Book excerpt: The health care industry in the United States is undergoing rapid changes to meet the unique needs of today09́s population, while staying continually mindful of escalating costs associated with the delivery of care. In order to ensure sustainability through maximum reimbursement, meanwhile maintaining customer bases, health care entities are focusing more on the quality of the care delivered and achieving the best possible outcomes for patients receiving that care. Electronic clinical decision support (CDS) is one way many organizations are ensuring statistically proven standards of care are used to realize improved patient outcomes (Calloway, S., Akilo, H. A., and Bierman, K., 2013). CDS uses various electronic functions built into the electronic health record (EHR) to guide providers in the care of patients, including prescribing medication, ordering electronically, and checking drugs and allergies for any interactions. Studies have revealed health care entities using CDS have lower rates of patient mortality, improved patient outcomes, and reduced costs associated with patient care (Agrawal, 2009). After receiving approval from the appropriate authorities, a project plan to build and install CDS within the EHR is developed. Following the education of clinical staff on the use of CDS, the implementation of the project is initiated for widespread use among providers in the acute care setting. The effect on benchmarks such as quality of care and patient outcomes are measured through statistics already recorded by the organization. Patient education will take place through flyers given to each patient at the point of admission, and staff education will occur in a classroom setting. For patients in the acute care setting, this project ultimately seeks to answer the following: is the use of CDS, over not using CDS, associated with improved patient outcomes by the time of discharge?

For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.
