Book excerpt: Introduction: L'évaluation de la complexité du patient dans toutes ses dimensions bio-psycho-sociales est au coeur de la pratique des soins palliatifs. Il n'existe pas d'échelle validée permettant d'objectiver cette complexité. Une étude menée par Luc Chevalier a étudié la fidélité inter-juges d'une échelle de gradation de la complexité du patient en soins palliatifs, créée en 2012 à partir de la grille GEMAU. Les résultats retrouvaient une qualité d'accord inter-juges médiocre pour 5 des 10 items de l'échelle et un accord de qualité modérée pour 2 des 10 items. Une deuxième étude menée par Charlotte Decherf-Crombé a cherché à identifier ces discordances, via une étude qualitative prospective d'observation passive. Les résultats ont révélé plusieurs facteurs de discordance. De ces résultats, une nouvelle échelle a été élaborée. Matériel et méthode: Il s'agit de la poursuite du processus d'élaboration de cette nouvelle échelle, par l'obtention d'un consensus d'experts selon une méthode Delphi. Cette étude a été effectuée auprès de 11 médecins travaillant dans 4 USP de la région Nord-Pas-de-Calais. Il leur a été demandé de noter sur une échelle de Likert de 1 à 4, la pertinence des 9 items de l'échelle. Les items retenus devaient obtenir une moyenne supérieure à 3. Résultats: 9 des 11 experts ont participé à cette étude. 7 items ont été validés au premier tour. L'item 5 « troubles neuropsychiatriques» et l'item 6 « questionnement sur la prise en charge » ont dû être reformulés. Deux tours ont permis l'obtention d'un consensus. Conclusion: Cette étude a permis une approbation de l'échelle de gradation de la complexité du patient en soins palliatifs via une méthodologie Delphi. Le recrutement d'experts ayant participé au 2 précédentes études, a garanti une bonne pertinence dans le processus de validation de cet outil. Une étude de fidélité inter-juges devra être réalisée sur cette nouvelle échelle.
