Kernberg reviews some of the recent developments and controversies in psychoanalytic theory and technique. Gathering together both previously published articles and extensive new material, Dr. Kernberg examines such issues as the new psychoanalytic views of homosexuality, bisexuality, and the influence of gender in the analytic relationship. He explores the application of psychoanalysis to non-clinical fields, including the problem of psychoanalytic research and its clinical implications, the validation of psychoanalytic interventions in the clinical process, and the challenges of psychoanalytic education. He shows how psychoanalysis can be helpful in addressing such cultural problems as socially sanctioned violence. And he asserts the continued relevance of object relations theory and its compatibility with Freud’s dual drive theory.

Available in PDF, EPUB and Kindle. Book excerpt: Freud's Beyond the Pleasure Principle constitutes a major landmark and a real turning point in the evolution of psychoanalytic theory. Pushing aside the primacy of the tension-discharge-gratification model of mental dynamics, this work introduced the notion of a "daemonic force" within all human beings that slowly but insistently seeks psychic inactivity, inertia, and death. Politely dismissed by some as a pseudo-biological speculation and rapturously espoused by others as a bold conceptual advance, "death instinct" became a stepping stone to the latter conceptualizations of mind's attacks on itself, negative narcissism, addiction to near-death, and the utter destruction of meaning in some clinical situations. The concept also served as a bridge between the quintessentially Western psychoanalysis and the Eastern perspectives on life and death. These diverse and rich connotations of the proposal are elucidated in On Freud's "Beyond the Pleasure Principle". Other consequences of Freud's 1920 paper - namely, the marginalization of ego instincts and the "upgrading" of aggression in the scheme of things - are also addressed.

Available in PDF, EPUB and Kindle. Book excerpt: "Hatred, Emptiness, and Hope offers an updated vision of psychoanalytic object relations theory, revealing its application to transference focused psychotherapy (TFP), a treatment approach derived from and related to psychoanalysis. With empirical studies carried out by the Personality Disorders Institute at Weill Cornell as a basis, this volume expands the applications of TFP, illustrating it at work in scenarios that include severe personality disorders, disturbances in sexuality and love relations of narcissistic personalities, inpatient hospital treatment, and group settings. It also looks at the implications of new developments in neurobiology on psychoanalytic object relations theory. Readers will benefit from a discussion of the practice of TFP itself, with chapters that tackle the supervision of psychoanalysis, challenges for the future of psychoanalysis, and innovations that can serve to strengthen its role as a profession, a treatment approach, and a social organization within mental health sciences. It focuses on the analysis of particular clinical features of personality pathologies and describes the consideration of contemporary psychoanalytic object relations theory as a general theoretical frame of treatment that permits to conceptualize both normal personality functioning and the very nature of personality disorders. This volume also includes my recent efforts to contribute to the understanding of the relationship between neurobiological dispositions and their interaction with psychodynamic developments, again, both in normality and psychopathology. Finally, this volume explores the application of object relations theory to group processes, love relations, and therapists' training"--

Book excerpt: In this book, Kernberg first distinguishes, then interrelates psychoanalytic, clinical psychiatric, and neurobiological perspectives in a variety of areas, beginning with severe personality disorders and extending to love, destructiveness, mourning, spirituality, and the future of psychoanalytic inquiry. Dr. Kernberg is renowned for his work with borderline and narcissistic patients, and in this book, he offers new approaches to the diagnosis and treatment of the most severe personality disorders, particularly the spectrum of severe narcissistic psychopathology. His effort to relate psychoanalytic to neurobiological findings continues in two fascinating areas -- the study of sexual love and of religious experience -- and he examines object relations theory in relation to these two phenomena. Kernberg's analysis of love and aggression is both bold and nuanced and will captivate the professional psychotherapist as well as the psychologically astute general reader.

Book excerpt: This groundbreaking first volume of the Series has a number of features that set it apart from other books on this subject: Firstly, it focuses on interpersonal, humanistic and ecological views and approaches to P/MH nursing. Secondly, it highlights patient/client-centered approaches and mental-health-service user involvement. Lastly, it is a genuinely European P/MH nursing textbook – the first of its kind – largely written by mental health scholars from Europe, although it also includes contributions from North America and Australia/New Zealand. Focusing on clinical/practical issues, theory and empirical findings, it adopts an evidence-based or evidence-informed approach. Each contribution presents the state-of-the-art of P/MH nursing in Europe so that it can be transferred to and implemented by P/MH nurses and the broader mental health care community around the globe. As such, it will be the first genuinely 21st century European Psychiatric Mental Health Nursing book.

Book excerpt: The papers featured in Attachment and Sexuality create a dense tapestry, each forming a separate narrative strand that elucidates different configurations of the relationship between attachment and sexuality. As a whole, the volume explores the areas of convergence and divergence, opposition, and integration between these two systems. It suggests that there is a bi-directional web of influences that weaves the attachment and sexual systems together in increasingly complex ways from infancy to adulthood. The volume’s unifying thread is the idea that the attachment system, and particularly the degree of felt security, or lack thereof in relation to early attachment figures, provides a paradigm of relatedness that forms a scaffold for the developmental unfolding of sexuality in all its manifestations. Such manifestations include infantile and adult, masturbatory and mutual, and normative and perverse. Also central to the papers is the idea that the development of secure attachment is predicated, in part, on the development of the capacity for mentalization, or the ability to envision and interpret the behavior of oneself and others in terms of intentional mental states, including desires, feelings, beliefs, and motivations. Topics discussed in the book will help to shape the direction and tenor of further dialogues in the arena of attachment and sexuality.

Available in PDF, EPUB and Kindle. Book excerpt: Training in psychoanalysis is a long and demanding process. However, the quality of education available is hugely variable across the world. The structure of psychoanalytic education, centered on the hierarchical "training analysis" system, reflected a concerted effort to maintain a stable and high quality educational process. However, throughout time this system has become a major source of institutional contradictions that affect the training of candidates, the scientific developments within psychoanalysis, and the nexus of psychoanalytic theory and practice with the surrounding scientific, social and cultural world. Psychoanalytic Education at the Crossroads examines the ways in which group processes, the hierarchal culture in institutes, the influence of individual personalities, the lack of research and the faults in supervision can all stifle creativity and hinder candidates’ progress. In this compelling work, Otto Kernberg sets out clear suggestions for how these issues can be addressed, and how he sees the future of psychoanalytic education across all psychoanalytic settings and schools of thought. The first part of this volume is focused primarily on the analysis of the nature of these problems and their effects on the personal analysis and supervision of candidates; on theoretical and clinical seminars; on selection, progression, and graduation; on educational principles and requirements, developments of theory and technique and, in particular, limitation versus expansion of the realm of interests and applications of psychoanalysis. The second part of this volume deals with proposals of solutions to the problems encountered, and major suggestions for innovation in psychoanalytic education. The author’s work in this area has been hugely influential. Kernberg has made a substantive difference in the development of psychoanalytic institutes and education, and continues to do so. Psychoanalytic Education at the Crossroads will be essential reading to anyone involved in psychoanalytic education, whether as a psychoanalyst, psychoanalytic psychotherapist, trainee, trainer, or supervisor.

Book excerpt: Some sixty years after the "Controversial Discussions" in the early 40s, this passionate book resurrects their spirit on a global scale. Under the aurthor's generous, tactful yet strong leadership, a small discussion group of noteworthy analysts of the International Psychoanalytical Association, coming from all the theoretical and geographical regions in today's psychoanalytic Babel, met several times over three years in order to deal, by way of the detailed discussion of their clinical experiences, with what to many of those involved was and still is a polemical concept: that of the borderline patient. Such a concept, widely accepted in the United States, remains controversial in many parts of the psychoanalytic universe, mainly in what concerns the multifaceted relationship between psychoanalytic and psychiatric categories. To be remarked upon is the sincerity put to play by the participants in expressing their doubts, their agreements and their disagreements in the heady process of developing a grasp on the others' viewpoint.

The book is both an introduction to and a statement of mainstream American psychoanalysis today and will be a standard reference for psychoanalytic trainees, authors, and teachers. Under the direction of the editors and a distinguished panel of advisors, the contributors present a broad overview of more than forty key clinical and theoretical concepts. They define each concept, trace its historical development within psychoanalysis, describe its present status, discuss criticisms and controversies about it, and point out emerging trends. A selected reference list is supplied for each concept. Together, the articles provide a systematic examination of the theoretical infrastructure of psychoanalysis. The book has been designed as a companion volume to Psychoanalytic Terms and Concepts, a glossary edited by Drs. Moore and Fine under the auspices of the American Psychoanalytic Association.

Book excerpt: In this first-of-kind book, senior psychoanalysts from around the world offer personal reflections on their own training, what it was like to become a psychoanalyst, and what they would like most to convey to the candidate of today. With forty-two personal letters to candidates, this edited collection helps analysts in training and those recently entering the profession to reflect upon what it means to be a psychoanalytic candidate and enter the profession. Letters tackle the anxieties, ambiguities, complications, and pleasures faced in these tasks. From these reflections, the book serves as a guide through this highly personal, complex, and meaningful experience and helps readers consider the many different meanings of being a candidate in a psychanalytic institute. Perfect for candidates and psychoanalytic educators, this book inspires analysts at all levels to think, once again, about this impossible but fascinating profession and to consider their own psychoanalytic development.
