Book excerpt: The medical research landscape in the United States is supported by a variety of organizations that spend billions of dollars in government and private funds each year to seek answers to complex medical and public health problems. The largest government funder is the National Institutes of Health (NIH), followed by the Department of Defense (DoD). Almost half of DoD's medical research funding is administered by the Congressionally Directed Medical Research Programs (CDMRP). The mission of CDMRP is to foster innovative approaches to medical research in response to the needs of its stakeholdersâ€"the U.S. military, their families, the American public, and Congress. CDMRP funds medical research to be performed by other government and nongovernmental organizations, but it does not conduct research itself. The major focus of CDMRP funded research is the improved prevention, diagnosis, and treatment of diseases, injuries, or conditions that affect service members and their families, and the general public. The hallmarks of CDMRP include reviewing applications for research funding using a two-tiered review process, and involving consumers throughout the process. Evaluation of the Congressionally Directed Medical Research Programs Review Process evaluates the CDMRP two-tiered peer review process, its coordination of research priorities with NIH and the Department of Veterans Affairs, and provides recommendations on how the process for reviewing and selecting studies can be improved.

Book excerpt: Problems stemming from the misuse and abuse of alcohol and other drugs are by no means a new phenomenon, although the face of the issues has changed in recent years. National trends indicate substantial increases in the abuse of prescription medications. These increases are particularly prominent within the military, a population that also continues to experience long-standing issues with alcohol abuse. The problem of substance abuse within the military has come under new scrutiny in the context of the two concurrent wars in which the United States has been engaged during the past decade-in Afghanistan (Operation Enduring Freedom) and Iraq (Operation Iraqi Freedom and Operation New Dawn). Increasing rates of alcohol and other drug misuse adversely affect military readiness, family readiness, and safety, thereby posing a significant public health problem for the Department of Defense (DoD). To better understand this problem, DoD requested that the Institute of Medicine (IOM) assess the adequacy of current protocols in place across DoD and the different branches of the military pertaining to the prevention, screening, diagnosis, and treatment of substance use disorders (SUDs). Substance Use Disorders in the U.S. Armed Forces reviews the IOM's task of assessing access to SUD care for service members, members of the National Guard and Reserves, and military dependents, as well as the education and credentialing of SUD care providers, and offers specific recommendations to DoD on where and how improvements in these areas could be made.

Book excerpt: The Pres¿s. budget request for the DoD Defense Health Program has increased steadily in recent years. For ex., from FY 2005 to FY 2009, the budget request for the program increased from $17.6 billion to $23.6 billion, an increase of 34%. DoD has attributed a majority of this increase to growth in medical care, dental care, and pharmaceuticals provided in the private sector to active duty personnel and other eligible beneficiaries. These private sector expenses are funded through the Defense Health Program¿s Private Sector Care Budget Activity Group (BAG). From FY 2005 to FY 2009, the budget request for BAG increased by 36% -- from $9.0 billion to $12.2 billion. This report is a review of the FY 2009 budget request for the BAG. Charts and tables.

Book excerpt: At the request of the U.S. Army, the Board on Army Research and Development of the National Academies of Sciences, Engineering, and Medicine conducted a 3-day workshop to explore how the Army can improve its strategic medical infrastructure planning with a view to 2035. The workshop, held July 14-16, 2020, brought together experts and key stakeholders from academia, industry, and government. The Army requires fidelity, consistency, and predictability in planning and managing research, development, test, and evaluation resources for medical infrastructure across all appropriation sources to effectively develop, deliver, and respond to military medical capability needs. In response to the Army's requirement, the workshop was designed to address the components of a sustainable, reinforcing enterprise framework (organizational and fiscal). Presentations and discussions examined roles, responsibilities, and coordinating mechanisms among major stakeholders of battlefield medicine; case studies of comparably complex non-government enterprise solutions; and opportunities to link ends, ways, and means for improvements. This publication summarizes the presentations and discussion of the workshop.

On July 13, 2000, Congress enacted Public Law No. 106-246, "Making Appropriations for Military Construction, Family Housing, and Base Realignment and Closure for the Department of Defense for Fiscal Year Ending September 30, 2001, and for Other Purposes." Public Law No. 106-246 appropriated an additional $1.3 billion to cover critical shortfalls in funding managed care support contracts. In House Conference Report No. 106-710, which accompanied Public Law No. 106-246, conferees directed the Inspector General, DoD, in coordination with the General Accounting Office, to examine the fund control practices of the TRICARE (the military health care system) program. In addition, DoD was directed to report to the congressional Defense committees within 60 days after enactment on whether DoD violated fiscal law or departmental regulations related to the shortfalls. Public Law No. 106-246, section 105, specifically appropriated $615.6 million of the $1.3 billion for the Defense Health Program with the following stipulations specified in section 106.
