Book excerpt: Objectives: To determine the skeletal and dentoalveolar effects produced by the MARA functional appliance in the treatment of growing patients with Class II malocclusions. Materials and Methods: A retrospective study was conducted using lateral cephalograms of patients consecutively treated with the MARA appliance (n=40) during their skeletal growth spurt as evaluated by the improved cervical vertebral maturation method recommended by Baccetti et al. A comparison was made with 24 untreated Class II control subjects obtained from the University of Michigan growth study and matched with the experimental groups for skeletal age, sex and craniofacial morphology. Cephalograms were taken at three time points: [T1] pre-treatment; [T2] post-functional appliance treatment; and [T3] fixed orthodontic treatment completion. Treatment changes were evaluated between the time points using 35 variables. Data were analyzed using independent t-tests. Results: At the post-functional appliances' phase (T1-T2), the MARA appliance showed significant increases in the total mandibular length, ramus height and anterior/posterior facial heights. These effects continued during the fixed orthodontic treatment stage (T2-T3). The net changes (T1-T3) revealed significant mandibular growth enhancement with the MARA appliance (+2. 7mm) in comparison to untreated controls. The appliance also caused 5. 2o of flaring in the mandibular incisors, as well as significant decreases in overjet, overbite and inter-incisal angle. Conclusion: Overall, the MARA appliance showed significant skeletal and dentoalveolar changes resulting in normalization of the Class II malocclusion.

The experimental group (n = 30) required maxillary expansion to correct the deficiency in the palatal, and the control group (n = 30) had fixed orthodontic treatment only. Subjects age, gender, predominant mode of breathing, length of treatment and initial and final measurements of upper and lower pharyngeal airway were recorded. Results using Student t-test showed that age and gender were no statistically significant difference between the two groups (p = 0.2810). Mouth breathers distribution showed no statistically significant difference between the two groups (p = 0.7866). Upper pharyngeal airway increased an additional 0.77 mm in the expansion group which was statistically significant (Student's t-test, t = 2.492), while the lower pharyngeal airway showed no statistically significant difference (Student's t-test t = 1.041). In conclusion rapid maxillary expansion during orthodontic treatment had a positive affect on the upper pharyngeal airway, with no significant change on the lower airway.

Book excerpt: Transverse maxillary deficiency is a common skeletal malocclusion that is diagnosed when the maxilla is narrow in relation to the mandible. The malocclusion develops during growth and usually does not correct without treatment. Some health problems thought to be associated with a narrow maxilla include narrowing of the pharyngeal airway and the nasal cavity and increased nasal resistance. In short, problems that make it more difficult to breath. Adult patients seeking an alternative to surgical expansion can now turn to bone-borne expanders utilizing Temporary Anchorage Devices (TADs) such as the maxillary skeletal expander (MSE) which is able to orthopedically expand the maxilla transversely at any age. A preliminary study performed at UCLA suggested that patients treated with MSE had improvement in airway volume and airflow immediately following expansion. This study investigated the effects of MSE on airway improvement using three-dimensional cone beamed computed tomography (CBCT) to measure volume changes in the upper airway and a computational fluid dynamic model (CFD) to evaluate the changes in airflow for sixteen patients at the UCLA Orthodontics Clinic at two timepoints: pre-expansion (T0) and post-expansion (T1). Treatment with the MSE caused a statistically significant increase in the volume of the airway after expansion as compared with the control group. Furthermore, CFD analysis showed that treatment with the MSE caused a statistically significant reduction in the airway resistance. The airway resistance of the MSE group was no longer statistically different from the control group after expansion. There was no correlation between the volume increase and the decrease in airway resistance. Overall, there was a significant increase in total airway volume, oropharyngeal volume, nasopharyngeal volume, and nasal cavity volume with MSE treatment immediately after expansion, but there was no correlation between volume increase and the improvement in breathing metrics, namely airway resistance. These results suggest that treatment of maxillary constriction using the MSE appliance may show positive effects in improvement of the upper airway volumes and reduction of the upper airway resistance.
