Book excerpt: La société actuelle est confrontée aux problèmes récurrents de la violence des enfants : violence à l'encontre de l'environnement physique et humain mais aussi retournée sur soi. La thèse est une contribution à l'étude de l'émergence de cette violence chez des enfants tout-venant des deux sexes considérés aux âges de 7-8 ans (âge de la sociabilité) et 10-11 ans (âge de la socialisation). L'approche théorique appréhende les processus socialisateurs au niveau affectif; intellectuel et psychosocial. Elle permet de mettre en avant qu'ils sont fondés sur des influences externes dues à la famille, à l'école et à la société d'une part, sur des influences internes, individuelles dû aux transformations physiologiques, cognitives et affectives d'autre part. La problématique et les hypothèses reposent premièrement sur l'impact socialisateur des styles éducatifs familiaux, deuxièmement sur l'impact scolaire en terme de normalisation comportementale des apprentissages sociaux, troisièmement sur l'impact des transformations de l'estime de soi entre les âges considérés ; les trois domaines analysés interagissant entre eux, en particulier en fonction du statut et du rôle dévolus aux 84 enfants concernés dans les familles et à l'école au sein de leurs milieux semi-ruraux du Vimeu. L'analyse psychosociologique et clinique des données recueillies à l'aide d'outils étalonnés (Dynamique Personnelle et Images, Nathan Scolaire, Echelle de développement psychosocial de Zazzo et Hurtig, Test d'Estime de soi de Coopersmith) permet de saisir : premièrement l'impact fondamental des styles éducatifs auxquels sont soumis les enfants, deuxièmement le versant réactionnel individuel de ceux-ci ; lequel s'exprime de manière égale ou différente à l'école. L'étude constate que vis-à-vis des mères souvent perçues comme les agents éducatifs premier, mais aussi vis-à-vis des pères : autoritarisme ne fait pas autorité de manière systématique au regard des enfants. Son effet est déstabilisant et provoque soit la réactivité (agressive), soit la passivité (victimisation), soit l'alternance des deux conduites. Les enfants de 7-8 ans sont essentiellement en attente d'un humanisme adulte fait de reconnaissance, de considération et d'écoute de leur personne. Ceux qui sont socialisés dans le laxisme se caractérisent fréquemment par une estime de soi déséquilibrée par excès souvent, par défaut quelquefois. Certains apparaissent de manière manifeste en attente de rencontres résilientes. Le cheminement potentiel vers l'agressivité et la violence se manifeste au travers de deux tendances : d'abord celle de l'agression avec la présence fréquente de conduites à risque à l'encontre d'autrui ou de soi souvent exprimées sur un mode ludique. Elle est renforcée par la familiarité avec les jeux électroniques qui dénaturent la réalité des faits violents ; ensuite celle de la victimisation exprimée au travers d'une estime de soi déficitaire ou déséquilibrée également sensible dans un développement psychosocial manquant d'autonomie réelle et véritable. Les troubles relationnels constatés se manifestent par un niveau d'hyperactivité ou au contraire par une passivité comportementale sensible dans les processus vitaux d'accommodation et d'assimilation des fonctions de socialisation vécus à l'école et dans les familles : ils portent sur la manière déséquilibrée d'accéder aux sens de la morale, de l'équité, de l'échange, de la justice et de la citoyenneté

Book excerpt: La présente étude pilote visait à documenter de façon préliminaire l'effet d'une thérapie cognitive-comportementale (TCC) avec participation parentale des troubles anxieux adaptée aux symptômes médicalement inexpliqués (SMI) chez des enfants âgés de 8-12 ans. Cette étude a documenté qualitativement le déroulement de la TCC en décrivant l'adhésion au traitement, les principaux défis rencontrés par les participants et les intervenants ainsi que la participation parentale au traitement. L'étude visait également à documenter quantitativement l 'effet du traitement sur : 1) l'anxiété (taux de rémission des troubles anxieux et symptômes anxieux) ; 2) les SMI (interférence et intensité des SMI, somatisation) ; et 3) les pratiques parentales. Pour ce faire, 9 enfants et leurs parents ont été recrutés par le centre mère-enfant Soleil du CHUL de Québec et par la liste courriel de l'Université Laval. L'étude repose sur un devis de séries de réplications d'études de cas à niveaux de bases multiples non concurrents de type A-B. À la suite d'une première évaluation des troubles anxieux et sur la nature des SMI, les participants ont été répartis aléatoirement à un niveau de base de 2 à 4 semaines avant de commencer le traitement nommé « Traque Ta Trouille pour les SMI » d'une durée de 11 à 14 semaines. Les enfants et les parents ont ensuite été évalués à la suite du traitement et lors d'un suivi 6 mois. Pendant le traitement, les principaux défis rencontrés étaient de compléter les exercices thérapeutiques par les familles ainsi que l'implication parentale dans les exercices à faire à la maison. Selon les résultats obtenus aux entrevues diagnostiques, au moins un trouble anxieux primaire (62,5%) ou secondaire (75%) était en rémission au post-traitement chez les huit participants (100%). Au suivi 6 mois, les résultats ont révélé un taux de rémission complet de 42,78%. Au questionnaire sur les SMI, une diminution statistiquement significative a été retrouvée pour l'interférence des SMI dans le temps. Les analyses par régressions segmentées calculées à partir des données quotidiennes ont révélé une augmentation de l'anxiété et des SMI lors de l'introduction du traitement, autant selon les enfants que les parents. Cette augmentation des symptômes était suivie d'un retour au niveau initial d'anxiété à la fin du traitement et à une amélioration des SMI selon les enfants. Selon les parents, seuls les SMI se sont améliorés à la fin du traitement. Une diminution significative des pratiques parentales de surprotection a été retrouvée. Les résultats de ce mémoire doctoral soulignent la pertinence d'une TCC adaptée pour la comorbidité SMI et l'anxiété et l'importance de poursuivre les recherches sur l'adaptation d'un tel traitement pour améliorer l'efficacité ainsi que l'importance d'une participation parentale dans le traitement des enfants souffrant de cette comorbidité.
