Book excerpt: Background- Black women are two times more likely to die from cervical cancer than White women, despite Black women having higher cervical screening rates than White women. One of the factors contributing to the racial disparity in cervical cancer mortality has been Black women's decreased likelihood to adhere to medical recommendations for follow-up after an abnormal Pap smear. Over the past decade, there has been attention placed on clinical provider implicit bias as a contributing factor to racial disparities that exist within healthcare. Though implicit bias has been recognized as a barrier to adherence to medical recommendations, there have been no studies to date that have explored patient perceived experiences of provider bias as a possible factor in adherence to follow-up recommendations after an abnormal Pap smear for Black women. Methods- A cross-sectional study design was used to determine a significant predictive relationship between perceived discrimination and intention to follow-up after an abnormal Pap smear among black women in comparison to other races. Seven predictors of discrimination along with intention to follow-up after an abnormal Pap smear were measured via survey. Results- 250 subjects were surveyed to capture their experiences of patient-perceived discrimination and their intention to follow-up after an abnormal Pap smear result. Black women did not indicate a significant relationship between patient perceived discrimination and intention to follow-up after an abnormal Pap smear when compared to other races. Conclusion- Despite results, Black women still continue to experience a disparity in cervical cancer outcomes, with literature supporting a decrease in follow-up after an abnormal Pap smear. Additional studies should be conducted to explore the role that patient perceived discrimination plays in patient follow-up after an abnormal Pap smear result.

Book excerpt: Background: Cervical cancer among Black women is a major individual and public health concern. Despite advancements in medical technology and policy, disparities in cervical cancer diagnosis and mortality still exist between White and Black women, with Black women having higher rates of diagnosis (10.0 vs 7.1 per 100,000) and mortality (4.1 vs 2.0 per 100,000). Previous studies have focused heavily on barriers to obtaining cervical cancer screening among Black women and efforts to improve screening rates. Despite speculation by researchers that gaps in receipt of quality follow-up care may contribute to disparities, its role in disparate mortality rates between Black and White women has yet to be formally studied. Purpose: The goals of the present research were to (1) assess differences in Pap screening rates and (2) rates of adherence to recommended follow-up care after abnormal Pap results between Black and White women, (3) evaluate how specific factors of the Health Belief Model (HBM) influence the decision-making process for obtaining Pap screening and (4) receiving recommended follow-up Pap care, and (5) evaluate the role of ethnic centrality as a factor in health decision-making. Results: Results indicated that HBM facets functioned similarly in predicting engagement in Pap screening and follow-up care, although there were differences in the influence of different facets by race. Ethnic centrality showed little applicable influence on adherence to follow-up care among Black women. An exploratory analysis revealed that doctor recommendation was the most influential factor predicting follow-up care use for both racial groups. Conclusion: The present study offers support for increased education and training on provider recommendation of appropriate follow-up care, as well as effective provider communication of results and services using messaging that appeals to the values and concerns of patients.

Book excerpt: Background: Cancer is the second leading cause of death in the United States, accounting for 22% of all deaths among women. Despite the importance of the Pap test in preventing and detecting cervical cancer, screening rates among poor women remain low. The pathways linking poverty with lower Pap test use remain unclear. The screening disparity for this low-cost test suggests that poor women may face other transaction costs, opportunity costs and barriers in accessing Pap tests that are not faced by women living above poverty. Data/Methods: The data set merged the 2005 California Health Interview Survey, 2000 Census, and safety net clinic data. I examine elements of the patient-provider interaction (racial/ethnic discrimination) and healthcare system (distance to the nearest safety net clinic) as possible mediators between individual-level poverty and Pap tests. I then examine the role of individual-level poverty in moderating the association between discrimination and Pap test use and safety net distance and Pap test use. I also examine whether the neighborhood-level poverty moderates the association between individual-level poverty and Pap tests, discrimination and Pap tests, and distance and Pap tests. The sample included women, 18-64, with a physician visit in the previous 5 years and no history of cervical cancer. Mediation was tested using Generalized Estimating Equations (GEE) and a weighted product-of-coefficients test. Moderation was tested using GEE and including interaction terms. Results: Poor women were significantly less likely to report a timely Pap test, had significantly higher odds of reporting discrimination, and were more likely to reside closer to a safety net clinic. Discrimination did not predict Pap tests and was not a mediator between individual-level poverty and Pap tests. Safety net distance predicted Pap tests and was a mediator between individual-level poverty and Pap tests. Neither individual-level poverty nor neighborhood-level poverty modified the proposed relationships with Pap test use. Conclusion: Racial/ethnic discrimination did not explain disparities in Pap tests. Proximity to a safety net clinic was shown to be protective against disparities and may play a role in addressing disparities in screening. Individual-level poverty and neighborhood-level poverty did not influence the role of proposed mediators in predicting Pap tests.
