difference between the two systems is that in Spain, unlike the United States, essentially all heads of household work in the covered sector and thus have a full access to public health care for themselves and for their dependents.

Book excerpt: This dissertation consists of three empirical papers exploring policy-relevant questions in Public Finance and Labor Economics. In particular, it inquires into social insurance programs, the purpose of which is to protect individuals against adverse events. More generous benefits, however, often lead to unintended behavioral responses even as they provide greater protection. This dissertation focuses on identifying and quantifying the distortionary effects of social insurance, providing an input to optimal design. The first chapter investigates the effect of Social Security dependent benefit provisions on the labor force participation of married women aged 25-54. Many provisions of the Social Security program may distort an individual's work incentive. In particular, the availability of dependent benefits may reduce the net return to work since secondary earners, who are likely to claim benefits based on their spouses' earnings records, pay the full payroll tax without receiving marginal benefits for additional earnings. I rely on differences in Social Security coverage among husbands by state and sector to identify the impact on the labor supply of their wives. The results show that married women tend to reduce their labor supply when dependent benefits are available, suggesting that changes in the Social Security system that strengthen the relationship between earnings and benefits would have a positive effect on the labor supply of married women. The second chapter analyzes how Social Security dependent benefit provisions affect women's divorce behavior. Under the current Social Security system, a divorced woman is eligible to receive dependent benefits based on her ex-spouse's earnings record if her marriage lasted at least 10 years and she remains unmarried after divorce. Using data from the Panel Study of Income Dynamics, I estimate a discrete time duration model of the probability of divorce. The results suggest that married women are likely to delay divorce to preserve the option of receiving dependent benefits if their marriages are near 10 years duration. This effect is stronger for women whose earnings are much lower than their husbands' or whose predicted remarriage probabilities are low, so are those most likely to value the option. The final chapter examines the effect of extended parental coverage on young adults' labor market choices. Young adults aged 19-29 are significantly less likely to have health insurance since most family insurance policies cut off dependents when they turn 19 or finish college. In recent years, several states have expanded eligibility to allow young adults as old as 30 to remain covered under their parents' employer-provided health insurance. For those who qualify for these benefits, the expansion of parental coverage partially reduces the value of being employed by a firm that provides health insurance since adult children can now get health insurance through another channel. We employ quasi-experimental variation in the timing and generosity of states' eligibility rules to identify the effect of the policy change on young adults' labor market choices. Our results suggest that the expansion of parental coverage increases the group coverage rate and reduces labor supply among young adults, particularly in full-time employment.
