Book Synopsis A Human Error Approach to Aviation Accident Analysis by : Douglas A. Wiegmann
Download or read book A Human Error Approach to Aviation Accident Analysis written by Douglas A. Wiegmann and published by Routledge. This book was released on 2017-12-22 with total page 174 pages. Available in PDF, EPUB and Kindle. Book excerpt: Human error is implicated in nearly all aviation accidents, yet most investigation and prevention programs are not designed around any theoretical framework of human error. Appropriate for all levels of expertise, the book provides the knowledge and tools required to conduct a human error analysis of accidents, regardless of operational setting (i.e. military, commercial, or general aviation). The book contains a complete description of the Human Factors Analysis and Classification System (HFACS), which incorporates James Reason's model of latent and active failures as a foundation. Widely disseminated among military and civilian organizations, HFACS encompasses all aspects of human error, including the conditions of operators and elements of supervisory and organizational failure. It attracts a very broad readership. Specifically, the book serves as the main textbook for a course in aviation accident investigation taught by one of the authors at the University of Illinois. This book will also be used in courses designed for military safety officers and flight surgeons in the U.S. Navy, Army and the Canadian Defense Force, who currently utilize the HFACS system during aviation accident investigations. Additionally, the book has been incorporated into the popular workshop on accident analysis and prevention provided by the authors at several professional conferences world-wide. The book is also targeted for students attending Embry-Riddle Aeronautical University which has satellite campuses throughout the world and offers a course in human factors accident investigation for many of its majors. In addition, the book will be incorporated into courses offered by Transportation Safety International and the Southern California Safety Institute. Finally, this book serves as an excellent reference guide for many safety professionals and investigators already in the field.

Available in PDF, EPUB and Kindle. Book excerpt: Australia has an enviable record for airline safety - No one has ever died in an accident involving a commercial jet aircraft in Australia. The reasons behind this have been the source of much speculation and theories tend to focus on issues related to the natural environment and even luck. However, with human error being present in arguably 100% of aircraft accidents, it seems reasonable that a good safety record is at least partly the consequence of human intervention. This text uses Australian aviation as a case study of a safe system to explore the interactions between the natural, operational and human environments. Based on doctoral research including a major survey of pilot and air traffic controller perceptions, the book is unusual in that it looks at positive examples in safety rather than taking the traditional reactive approach to safety deficiencies.

Download or read book TERI Energy & Environment Data Diary and Yearbook (TEDDY) 2016/17 written by TERI and published by The Energy and Resources Institute (TERI). This book was released on 2018-03-22 with total page 289 pages. Available in PDF, EPUB and Kindle. Book excerpt: TERI Energy & Environment Data Diary and Yearbook (TEDDY) is an annual publication brought out by The Energy and Resources Institute (TERI) since 1986. It is the only comprehensive energy and environment yearbook in India that provides updated information on the energy supply sectors (coal and lignite, petroleum and natural gas, power, and renewable energy sources), energy demand sectors (agriculture, industry, transport, household), and local and global environment sectors (environment and climate change). The publication also provides a review of the government policies that have implications for the sectors of the Indian economy. In TEDDY, an account of India’s commercial energy balances is given, which provide comprehensive information on energy flows within different sectors of the economy and how they have been changing over time. These energy balances and conversion factors are a valuable ready reckoner for researchers, scholars, and organizations working in the energy sector. After the introductory chapters, for the ease of readers, TEDDY has been divided into sections on energy supply, energy demand, and local and global environment. Interactive graphs, figures, maps, and tables have been used throughout the chapters to explain facts, which make the book an interesting read. In addition, detailed tables at the end of each chapter represent statistical data on each of the above-mentioned sectors. The publication is accompanied by a complimentary CD containing full text. The publication has more than 15,000 readers across the globe and is often cited in international peer-reviewed journals and policy documents.

Download or read book Rational Accidents written by John Downer and published by MIT Press. This book was released on 2024-01-16 with total page 341 pages. Available in PDF, EPUB and Kindle. Book excerpt: An unflinching look at the unique challenges posed by complex technologies we cannot afford to let fail—and why the remarkable achievements of civil aviation can help us understand those challenges. Nuclear reactors, deep-sea drilling platforms, deterrence infrastructures—these are all complex and formidable technologies with the potential to fail catastrophically. In Rational Accidents, John Downer outlines a new perspective on technological failure, arguing that undetectable errors can lurk in even the most rigorous and “rational” assessments of these systems due to the inherent limits of engineering tests and models. Downer finds that it should be impossible, from an epistemological viewpoint, to achieve the near-perfect reliability that we require of our most safety-critical technologies. There is, however, one such technology that demonstrably appears to achieve these “impossible” reliabilities: jetliners. Downer looks closely at civil aviation and how it has reckoned with the problem of failure. He finds that the way we conceive of jetliner reliability hides the real practices by which it is achieved. And he shows us why those practices are much less transferrable across technological domains than we are led to believe. Fully understanding why jetliners don't crash, he concludes, should lead us to doubt the safety of other “ultra-reliable” technologies. A unique and sobering exploration of technological reliability from an STS perspective, Rational Accidents is essential reading for understanding why our most safety-critical technologies are even more dangerous than we believe.

Download or read book High Frontier written by William F. Trimble and published by University of Pittsburgh Pre. This book was released on 2010-11-23 with total page 359 pages. Available in PDF, EPUB and Kindle. Book excerpt: From the early days of hot air ballooning to supersonic aircraft, High Frontier chronicles the history of flight in Pennsylvania. Early experimentation with lighter-than-air craft in the nineteenth century was followed by significant advances in aerodynamics, the advent of the airplane, and its gradual acceptance by the public. The state had its own contingent of inventors and aviators, who flew and crashed their homemade machines in countless exhibitions. After World War I commercial flights took wing, including government airmail delivery, and expanded airports, federal and state regulation of aeronautics laid the groundwork for the growth of the industry.
