Available in PDF, EPUB and Kindle. Book excerpt: The increase in average life expectancy observed over recent decades has brought new challenges to nephrology practice. Several renal diseases are more frequent in elderly patients today, and even in healthy older individuals renal physiology has its own peculiarities. This leads to the need for a specific approach on renal aging and renal function in the elderly. However, despite the relevance of this topic, there are few books exclusively dedicated to geriatric nephrology. This book fills this gap by presenting a comprehensive overview of the differences between normal renal aging and chronic renal disease in the aged, and describes the particular aspects of nephropathy in the elderly, addressing it in a practical and clinically applicable fashion. Clinical Nephrogeriatrics bridges the gap between nephrology and geriatrics, discussing the most important issues concerning the relationship between these two medical specialties. This evidence-based resource, written by prominent specialists with extensive clinical experience, offers nephrologists, geriatricians, family physicians, and other medical professionals valuable practical guidelines and insights to enhance their clinical practice and provide optimal care.

Available in PDF, EPUB and Kindle. Book excerpt: This is an original work dedicated to an important and current topic in nephrology: organ crosstalk in acute renal injury. Acute kidney injury is a prevalent and serious syndrome. The related mortality to this syndrome has not been significantly reduced in the last decades despite the advances in renal replacement therapy technology. For this reason, a new approach to obtain an effective treatment is urgently needed. The organ crosstalk perspective could be useful in order to achieve this objective since it implies a very early acute kidney injury diagnose and treatment. This book includes a comprehensive review of the organ crosstalk perspective in acute kidney injury. Organized in two parts, the chapters in the first part present general subjects as an introduction to analysing the crosstalk concept. These chapters give an overview of the concepts of biosemiotics, hormones, neurotransmitters, cytokines, signalling and adhesion molecules, organ intercommunication, and epigenetics. The second part gathers chapters on the different kidney crosstalk, such as brain, lung, heart, liver, gut, and placenta. Finally, there is a chapter dedicated to the influence of renal replacement treatment on kidney crosstalk. Since acute kidney injury is a topic of interest to many health professionals, this work will be a useful resource to general practitioners, internal medicine physicians, geriatricians, nephrologists, critical care therapists, immunologists, and also biologists and bioengineers.

Book excerpt: This unique book reviews the information available in the literature regarding the new syndrome, frailty, in patients with various renal conditions, such as acute kidney injury, chronic kidney disease, as well as dialysis and transplant patients. The topic is of importance in nephrology, specifically nephrogeriatrics, since frailty is a condition affecting many elderly patients and which is becoming increasingly common in medical practice. As such, there is a considerable need for information to assist professionals treating these patients. The book includes chapters on the frailty syndrome (definition, evaluation and treatment), the main geriatric syndromes (gait disorder, falls, incontinence, and delirium), the main renal syndromes (acute renal injury, chronic kidney disease) as well as dialysis and kidney transplant, and the relationship between geriatrics and renal syndromes. Frailty and Kidney Disease: A Practical Guide to Clinical Management is an essential resource for general practitioners, researchers, internal medicine physicians, geriatricians, and nephrologists.

Book excerpt: Controversies and trends have been a constant presence in nephrology. Likewise, since its inception and throughout its development, renal replacement therapy has been shrouded in an atmosphere of uncertainty, always with an eye on the future. Renal Replacement Therapy: Controversies and Future Trends is a collection of independent chapters written by eminent authors, focused on topics relevant to modalities of renal replacement therapy. This book addresses controversies and future trends in peritoneal dialysis, hemodialysis, and kidney transplantation. It covers issues from kidney patient education to new technologies in end stage renal disease management, including trends in arteriovenous access, convective therapies, the concept of incremental hemodialysis, fluid overload in hemodialysis and iron therapy. Regarding renal transplantation, it discusses a variety of subjects, highlighting xenotransplantation, organ bioengineering, immunosuppressive therapies, and living donor bioethics issues. This book may appeal to nephrologists and physicians involved with renal replacement therapy and all those interested in the advances and challenges still facing medicine. It is, at the same time, a didactical textbook providing valuable and updated medical information and also represents the current thinking of great authors of our time on the subject. To a certain extent, it will also have historical value and may be considered a landmark of ideas in the field of nephrology.

Available in PDF, EPUB and Kindle. Book excerpt: The year was 1943. As a third-year medical student at Stanford, I was about to witness the beginning of a medical miracle. Dr. Arthur Bloomfield, Professor of Medicine, had selected my patient, a middle aged man, who was dying of acute pneumococcal pneumonia, as one of the first patients to receive miniscule doses (by today's standards) of his meagre supply of a new drug - penicillin. The patient's response amazed everyone especially this impressionable medical student. The rest of the story is history. With one stroke, the introduction of penicillin removed from the medical scene the 'friend of the aged' - lobar pneumonia. The consequences, which no one could have imagined at the time, are still becoming manifest as other 'miracles' such as respirators, artificial kidneys and many potent new antibiotics have come upon the scene. All of us are aware that these miracles have created a variety of new challenges around the states of dying and near dying. We have no easy answers for these problems. Nevertheless as dialysis techniques, especially CAPD, are applied more widely to the treatment of the elderly, the task of helping the patient meet death with dignity becomes increasingly important and vexing because once begun, dialysis is difficult to terminate.
