Book excerpt: Objective: This multicentric study aimed to evaluate the feasibility of sentinel lymph node (SLN) mapping in clinically uterine-confined endometrial cancer. Methods: Patients who underwent primary surgery for endometrial cancer with SLN algorithm were reviewed. Indocyanine green (ICG) or blue dye was used as tracer. SLNs and/or suspicious lymph nodes were resected. Side specific lymphadenectomy was performed when mapping was unsuccessful. SLNs were ultra-staged on final pathology.Results: 357 eligible patients were analyzed. The median age was 59 years. The median resected SLN number was 2 (range 1-12) per patient. Minimal invasive and open surgeries were performed in 264 (73.9%) and 93 (26.1%) of the patients, respectively. ICG was used in 231 (64.7%) and blue dye in 126 (35.3%) patients. Dyes were injected into the cervix in 355 (99.4%) patients. In 71 (19.9%) patients only SLN biopsy was done whereas 169 (47.3%) underwent pelvic and 117 (32.8%) underwent pelvic and paraaortic lymphadenectomy. The overall, unilateral and bilateral SLN detection rates were 91.9%, 20.5% and 71.4%, respectively. The mapping rates by using ICG or blue dye were comparable (p=0.526).There were 43 (12%) patients with lymphatic metastasis. SLN algorithm were not able to detect 3 of 43 metastatic patients who had isolated paraaortic metastasis. Negative predictive value, sensitivity and false negative rate were 98.80% (CI: 96.51-99.59), 92.5% (CI: 79.61-98.43) and 2.6%, respectively. The risk of non-SLN involvement in patients with macrometastatic SLN, micrometastatic SLN and isolated tumor cells in SLN were 61.2%, 14.3% and 0%, respectively.Conclusions: SLN biopsy has high accuracy in detecting lymphatic metastasis. However, one third of the cases with metastatic SLN(s) also had non-SLN involvement and this risk increases to two third of the cases when SLN has macrometastasis. The effect of leaving these nodes in situ on survival should be evaluated in further studies.

Book excerpt: Introduction: Lymphatic drainage pathways in vaginal cancer depend on the localization of the tumor. The upper two thirds of the vagina usually drain primarily to deep pelvic nodes, the lower one-third drains into the inguinal and femoral nodes (1). However, some studies documented large variance of the lymph drainage (2). The recomended surgical approach for distal vaginal lesions involved radical resection of infiltrated vagina to achieve adequate negative margins and inguinal lymph node dissection.Results: 72 year-old woman has been referred to University hospital of Ostrava with histologically confirmed adenocarcinoma of the vagina. Gynaecological examination revealed the exophytic tumor size of 3x1.5x1.5 centimeters involving lower posterior wall of the vagina. The distance from the vaginal entrance was 1 centimeter. Ultrasounography detected vascularised exophytic tumor of distal part of vagina with no spread to the rectal wall or lymfadenopathy in the groins and in the pelvis. Surgical treatment has been recommended. The radiolabeled technetium-99 m colloid has been injected in four quadrants around the lesion directly into the junction of tumor and normal vaginal mucosa two hours before surgery. Following lymphoscintigraphy showed no activity in the groins but detected sentinel lymph nodes in pelvis. In operating room the isosulfan blue has been injected in the same manner. Pelvic sentinel nodes were detected by gamma counter and blue dye and removed by laparoscopic approach. After systematic pelvic lymphadenectomy the radical excision of vaginal tumor with clear margin was performed. Histological findings confirmed adenocarcinoma of vagina, clear margins of excision and no involvement of lymph nodes. Conclusion:The lymph drainage of the vagina has a large variance. Preoperative lymphoscintigraphy is able to identify the nodes most at risk for metastatic disease (the sentinel nodes) and helps to choose optimal surgical approach in treatment of vaginal cancer patients.References:1.tHacker NF.
