C'est la première méthode de contraception dans le monde. En France, malgré une excellente couverture contraceptive, le taux d'interruption volontaire de grossesse reste stable, en partie du fait de la mauvaise observance des contraceptions orales. Majorer l'utilisation du dispositif intra-utérin, afin d'adapter au mieux la méthode contraceptive à chaque femme, pourrait réduire le taux de grossesses non désirées. Une enquête d'opinion a été réalisée auprès de 121 femmes de 18 ans à la ménopause consultant en cabinets de médecine générale, afin d'évaluer leur connaissance de ce moyen de contraception et les réticences associées. Le taux de satisfaction chez les porteuses de DIU dans cet échantillon est de 85%. Malgré un taux de 40% d'opinion favorable au DIU sur l'ensemble des femmes interrogées, on retrouve de nombreuses réticences à utiliser ce moyen de contraception. Ce sont principalement des craintes liées à son manque d'efficacité. Mais aussi des préoccupations pour leur santé, une gêne lors de l'utilisation du DIU. La notion de «corps étranger» est souvent évoquée. Ces réticences ne découlent pas d'un manque de connaissance. Seules 5% des femmes ne sont pas du tout informées, et la moitié disent avoir «souvent» entendu parler du DIU. Les sources d'informations sont diverses, mais 82% des femmes déclarent qu'elles proviennent de leur entourage. Cette origine, également retrouvé dans la littérature internationale, remet en cause leur validité et peut expliquer la persistance d'idées fausses sur le DIU. Les professionnels de santé peuvent dissiper ces rumeurs s'ils bénéficient eux même de bonnes connaissances sur le DIU et sur l'évolution de son utilisation pratique: chez les nullipares et en contraception d'urgence, risque infectieux associé essentiellement à la pose et majoré uniquement les vingt premiers jours, risque de grossesse extra-utérine plus faible qu'en l'absence de contraception, et enfin absence d'interaction avec les AINS. Il faudra du temps pour vaincre les réticences les plus tenaces, mais avec les progrès techniques du DIU et les nombreuses études visant à rétablir la vérité sur cette méthode contraceptive, son utilisation devrait pouvoir s'élargir. Les médecins généralistes qui suivront de plus en plus de patientes sur le plan gynécologique, auront un rôle essentiel dans l'accès des patientes à la contraception. Une formation reste nécessaire, en particulier pour se familiariser avec la pose, si l'on veut proposer cette méthode

Cependant leur utilisation demeure faible chez ces patientes et reste l'apanage des femmes multipares. Nous avons voulu savoir quelles sont les connaissances et réticences des nullipares sur ce moyen contraceptif. Pour cela, nous avons proposé aux femmes nullipares âgées de 15 à 45 ans du département de l'Essonne un questionnaire anonyme accessible par internet. 166 questionnaires ont été analysés. 25 % n'avaient pas de moyen de contraception médicalisé. 5 % uniquement ont choisi le dispositif intra-utérin comme moyen de contraception. Les principales réticences évoquées sont le manque d'information, la peur de la douleur et avoir un corps étranger dans le corps. Les deux principales motivations sont l'absence d'hormone et l'efficacité. L'âge, le niveau d'étude, le lieu d'habitation et le statut conjugal n'influent pas sur le choix ou non de cette méthode. 60 % des femmes avaient reçu préalablement une information de la part d'un professionnel de santé sur le stérilet. Le matériel a été montré uniquement à 15 % d'entre elles. Son utilisation en contraception d'urgence est totalement méconnue. Une meilleure information des patientes nullipares semble souhaitable, grâce aux médecins généralistes formés à la gynécologie ou par des ressources adaptées tel que l'utilisation d'un modèle de DIU de démonstration, la mise à disposition des patientes de brochures informatives dans la salle d'attente et d'adresse des sites internet fiables. Cela permettra de lutter contre les préjugés et de donner accès à une contraception plus adaptée et plus efficace. De nouvelles études explorant les connaissances des femmes sur le stérilet avant et après la délivrance d'une information correcte, actualisée et adaptée seraient intéressantes.

Book excerpt: Contexte : le Dispositif intra-utérin est le deuxième moyen de contraception le plus utilisé en France après la pilule contraceptive. Bien que les recommandations de la Haute Autorité de Santé (HAS) le présente comme un moyen de contraception hautement efficace qui peut être proposé à toutes les femmes, seulement 3% des nullipares y ont recours. Objectif : L'objectif de l'étude est d'explorer les représentations et les connaissances des nullipares sur le DIU et de définir les freins à l'accès et au choix de cette méthode. Méthodes : Il s'agit d'une étude qualitative par entretien semi dirigé, menée entre janvier et juillet 2018 auprès de vingt-cinq jeunes femmes nullipares de 18 à 34 ans dans le département de la Marne (51), utilisant ou non un DIU. Les entretiens ont été retranscrits puis soumis à une analyse thématique des données. Résultats : Les femmes interrogées voient le DIU comme une contraception de longue durée d'action, naturelle non hormonale et peu contraignante. Malgré des bonnes connaissances sur son mécanisme d'action, des idées reçues et de fausses représentations persistent par un manque d'informations médicales et empêchent son utilisation. Il semble que trop de préjugés perdurent chez les professionnels de santé. Le DIU reste un sujet de discorde parmi les médecins encore nombreux à refuser la méthode chez les nullipares redoutant la survenue de complications infectieuses plus fréquentes. Les candidates insatisfaites de leur méthode contraceptive actuelle se sont renseignées sur le DIU mais craignent des effets indésirables repoussant son utilisation : le risque de douleur lors de son insertion, des dysménorrhées et ménorragies ainsi que l'aspect invasif du corps étranger. Ces connaissances proviennent le plus souvent de l'entourage proche (amies, mère, sœur), de Internet, des médias TV ou encore de l'enseignement scolaire (cours de SVT, cours d'éducation à la sexualité) avec des informations parfois approximatives voire complètement fausses. Le choix d'une contraception par DIU au cuivre dans cet échantillon apparait comme une démarche personnelle, avec des femmes désireuses d'un retour au naturel, sans hormones. Aucune participante ne connait le DIU comme contraception d'urgence. Conclusion : Le DIU est encore trop méconnu chez certaines femmes et les nombreuses craintes que la méthode engendre semblent limiter son utilisation. Pourtant le DIU présente les avantages correspondant aux souhaits des nullipares : facilité d'observance, bonne tolérance, longue durée d'action et efficacité. Encore trop de préjugés perdurent chez les nullipares et chez les professionnels de santé et représentent un frein à son utilisation. Mieux informer les jeunes femmes (au cabinet de médecine générale, à l'école) sur les contraceptions disponibles notamment sur le DIU faciliterait son accès et contribuerait probablement à diminuer le nombre de grossesses non désirées.

Nelly Oudshoorn explains why it is that, although the technical feasibility of male contraceptives was demonstrated as early as the 1970s, there is, to date, no male pill. Ever since the idea of hormonal contraceptives for men was introduced, scientists, feminists, journalists, and pharmaceutical entrepreneurs have questioned whether men and women would accept a new male contraceptive if one were available. Providing a richly detailed examination of the cultural, scientific, and policy work around the male pill from the 1960s through the 1990s, Oudshoorn advances work at the intersection of gender studies and the sociology of technology. Oudshoorn emphasizes that the introduction of contraceptives for men depends to a great extent on changing ideas about reproductive responsibility. Initial interest in the male pill, she shows, came from outside the scientific community: from the governments of China and India, which were interested in population control, and from Western feminists, who wanted the responsibilities and health risks associated with contraception shared more equally between the sexes. She documents how in the 1970s, the World Health Organization took the lead in investigating male contraceptives by coordinating an unprecedented, worldwide research network. She chronicles how the search for a male pill required significant reorganization of drug-testing standards and protocols and of the family-planning infrastructure—including founding special clinics for men, creating separate spaces for men within existing clinics, enrolling new professionals, and defining new categories of patients. The Male Pill is ultimately a story as much about the design of masculinities in the last decades of the twentieth century as it is about the development of safe and effective technologies.
