Book excerpt: The purpose of this study is to identify the needs of older adults with Alzheimer's disease (AD) and related dementias (ADRD) admitted to a rehabilitation setting where they are expected to physically and mentally function to their optimal level of health. To date, no studies have identified the needs and concerns of ADRD patients in rehabilitation settings. The Needs-Driven Dementia-Compromised Behavior (NDB) Model, the researcher's clinical experience, and the state of the current scientific literature will help guide the study. An exploratory qualitative research approach was employed to gather data and discover new information about the ADRD patient's needs and related behavioral outcomes. The qualitative findings on the discrepancies and similarities in perceptions of ADRD patient needs were obtained by examining formal and informal caregivers' perceptions. The researcher recruited registered nurses and certified nurse assistants (RNs and CNAs, formal) and family/friends (informal) who have provided care to patients in inpatient rehabilitation facilities to participate in focus groups and individualized focused interviews. The data were collated and analyzed using a thematic analysis approach. The overarching theme that developed as a result of this approach revealed discordant perceptions and expectations of ADRD patients' needs between the formal and informal caregivers with six subthemes: communication and information, family involvement, rehabilitation nurse philosophy, nursing care, belonging, and patient outcomes. The researcher provided recommendations to help support these needs. These findings will help guide the development of nurse-lead interventions for ADRD patients in a rehabilitation setting.

Book excerpt: The study included 100 participants who were caregivers to patients who were diagnosed with Alzheimer's disease within the last year. Participants included 88 females and 12 males who were at least 18 years old. Participants were given versions of the CERAD and the BDS that were modified to be appropriate for caregiver ratings and endorsed the frequency of behavioral changes that occurred within the last month. A new version of the CERAD was developed made up of 29 items that allowed for an online administration. Results of the study suggested that the most frequently observed behavioral changes were behaviors associated with difficulty in emotional regulation. Behavioral changes associated with a distorted sense of reality were also frequently endorsed. While this finding appears to be unique in the literature it is possible that it was due to the assessments being modified and available to be completed online rather than an office setting. Implications for clinical care and future research are discussed.

Book excerpt: Increases or decreases in the frequency of problem behavior in the early evening hours, have often been reported in individuals with Alzheimer's disease and related dementias (ADRD). Reports of changes in problem behavior during later hours of the day are so common that some investigators refer to the collection of symptoms as "sundowning." To date, reports of sundowning tend to be based on subjective reports rather than empirical evidence, thereby provoking questions about the validity of predictors such as time of day, ongoing activities, or other variables present in the environment. The purpose of this study was to define better a possible temporal relation between wandering in the context of sundowning syndrome in individuals with Alzheimer's-related dementia, through the utilization of a scatterplot system that includes data collection by time of day. Using actigraphy devices and in situ behavior data collection, researchers observed that, for four women diagnosed with Alzheimer's disease, two exhibited patterns of wandering that was perhaps consistent with sundowning while at an adult day facility. Furthermore, when data were collected for 24 hr for one participant, findings remained consistent across all hours of the day and days of the week. Findings indicate the utility of the data and are discussed in terms of potential non pharmacological interventions to improve quality of life for individuals with ADRD, while reducing stress on caregivers to promote aging in place.
