Book excerpt: Spending on the NHS is the fastest growing area of public expenditure, with a budget for 2004-05 of £69.7 billion, rising to £76.4 billion in 2005-06 and £92.6 billion in 2007-08. Despite the increased resources, the NHS reported an overall deficit of £251 million (including Foundation Trusts) in 2004-05, the first time since 1999-2000 that the NHS as a whole had overspent. In 2005-06, the overall deficit increased to £570 million, with a rise in both the number of NHS organisations (Strategic Health Authorities, Primary Care Trusts, NHS Trusts and NHS Foundation Trusts) reporting a deficit and the proportion of those bodies reporting a deficit. Following on from a report (HC 1059-I, session 2005-06; ISBN 9780102938159) published in June 2006, jointly prepared by the National Audit Office and the Audit Commission, the Committee's report examines three main issues: the factors that led to the deficits, the impact on the organisations involved, and the steps taken to recover the deficits. Amongst its findings, the Committee concludes that there are a number of reasons why NHS bodies are in deficit, with most organisations in deficit tending to have had a deficit the previous year. Bodies already in deficit looking to turn their financial position around can be disadvantaged as they are expected to recover that deficit in the next financial period. The NHS has been under significant financial pressure to meet the costs of national pay initiatives which the Department of Health had not fully costed, and as some NHS bodies have coped better than others in managing these cost pressures, this indicates that the standard of financial management expertise varies across the NHS, as does the level of clinical engagement in financial matters.

Book excerpt: In January 2004, the Inland Revenue entered into a contract with Capgemini to provide IT services to support the Department's business. The contract, known as ASPIRE (Acquiring Strategic Partners for the Inland Revenue), replaced two previous contracts with EDS and Accenture and, following the merger of the Inland Revenue and HM Customs & Excise in 2005, the latter's IT services contract with Fujitsu was incorporated within ASPIRE in April 2006. This change from one supplier to another was the first of this scale in the public sector, and the contract provides wider lessons for the public sector in re-competing major contracts, particularly relating to the payment of transition costs. The cost of the contract has risen from £2.83 billion to £8.5 billion over the 10 year term. Following on from a NAO report on this topic (HCP 938, session 2005-06; ISBN 9780102939170) published in July 2006, the Committee's report examines the procurement process, the transition to a new supplier and the performance of the ASPIRE contract to date. Findings include: i) before concluding the deal, the Department should have evaluated bids against a range of demands for IT services and analysed the effect of different scenarios on suppliers' prices and profit margins; ii) it should have evaluated the performance of consultants and the lessons to be learned from their use, not only for their own benefit but for that of other departments; iii) by contributing to bid costs and paying transition costs to secure competition for the contract, the Department incurred a premium of £51.9 million; iv) it should set more challenging performance targets to impose sufficient discipline on suppliers; and v) the Government should not be placed in the invidious position of having to commission further work from a contractor in order to recover compensation for underperformance.

NHS Shared Business Services is a joint venture between the Department of Health and Xansa PLC selling procurement, finance and accounting services to 89 NHS organisations out of a total of 416 potentially eligible NHS bodies. It is not yet making a profit and has paid no dividend to either the Department of Health or Xansa. It needs to attract a further 22 customers simply to break even, and approximately 180 more customers to deliver its forecast savings to the taxpayer of £250 million by 2014-15. HM Prison Service's Shared Services Centre provides finance, procurement and human resources services to all 128 Prison establishments, and the system is now working well.

The Committee states that oversight of the Single Payment Scheme is a singular example of comprehensively poor administration on a grand scale. With a paucity of good management information in the Agency and the complacent oversight by the Department having acted to obscure the true situation. A focus over the last two and a half years in bringing forward payments to farmers has enabled the Agency to bring its deadline forward by nearly seven weeks, but this is still six weeks off the deadline it had planned and a long way short of the standards set in Wales, Scotland and Northern Ireland. Also there has been a negligible attention to the protection of tax payers' interests. The Rural Payments Agency has spent £350 million on a cumbersome IT system that can only be supported at huge cost and which is increasingly at risk of becoming obsolete, with the data held in the system remaining riddled with errors and efforts to recover overpayments having been slow, disorganised and haphazard. The Committee identifies poor leadership within the Agency and a lack of attention by the Department. Each claim costs over six times more to process in England than in Scotland. Further the Department was not able to demonstrate an adequate grasp of the costs of administering the scheme. The Committee states that responsibility rests with the Accounting Officers to resolve this misadministration.

The MoD's assessment of its expected achievements against its six Public Service Agreement (PSA) targets, which run until the end of March 2008, has deteriorated since the previous year's Annual Report and Accounts. At the end of 2007, the MoD did not expect to meet the target relating to generating forces and expects "only partly" to meet targets relating to recruitment and retention, and defence equipment procurement. The failure to meet the target for generating forces is a consequence of the continuing high levels of deployment of the Armed Forces. The Committee is concerned that the Armed Forces have been operating at or above the level of concurrent operations they are resourced and structured to deliver for seven of the last eight years, and for every year since 2002. Achieving manning balance in all three Service continues to be a challenge. Shortages remain within many specialist trades in all three Armed Services, but especially in the Army Medical Service. The report notes the failure to meet harmony guidelines in the Army and the Royal Air Force - another indicator of the pressure on the Armed Forces from the continuing high level of operations - and another target missed by all three services is for ethnic minority recruitment. The MoD continues to experience substantial forecast cost increases on equipment programmes, and the report notes delays in delivering equipment programmes to the planned in-service dates. The MoD faces difficult choices in the face of expected cuts in the defence programme and the management of a streamlining exercise to reduce civilian posts in the headquarters.

The Commission took up its new powers, and those of the former Commission for Racial Equality, the Disability Rights Commission and the Equal Opportunities Commission (the Legacy Commissions), on 1 October 2007. Serious errors were made in setting up the Commission, not helped by three changes of sponsor department in the months immediately before its launch. The Commission now accepts that it was not ready for business when the doors opened on 1 October 2007 and that its set-up process, which cost £39 million, was flawed and inefficient: it did not have in place sufficient senior staff, a transition strategy, business strategy, organisation design or job descriptions. Skills gaps were filled by bringing back former employees of the CRE as consultants, even though they had all received severance payments. The Treasury, which should have been consulted about these appointments, did not grant approval retrospectively as the Commission could not prove that the re-engagements gave good value for money. This expenditure was therefore deemed irregular and the Comptroller and Auditor General issued a qualified opinion on the Commission's 2006-08 accounts. This is not the way that the Public Accounts Committee expects public bodies to be run. Strong controls need to be in place to ensure that such errors do not recur.
