Book excerpt: INTRODUCTION : Depuis leur création il y a plus d'une vingtaine d'années, les médicaments génériques sont une source non négligeable d'économies pour le système de soins. Pour les personnes âgées, premières consommatrices de médicaments, ils constituent un fort risque de iatrogénie. Cette étude a pour but d'évaluer les connaissances des personnes de plus de 80 ans sur les médicaments générique, d'en connaître leur perception et leur proposition pour améliorer la situation actuelle. MATERIEL ET METHODE : c'est une étude qualitative en focus group réalisée de septembre à octobre 2013 auprès de personnes autonomes de plus de 80 ans. RESULTATS : 3 focus group de 6 personnes ont montré que les personnes âgées sont très attachées à leur traitement, la majorité dit être favorable et maîtriser les génériques mais au prix d'une extrême vigilance. Ils déplorent le perte de repère engendrée par les changements de présentation des comprimés et de leurs boites. CONCLUSION : Dans cette population fragile des plus de 80 ans, la prescription de médicaments génériques paraît engendrer actuellement un risque non négligeable de iatrogénie

Book excerpt: La France est l’un des premiers consommateurs de médicaments en Europe, les économies générées par la promotion du médicament générique tiennent un rôle majeur dans le maintien du système de santé actuel. Les patients de plus de 65 ans représentent une cible idéale puisqu’ils représentent 44% de la dépense médicamenteuse ambulatoire, cependant aucune étude française n’étudie leur perception des génériques. L’objectif principal de cette étude est d’évaluer la perception des patients de plus de 65 ans sur la facilité d’emploi des médicaments génériques. Les objectifs secondaires sont l’évaluation de leur perception sur l’efficacité, la tolérance, le prix, la dangerosité de ces derniers ; l’évaluation de l’information qu’ils reçoivent et de son incidence sur la perception des génériques. Nous avons réalisé une étude longitudinale prospective multicentrique où 165 patients ont été interrogés lors d’une consultation de médecine générale. 20% [13,9-26,1] des patients estiment que les médicaments génériques sont plus difficiles à utiliser que leurs équivalents princeps et cette proportion augmente avec le nombre d’antécédents médicaux. 48,5% [40,9-56,1] des patients pensent que les génériques sont moins efficaces alors que 32,7% [25,5-39,9] les perçoivent comme plus dangereux. Ils sont moins chers pour 76,4% [69,9-82,9] des patients et 36% [28,7-43,3] pensent qu’ils ont plus d’effets secondaires. Cette étude montre, que malgré une bonne perception de la facilité d’utilisation des médicaments génériques, il persiste un climat de défiance entretenu par l’image que véhiculent les médias et un manque d’implication des prescripteurs. L’information des patients reste un levier majeur dans la promotion des génériques et pourrait bénéficier d’une meilleure collaboration entre deux acteurs majeurs, le médecin et le pharmacien.

Les sujets âgés de plus de 65 ans représentent plus de 44% de la population. Les campagnes de promotion des génériques se font plus présentes dans notre quotidien via différents intervenants. L'objectif principal de cette étude est d'évaluer la perception de l'efficacité des génériques par les patients âgés de plus et moins de 65 ans. Les objectifs secondaires sont d'évaluer la perception du coût, de la qualité et de la sécurité d'emploi des génériques en fonction de la condition sociale et du traitement des patients, de l'information délivrée, et du ressenti de notre système de santé actuel. Nous avons réalisé une étude transversale et multicentrique auprès de 516 patients bas normands. Trente-deux pourcent pensaient que les génériques étaient moins efficaces que les princeps. Ils étaient de moins bonne qualité pour 33,3% d'entre eux, moins chers pour 73,2% et avaient plus d'effets indésirables pour 33,9%. Les patients âgés de plus de 65 ans pensaient que les génériques étaient moins efficaces (43% versus 30%, p = 0,038). Il n'y avait pas de différence significative concernant les critères de jugement secondaires. Un climat de défiance persiste autour des génériques, considérés comme bas de gamme notamment chez les patients les plus âgés. L'information délivrée aux patients notamment par le pharmacien ou le médecin généraliste, permet d'améliorer l'acceptation et la confiance envers les génériques. La pérennisation d'une meilleure acceptation de ces médicaments nécessite une meilleure collaboration entre médecins, pharmaciens, autorités de santé et firmes pharmaceutiques.

Ils sont cependant toujours sujet à controverse, et peinent à remplir leurs objectifs d'utilisation en France malgré de nombreuses campagnes et textes de lois en leur faveur. Cette étude a pour but d'analyser la perception de la population française à l'égard des médicaments génériques, et plus spécifiquement des patients en demande de non-substitution. Cette étude a été réalisée sous forme d'enquête qualitative par entretiens semi-dirigés auprès de 19 patients exigeant que soit annotée la mention « non substituable » sur leurs ordonnances. Il en ressort une inquiétude marquée, notamment en ce qui concerne les différences de composition avec le princeps. Ce doute est accentué par des sources d'information biaisées, comme Internet, les proches ou les médias. Le médecin lui-même peut être à l'origine de cette crainte par son propre manque d'adhésion au générique. Le résultat est une utilisation fluctuante du générique, dépendante de la pathologie et de la façon dont est rédigée l'ordonnance. De plus, la réglementation encadrant la prescription du générique est perçue comme de plus en plus coercitive, privant le patient de son libre arbitre. Enfin, le médicament générique lui-même est perçu comme un frein à son adhésion, par la pluralité de ses formes, couleurs, emballages, pour un seul princeps. Cette étude réaffirme le rôle central du prescripteur dans l'adhésion du patient et la nécessité d'une médecine participative.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

For more than 40years theories of human action have assumed that cogni tion and action are merely two sides of the same coin. Approaches as different as S-O-R behaviorism,social learning theory, consistency theories,and expectancy value theories of motivation and decision making have one thing in common: they all assume that "thought (or any other type of cognition) can move any thing," that there is a direct path from cognition to behavior. In recent years, we have become more and more aware of the complexities in volved in the relationship between cognition and behavior. People do not always do what they intend to do. Aside from several nonpsychological factors capable of reducing cognition-behavior consistency, there seems to be a set of complex psychological mechanisms which intervene between action-related cognitions, such as beliefs, expectancies, values, and intentions,and the enactment of the be havior suggested by those cognitions. In our recent research we have focused on volitional mechanismus which presumably enhance cognition-behavior consistency by supporting the main tenance of activated intentions and prevent them from being pushed aside by competing action tendencies.
