These appendix materials were working documents. They contain a number of conclusions and proposed recommendations, several of which were adopted by the CAIB in Vol. 1. The other conclusions and proposed recommendations drawn in Vols. 2-6 do not necessarily reflect the views of the CAIB but are included for the record. When there is conflict, Vol. 1 takes precedence. It alone is the CAIB's official statement.

Available in PDF, EPUB and Kindle. Book excerpt: First published in 2004, this volume recognises that, as society changes, so must its organisations; as organisations change, so must their management competencies. The requirement for organisations to be flexible, innovative and adaptable in environments of increasing complexity and uncertainty is also a requirement of any organisation’s most essential resource: its people. They not only work for the organisation – they are the organisation. The second edition of this highly successful book provides a comprehensive and thought-provoking examination of the operational and strategic skills, demands and responsibilities of the modern workforce, and in particular its management. Reflected here are the manager’s new and changing priorities. Attention is given to the manager’s role, the motivation to work and succeed, and teamworking. Also emphasised is the new psychological contract, highlighting reliance on self whilst maintaining sensitivity to diversity concerns. Providing clear and decisive leadership requires the projection of a vision that captures the imagination of others, but inevitably managers face conflict and adversity. Working in Organisations therefore discusses how power and politics can be moulded to positive advantage. The challenges facing organisations go beyond cost and profitability, as political and environmental challenges have forcefully entered into managerial responsibility. The book asks 'What is the ultimate purpose and contribution of organisations?' and highlights profound governance and ethics concerns. The design of organisations is also explored, and how creating appropriate structures will in turn focus resources to achieve desired ends. This book provides a broad coverage of key issues, ranging from a close examination of the manager's job to a discussion of the corporate and social forces that determine our lives. Written in an easy-to-read style and bursting with case examples, Working in Organisations acts as mentor and guide to those whose quest is for ever greater sustainable accomplishment. Written by a distinguished team of authors, this book will continue to be welcomed as the definitive text on organisational culture and change for academics, researchers and managers around the world.

Book excerpt: Humanity's first reusable spacecraft and the most complex machine ever built, NASA's Space Shuttle debuted with great promise and as a dependable source of wonder and national pride. But with the Challenger catastrophe in 1986, the whole Space Shuttle program came into question, as did NASA itself, so long an institution that was seemingly above reproach. Wheels Stop tells the stirring story of how, after the Challenger disaster, the Space Shuttle not only recovered but went on to perform its greatest missions. From the Return to Flight mission of STS-26 in 1988 to the last shuttle mission ever on STS-135 in 2011, Wheels Stop takes readers behind the scenes as the shuttle's crews begin to mend Cold War tensions with the former Soviet Union, conduct vital research, deploy satellites, repair the Hubble Space Telescope, and assist in constructing the International Space Station. It also tells the heart-wrenching story of the Columbia tragedy and the loss of the magnificent STS-107 crew. As complex as the shuttle was, the people it carried into orbit were often more so--and this is their story, too. Close encounters with astronauts, flight controllers, and shuttle workers capture the human side of the Space Shuttle's amazing journey--and invite readers along for the ride. Eisenhower signed the National Aeronautics and Space Act, creating the National Aeronautics and Space Administration (NASA), which became operational on 1 October of that year. Over the next 50 years, NASA achieved a set of spectacular feats, ranging from advancing the well-established field of aeronautics to pioneering the new fields of Earth and space science and human spaceflight. In the midst of the geopolitical context of the Cold War, 12 Americans walked on the Moon, arriving in peace “for all mankind.” Humans saw their home planet from a new perspective, with unforgettable Apollo images of Earthrise and the “Blue Marble,” as well as the “pale blue dot” from the edge of the solar system. A flotilla of spacecraft has studied Earth, while other spacecraft have probed the depths of the solar system and the universe beyond. In the 1980s, the evolution of aeronautics gave us the first winged human spacecraft, the Space Shuttle, and the International Space Station stands as a symbol of human cooperation in space as well as a possible way station to the stars. With the Apollo fire and two Space Shuttle accidents, NASA has also seen the depths of tragedy. In this volume, a wide array of scholars turn a critical eye toward NASA’s first 50 years, probing an institution widely seen as the premier agency for exploration in the world, carrying on a long tradition of exploration by the United States and the human species in general. Fifty years after its founding, NASA finds itself at a crossroads that historical perspectives can only help to illuminate.

Launched by Congress or the president, sometimes by both at the same time, the investigations at the core of this book were driven by the search for answers about significant breakdowns in government performance. Light reveals which investigations were most effective, and why.

Book excerpt: The #1 New York Times bestseller that has all America talking—with a new afterword on expanding your range—as seen on CNN's Fareed Zakaria GPS, Morning Joe, CBS This Morning, and more. “The most important business—and parenting—book of the year.” —Forbes “Urgent and important. . . an essential read for bosses, parents, coaches, and anyone who cares about improving performance.” —Daniel H. Pink Shortlisted for the Financial Times/McKinsey Business Book of the Year Award Plenty of experts argue that anyone who wants to develop a skill, play an instrument, or lead their field should start early, focus intensely, and rack up as many hours of deliberate practice as possible. If you dabble or delay, you’ll never catch up to the people who got a head start. But a closer look at research on the world’s top performers, from professional athletes to Nobel laureates, shows that early specialization is the exception, not the rule. David Epstein examined the world’s most successful athletes, artists, musicians, inventors, forecasters and scientists. He discovered that in most fields—especially those that are complex and unpredictable—generalists, not specialists, are primed to excel. Generalists often find their path late, and they juggle many interests rather than focusing on one. They’re also more creative, more agile, and able to make connections their more specialized peers can’t see. Provocative, rigorous, and engrossing, Range makes a compelling case for actively cultivating inefficiency. Failing a test is the best way to learn. Frequent quitters end up with the most fulfilling careers. The most impactful inventors cross domains rather than deepening their knowledge in a single area. As experts silo themselves further while computers master more of the skills once reserved for highly focused humans, people who think broadly and embrace diverse experiences and perspectives will increasingly thrive.

Available in PDF, EPUB and Kindle. Book excerpt: NASA commissioned the Columbia Accident Investigation Board (CAIB) to conduct a thorough review of both the technical and the organizational causes of the loss of the Space Shuttle Columbia and her crew on February 1, 2003. The accident investigation that followed determined that a large piece of insulating foam from Columbia's external tank (ET) had come off during ascent and struck the leading edge of the left wing, causing critical damage. The damage was undetected during the mission. The Columbia accident was not survivable. After the Columbia Accident Investigation Board (CAIB) investigation regarding the cause of the accident was completed, further consideration produced the question of whether there were lessons to be learned about how to improve crew survival in the future. This investigation was performed with the belief that a comprehensive, respectful investigation could provide knowledge that can protect future crews in the worldwide community of human space flight. Additionally, in the course of the investigation, several areas of research were identified that could improve our understanding of both nominal space flight and future spacecraft accidents. This report is the first comprehensive, publicly available accident investigation report addressing crew survival for a human spacecraft mishap, and it provides key information for future crew survival investigations. The results of this investigation are intended to add meaning to the sacrifice of the crew's lives by making space flight safer for all future generations.

Available in PDF, EPUB and Kindle. Book excerpt: This book presents a comprehensive analysis of the contentious relationship between the White House and the scientific community from the FDR administration to the Obama administration. Toxic Mix?: A Handbook of Science and Politics takes a topic very much in the center of public debate in the last decade and places it in a revealing historical context. It follows the often contentious relationship of science and politics from the FDR era to the current Obama administration, highlighting the many highly charged moments when the two were in conflict. Toxic Mix? ranges across the major areas of scientific inquiry with public policy implications, including atomic energy, space science, public health, stem cells, sexual reproduction, environmental science, global warming, and evolution, to examine important events where political imperatives and scientific research were at odds. In addition, the book looks at another important area where politics and science are at cross purposes; immigration—as many of our most accomplished degree earners are foreign born and are unable to stay and work in the United States. A final chapter analyzes the attempts by the early Obama administration to build public policy that embraces science rather than manipulates it.
