Available in PDF, EPUB and Kindle. Book excerpt: In this groundbreaking volume, the authors bring us into the immediacy of the analyst's consulting room in direct confrontation with the thought disorder, delusions and hallucinations of their patients grappling with psychosis. From the early days of psychoanalysis when Freud explicated the famous Schreber case, analysts of all persuasions have brought a variety of theories to bear on the problem of schizophrenia and the other psychoses. Here, as William Butler Yeats notes, "the centre cannot hold" and any sense of self-esteem - positive feelings about oneself, a continuous sense of self in time and a functional coherence and cohesion of self - is shattered or stands in imminent danger. What makes psychoanalytic self psychology so compelling as a framework for understanding psychosis is how it links together the early recognition of narcissistic impairment in these disorders to the "experience-near" focus which is the hallmark of self psychology.

Book excerpt: The goal of psychotherapy as formulated in this revision of a classic text is to improve ego function of severely disturbed patients who are often hospitalized. This book shows why and how. It describes the psychotherapeutic techniques that aid patients to understand the meaning of the psychotic symbols so that they can experience reality and their emotions as separate entities. Medication effects and the neurobiology of psychotic and near psychotic patients are explained and evaluated in terms of specific ego dysfunction so that psychopharmacology may be targeted. With the first edition originally a recipient of the prestigious Heinz Hartmann Award, this valuable resource is a go-to guide for clinicians who treat patients suffering from crippling mental disorders.

Book excerpt: Psychosis and Near Psychosis offers a psychoanalytically-based approach to an integrated treatment of psychosis and near psychosis, achieved by organizing psychotherapy, medication, hospital and milieu interventions into a powerful therapeutic tool. The author navigates confidently between psychiatric and psychoanalytic approaches, between biological evidence and psychological assessments. According to Dr. Eric Marcus, since the past, so-called heroic psychoanalyses with psychotic patients have clearly been shown to fail, the time is now ripe again to discuss psychosis in terms of the broadened psychoanalytic theory, with the support of medication and a better understanding of the neuropsychological factors involved. This book, which maps out mental illness in concrete and innovative ways, will interest all researchers and clinicians eager to find the best means, both practical and theoretical, to initiate satisfying psychiatric therapies.

Available in PDF, EPUB and Kindle. Book excerpt: Ego Development and Psychopathology presents an original theoretical underpinning for classification and interpretation of the major mental disorders, one based largely on the author's clinical experience. Ausubel's central thesis is that the most significant and crucial factors predisposing persons to mental disorders are critical developmental defects that arise at vital transitional phases of ego development. He sees both normal and pathological personality functioning as most cogently explained in terms of an individual's ego structure at a given point in the life cycle. Ausubel relates his developmental theory to the phenomenology and related clinical problems of psychiatric diagnosis. He classifies mental disorders in terms of their developmental history. Such factors, in his opinion, offer the most precise delineation of etiological, functional, and phenomenological similarities and differences among the various psychiatric syndromes. He provides an overview of ego development, as well as major variants of the norm. He also discusses development of conscience and moral values, as well as psychopathological considerations that follow from deficiencies, defects, failure, and distortions in ego development. He examines at length classification of mental disorders, such as anxiety states, psychotic depression and mania, schizophrenia, autism, antisocial and narcissistic personality disorders, and defense mechanisms. Ausubel is careful to point out that ego development is not the only significant determinant of normal and aberrant personality. Genetic predispositions, situational stress, and sociocultural factors must always be taken into consideration since mental disorder is always a product of multiple causality. However, he believes ego development is by far the most critical factor, and hence offers the most for classification of mental illness. This intriguing study will be of interest to professionals as well as educated and concerned practitioners in the fields of psychology, psychiatry, psychoanalysis, child psychotherapy, and social work.

Available in PDF, EPUB and Kindle. Book excerpt: This best seller breaks new ground by demonstrating how mainstream psychoanalytic theory has evolved into a psychoanalytic developmental object relations theory. The authors summarize the contributions of the major ego psychologists from Freud to the contemporary theorists, including the new findings of the child observationalists. By integrating these several contributions, the authors are able to show that classical psychoanalysis and ego psychology combine to form a unified theory that provides a more profound understanding of the borderline and narcissistic conditions as well as of the neuroses. From this comprehensive synthesis of theory the authors derive techniques for the treatment of the more troubled patient and expand the classical technique for the psychoanalysis of neurosis. Recognizing that there are preoedipal developmental problems even in neurotic patients and that there are oedipal issues in the borderline and narcissistic patients, the authors demonstrate the applicability of developmental object realtions theory across the diagnostic spectrum and include a new chapter on termination. Using numerous case illustrations from their own practice, the Blancks have constructed an extraordinarily comprehensive and readable text that updates psychoanalytic theory for the nineties.

Available in PDF, EPUB and Kindle. Book excerpt: In Ego Psychology II, Gertrude and Rubin Blanck elaborate upon ego psychological theory, extending and broadening it into a psychoanalytic developmental psychology. They present the unifying proposal, derived from Freud's concept of an overall ego (the Gesamt Ich), that the ego is the organizing process itself. Out of this basic proposition, a holistic conception of psychological development evolves. Within the developmental framework established in Ego Psychology II symptom constellation is shown to be unreliable as a guide to diagnosis. A diagram of development is presented to convey that overall development rather than symptomatology provides guidelines for secure diagnosis and suggests how treatment is to be carried out. Treatment, in the form of ego-building techniques, evolves from recognition that developmental inadequacies cause pathological formations that become malformations in the structure. Ego Psychology II is valuable for psychotherapists, psychologists, psychoanalysts and social workers: the authors' extensive case-study material illustrates the theroy and technique of developmental psychology in vivid form. The authors show also how psychoanalytic developmental psychology updates drive theory, sheds new light on transference, redefines resistance and defense in the poorly structured personalities, clarifies the pathology of the borderline conditions of narcissism, and suggests reconsideration of the manner in which many neurotic formations are attained.

Book excerpt: Psychoanalysis of the Psychoses brings together a distinguished international set of contributors, offering a range of views and approaches, to explore the latest thinking in the psychoanalytic treatment of psychosis and related disorders. Drawing on findings from neuroscience, theory and clinical material from many schools of psychoanalytic thought, this book offers a comprehensive guide to understanding how psychosis is conceptualised from a psychoanalytic perspective. It looks at how to work with psychotic patients, typical problems in treating psychosis and the role of pharmacology. It demonstrates the relational dimension, capable of strengthening the patient’s observing Ego and facilitating the integration of the different areas of the personality. This process can identify and work through the main psychological stress factors involved in psychotic disturbances, transforming chaotic thoughts into springboards for important insights, and offering patients the precious chance to construct for the first time a creative relationship with their own existence. Psychoanalysis of the Psychoses will be of great interest to psychoanalysts and psychoanalytic psychotherapists as well as psychiatrists wishing to draw upon psychoanalytic ideas in their work.
