ICC abutments lacking structures for vertical stop might sink axially into the inner surface of the implant. Axial displacement is caused by factors u2014 machining tolerance and the settling and wedge effects. Machining tolerance and the settling effect are inevitable phenomena in both external and ICC implants. However, the wedge effect, which induces concentrated axial compressive force in the direction of abutment sinking, is observed only in ICC implants. Although axial displacement in screw-type prostheses has been reported previously, no information is available regarding axial displacement in cement-retained prostheses. Differences in implantu2013abutment connection type have been known to result in variations in axial displacement in various clinical situations. Three typical implantu2013abutment connectionsu2014the external-butt (EXT), internal-conical (INT), and internal-butt (INT-Butt) connectionsu2014have been known to provide satisfactory results in clinical practice.Aim/Hypothesis:The purpose of this study was to quantitatively evaluate axial displacement in three different implantu2013abutment connections in cement-retained monolithic zirconia and polymethylmethacrylate (PMMA) provisional prostheses with the computer aided designu2013computer aided manufacturing (CAD-CAM) custom abutments.Materials and Methods: Three types of implants with different connectionsu2014external and internal butt connection implants and an internal conical connection implantu2014were evaluated. In each implant group, implant replicas were embedded vertically and angulated at 15u00b0. Sixty titanium custom abutments were fabricated using a CAD-CAM system. The geometries and surface morphologies of custom and ready-madeabutments were comparatively evaluated by scanning electron microscopy. Micro-gaps in abutment connections were scanned byoptical microscopy during screw tightening, re-tightening, and cyclic loading. Zirconia and provisional acrylic prostheses were fabricated and evaluated for internal gap by an indirect silicone technique. Cemented prostheses on abutments were mounted on a universal testing machine and subjected to 250 N sine wave cyclic loads. Cumulative axial displacement was measured at loadingperiods of 3, 10, 100, and 1,000,000 and analyzed by repeated measures analysis of variance (ANOVA). Internal gap width and netchange in axial displacement were analyzed by multi-way ANOVA.Results: There were no significant differences in internal gap widths of prostheses according to implant or prosthesis type. However,measurement site and implant angulation were influencing factors on internal gap. Surface geometries and morphologies of custom abutments varied according to the implantu2013abutment connection. Machining tolerance control and precision fit were difficult to achieve with the custom CAD-CAM abutment. The internal and external conical connection implants exhibited the greatest and lowest axial displacements, respectively. Acrylic prostheses exhibited greater axial displacement than zirconia prostheses. The vertically positioned implant group exhibited greater axial displacement than the angulated implant group in the early cyclic loading periods. Axial displacement in the late cyclic loading period was relatively low.Conclusions and Clinical implications:Custom abutments for cement-retained prostheses exhibited greater axial displacement than screw-retained prostheses. Axial displacement mostly occurred in the early loading period and was self-limited. Axial displacement should be managed by provisional restoration, with consideration of implant connection type and angulation, abutment fabrication method, and occlusal force of the patient.

Book excerpt: A large number of studies described implant survive and their short and long-term success with 94.6% early success rates and 89.7% even after more than 10 years of function. The failure of implant supported rehabilitation was either mechanical or biological. Few studies have conducted follow-ups for more than 10 years with regard to specific fixture-abutment connection. The aim of this retrospective study is to evaluate the long-term reliability and the incidence of technical and biological complications on single crowns supported by: cement retained abutment (CRA) and screwed retained abutment (SRA). A total of 300 single implant supported crowns performed from 2004 to 2007 on 300 different patients aged between 40 and 75 years were analyzed. Patients were divided in 150 group A (SRA) and 150 group B (CRA) selecting by inclusion criteria.The research was so performed: periapical radiographs performed with bite block, where crestal bone resorption (RC) was measured. The values were classified in three categories: 2 mm, between 2 and 4 mm, and greater than 4 mm. Bleeding on probing (BOP) and probing depth (PD) were measured. A cut-off of 5mm was taken related to PD: 5 mm was considered a negative outcome. Prosthetic complications were recorded: abutment decementation, screw loosening and prosthetic fracture. The results were analyzed for statistical analysis. The data analysis showed a rate of 4% of implant failure during the 10 years follow up period. Therefore this data were not taken into consideration for complication analysis. Regarding biological aspect, results showed a positive BOP index at 84.2% of the sites under investigation. Specifically, SRA showed a BOP of 86,5% and CRA 81,4%. More over the probing depth (PD) >5mm on peri-implant soft tissues analysis demonstrated a rate of 20,9% for CRA and 13,8% for SRA. The crestal bone loss radiographic measurements demonstrated for the range of RC 2 mm, a value of 16% for SRA and 62% for CRA; RC 24 mm 70% for SRA and 31% for CRA and at the end RC 4mm revealed a 14% for SRA whilst 7 % for CRA. Regarding mechanical aspect of connection a total of 14,6% of complications occurred: 6,2% for SRA and 8,33% for CRA. Finally, about prosthetic aspects: 7,6% crown fracture for SRA and 2,78 %for CRA. The results from this 10-year retrospective study showed that the two methods have positive long-term follow-ups, although the complications encountered. RC was statistically greater in the SRA group. In this regard, the possibility of having a better coupling between parts in the CRA method encourages the clinical use of these in terms of lower bone resorption values and screw loosening.

Currently, the connection design between implant fixture and the abutment is in two forms: external hex type and internal conical type. The biomechanical information on the various implant systems and various implant placement option is not sufficient to determine reliable treatment plans and implant choices.This study aimed to compare the functional stresses generated in the internal conical joint implant, prosthesis and supporting bone in the posterior maxilla, using finite element method.Two internal joint type implant (u00d85 X 7mm, u00d85 X 13mm) with TS2 SA fixture design of Osstem (Seoul, Korea) were modeled by computer-aided design software (Solidwork 2018). The posterior maxillary bone and 3-unit all zirconia crowns were also simulated. Three different scenarios were modeled with various implant alignment (SI3; 3 unit fixed prosthesis on 3 short implant abutments, SI2; 3 unit fixed prosthesis on 2 short (7mm) implant abutments, AI2; 3 unit fixed prosthesis on 1 short implant abutment and 1 long (13mm) implant with an angled abutment). A load of 30 N was applied vertically and obliquely, respectively to each of 12 occlusal contact points of the crown.The maximum Mises stresses produced by oblique loading was observed near the connection interfaces of the abutment. The peak value was 430 MPa for SI3, 568 MPa for SI2 and 656 MPa for AI2 respectively. There was no significant difference in peak von Mises stress between models due to vertical loading. The maximum compressive stresses due to oblique load on the supporting bone adjacent to the implant were 145 MPa for SI3, 204 MPa for SI2 and 200 MPa for AI2, respectively.Internal joint type design of implant concentrated the stresses to the conical interface between fixture and abutment interface, demonstrating the wedge effect between them. Increasing the number of implants was the most effective contributing factor to reduce the stresses in the internal conical joint implant and supporting bone in our study models.

Of these aftermarket abutments this is wide variation, from stock to custom abutments, as well as material differences such as zirconia, titanium, or a combination of the two. This study was performed in two parts: the aim of Part I was to measure and compare different strains conferred to the bone around the implants for nine different abutments for cement-retained crowns on an implant with an internal hexagon platform. The aim of Part II was to measure and compare the force required to cause fracture of ten different abutments for cement-retained crowns on an implant with an internal hexagon platform. Material and Methods: Nine 4.1mm x 11.5mm Tapered Screw-Vent implants were placed into a 12" x 2" x 8mm resin block for strain measurements. Five abutment specimens of each of the nine different abutments (n = 44 total) were tried into one of the nine implants. Monolithic zirconia crowns were then fabricated for each of the nine different abutments and the crowns were cyclically loaded at thirty degrees two times at a frequency of 2Hz and strain was measured and recorded. Ten Tapered Screw-Vent Implants were then individually secured in a loading apparatus, and three abutment specimens of each of the ten different abutments (n = 30 total) were loaded at a 30 degree angle until fracture of the implant abutment complex was achieved. The strain to the resin block was determined using three-dimensional digital image correlation (3D DIC), an optical measurement technique. Commercial image correlation software (VIC 3D v7.0 , Correlated Solutions, Inc. Columbia, SC, USA) was used to analyze the strain around the implants. Data for maximal and minimal principle strains as well as load to fracture were compared using analysis of variance with a Tukey-Kramer post-hoc test (alpha = .05). Results: Strain measurements showed no significant difference between any of the abutments when looking at minimal (compression) principle strains (p>.05). For maximal (tensile) principle strains the Atlantis Zirconia abutment and the Zimmer Patient Specific Abutment showed the highest and second-highest strain around the implant, respectively, with the Atlantis Zirconia being significantly greater than all abutments excluding the Zimmer PSA, and the PSA being significantly greater than the AstraTech ZirDesign and Legacy Straight Contoured abutment in titanium as well as zirconia (p

Recently, new implant designs and surfaces have been introduced to decrease the risk of early failure of dental implantsThe purpose of this study was to evaluate the short-term clinical and radiographic efficacy of a novel implant design and to evaluate the relative importance of achieving primary stability at placement over a 36-months period. This retrospective study includes 64 partially or fully edentulous patients treated between June 2014 and September 2017. Outcome measures were: implant and prosthetic failures; peak insertion torque values ; biological and mechanical complications; marginal bone loss (MBL); sulcus bleeding index (SBI); and plaque score (PS). Clinical and radiographic checks were performed as routinely in the office at implant placement, clinical check-up before the permanent prosthesis at 3 months, and at the 6, 12, 24 and 36 months follow-up visits.A total of 119 (31 narrow platform 3,75 mm, 68 regular platform - 62 RP 4,3 and 6 RP 5mm- and 20 wide platform 5,5mm) NobelParallel Conical Connection implants were placed in both post-extraction sockets and healed sites. 28 implant were immediately loaded. The mean insertion torque was 46,09 Ncm. 108 out of 119 implants (90,75%) were inserted with a torque superior to 35 Ncm.Each patient received a single or multiple-unit prosthesis. After 36-month of follow-up, only one implant failed (1,19%) in a healed sitebefore prosthesis installation. No other biological or technical complications were observed during this short follow-up. The mean MBLat baseline was +1,23 mm The mean MBL from insertion was -0,51 mm , 5,04% of the implants (n=6) showed bone loss of more than1 mm. All implants showed no bleeding on probing and plaque accumulation was very low (4,20%).The NobelParallel Conical Connection implant can be considered as an effective treatment option for implant placement in the partially or fully edentulous patients over a 36-months period in both jaws. This implant provides high but controlled insertion torques in all bone qualities and appear to be less technique-sensitive than other implant systems because of its parallel apically tapered design and its simple, straight forward and versatile drilling protocol.

Mean periimplant bone resorption at 12 months of healing was 1.5 mm (SD 1.00) (-1.55mm external and -1.59 mm internal hexagon).At 12 months of follow-up, the mean values of PI, BOP and PD were 2, 0.8 and 2.24 mm respectively for external hexagon implants and 2, 1 and 2.1 mm respectively for internal hexagon implants. 3 screw loosed abutments were identified in the external and 2 in the internal connection implants. The statistical analysis did not reveal differences in bone resorption between external hexagon implants and internal hexagon connections nor for periimplant health parameters (PI, BOP and PD).

AimTo present 3-year follow-up results from a prospective singlecenter study evaluating the clinical outcome of taperedimplants with conical connection when used in one-stageguided bone regeneration (GBR) procedures at mediumhorizontal bone defects (u22654 to u22646 mm of residual horizontalbone width).Material and MethodsThis study was designed as a single-cohort prospectivesingle center clinical trial. Partially edentulous patients,having u22654 to u22646 mm of residual horizontal bone width, wereselected and treated with simultaneous implant installationand GBR. Implants (NobelReplace Tapered CC PMC, NobelBiocare) were inserted using a GBR sandwich approach: 2mm of autologous bone, and 2 mm of anorganic bovinebone were placed in layers around the implant and coveredusing a resorbable collagen membrane fixed with titaniumpins. Follow-up visits were performed at 15 days, 1-, 3- and6-months after implant insertion, with definitive prosthesisdelivery at 12 months after implant installation. Outcomeswere implant and prosthesis survival rate, report of anybiological and prosthetic complications, peri-implantmarginal bone loss, bleeding on probing (BOP), and probingpocket depth (PPD).ResultsForty-five consecutive patients (20 male, 25 female; meanage 52.1 years) received 63 implants with at least one GBRprocedure simultaneously. All patients returned for the followupexamination 3 years after implant insertion-GBR surgery.No implant or prosthesis failed, giving cumulative implant andprosthetic survival rates of 100%. No major prosthetic orbiological complications were recorded. Mean marginal bonelevelapical to the reference point at implant placement(baseline) was 0.12u00b10.14 mm, and 0.61u00b10.21 mm and0.77u00b10.22 mm at the definitive restoration delivery and 2years after final loading, respectively. The total meanmarginal bone loss by the 3-year visit was 0.65u00b10.18 mm.Mean BOP values did not alter significantly betweendefinitive restoration and 2-years after loading (1.23u00b10.93 vs1.17u00b10.78; P=0.569), and neither did mean PPD values(2.62u00b10.59 vs 2.60u00b10.54; P=0.765).ConclusionsTapered implants with conical connection and partial machinedcollar had all survived with no major complications at 3-yearsafter implant insertion, and mean marginal bone level changes atimplant sites were low. Thus, within the limitations of the presentstudy, simultaneous placement of tapered implants with conicalconnection and GBR seems a viable treatment option for oralrehabilitation at sites with medium horizontal bone defects.

When choosing materials for implant-supported prostheses, the clinician could choose the one that places less stress on the implant and surrounding tissues2. Use of ceramic materials for veneering implant-supported prostheses is common. Computer-aided design and computer-aided manufacturing (CAD-CAM) technique have provided many advantages starting with the speed of design, processing, fabrication and accuracy. New materials have been developed to achieve the optimum quality; these include Zirconia-reinforced Lithium Silicate Ceramic (ZLS) and a Hybrid ceramic containing a resin-reinforced glass network (Enamic). The screw access channel technique is frequently utilized to help clinicians retrieve cement-retained implant crowns when needed. One concern with screw access holes is the fracture resistance of the crown material after creating the access channel in a cement-retained crown. This study was performed in two parts: the aim in Part 1 was to measure and compare the influence of (ZLS) and (Enamic) with different ceramic CAD-CAM implant-supported crown materials for strain distribution generated around the implants during cyclic loading. Part 2 was performed in order to measure and compare the load to fracture values of different cement-retained crown materials with screw access channel. Material and Methods: Four 3.7mm X 10mmD platform Legacy 1 Implants (Implant Direct, Carlsbad, CA, USA) were placed into a 12" x 2" x 8mm resin block. Legacy Zirconia Straight Contoured Stock Abutment (3.5mmD/1mm Collar Height, Implant Direct, Carlsbad, CA, USA) (N=20) were torqued into the implant fixtures to support crowns that were milled from a virtual design using four different all-ceramic materials (N=20). A resin cement was used to cement crowns on zirconia abutments. Initially, the crowns were cyclically loaded two times at loads ranging from 5N to 400N at a thirty-degree angle and strain values were recorded. Three-dimensional digital image correlation (3D-DIC) and a commercial image correlation software (VIC 3D v7.0, Correlated Solutions, Inc. Columbia, SC, USA) were used to measure maximum and minimum principal strains (tensile and compressive) around the implants. Secondly, screw access channels were prepared on crowns to receive a vertical load from 0N until fracture and load-to-failure values of crowns were recorded and were statistically analyzed. Results: Strain measurements showed no statistically significant difference in the crown materials (p>.05). No statistical difference was found in the interaction between the material and the type of the strains in this study. These challenges are particularly important in the molar region. To meet the needs of patients requiring tooth replacement in such indications, wide diameter variable-thread tapered implants were developed. Further trials are necessary to evaluate the performance of wide diameter implants in every day clinical practice.The aim of this retrospective study was to evaluate the safety and efficacy of wide diameter variable-thread tapered implants primarily placed in fresh extraction sites in the molar region. Marginal bone level change, implant survival, and implant stability were evaluated.This is a retrospective, non-interventional, analysis of de-identified data from patients treated in one center. Patients who received at least one wide diameter variable-thread tapered implant (NobelActive WP, Nobel Biocare) in the molar region between April and October 2014 were included. Patients with bone defects were considered unsuitable to receive implants immediately. Grafting was performed in all fresh extraction sites using anorganic bovine bone matrix. No covering membranes were used. A provisional restoration was placed immediately after implant insertion when primary implant stability (insertion torque) was higher or equal to 35 Ncm. At last follow-up, implant stability was assessed by rocking and tapping and radiographs were taken as part of routine treatment. Descriptive statistical analysis was performed using SPSS 24 (IBM, USA).Ten wide diameter (5.5 mm) variable-thread tapered implants (length 7-13 mm) were placed in the mandibular molar region of 9 patients (6 female, 3 male). Book excerpt: BackgroundRecently, the use of short implants in anatomically restricted regions has been attracting the attention of many clinicians around the world, as it represents an excellent minimally-invasive treatment option. Nevertheless, when selecting short implants the prevalence of peri-implant bone resorption still remains to be elucidated. The DENTSPLY ASTRATECH IMPLANT EV SYSTEM used in this study is a system in which the operator is able to select the drilling protocol according to the bone quality.AimWe aim to analyze the clinical benefits of short implants (6mm) and at the same time we believe that the key to minimizing bone resorption is to obtain an appropriate initial stability taking into account the placement torque and at the same time by optimizing the bone quality and drilling protocol.Materials and Methods Twenty-six 6 mm short implants (DENTSPLY SIRONA Astra Tech Implant System EV) were placed in 16 patients. Among the 16 patients, 7 were treated with 6 mm implants in multiple sites. Bone quality, drilling protocol together with the placement torque were recorded. Then, changes in the degree of bone resorption, and the state of the peri-implant mucosal sulcus were measured from the time of delivery of superstructures to 2 years after setting. Of the twenty-six implants, ten were inserted in the maxilla and sixteen were inserted in the mandible. The diameter of nineteen implants was 4.2 mm and that of seven implants was 4.8 mm, all of straight type.ResultsBone quality of all the alveolar processes was type I for 3, type II for 8, and type III for 15. The mean placement torque was 34.2 Ncm. (Standard deviation [SD]: 8.16, range [r] 20 to 45 Ncm). The retention type of the superstructures was cement-retained for six implants (1 single crown, 5 splinted crowns), and screw-retained for twenty implants(6 single crowns and 14 splinted crowns). During the 2 years of follow-up after setting the superstructures, all implants were stable. The mean mesial Bone Level (BL) at the superstructure setting time was -0.05 mm. (SD: 0.62, r: −1.33 to 1.73 mm), the mean distal BL was 0.37 mm (SD: 0.74, r: -0.63to 2.26 mm). The mean mesial BL after 2-years was 0.33 mm (SD: 0.65, r: -0.96 to 1.52 mm), the mean the distal BL was 0.53 mm (SD: 0.63, r:-0.5 to 2.23 mm). The mean BL differences between the setting time and after 2-years were 0.38 mm on the mesial side (SD: 0.49, r: -0.25 to 1.88 mm) and 0.15 mm in the distal side (SD: 0.42, r: -0.98 to 84 mm)ConclusionsIn this retrospective study, we examined the clinical peri-implant tissue stability after placing 6 mm implants. Measurement of peri-implant bone levels showed that in most cases the bone-implant junction moved coronally. This phenomenon appeared to have resulted from continuous bone remodeling. There were no significant differences in bone levels variations depending on region,u3000superstructure extension or retention-type. This suggests that short implants are beneficial in clinical practice.
