Abuse of alcohol and drugs is related to numerous other health problems, such as the spread of acquired immune deficiency syndrome (AIDS) on all continents. Health and social services today cannot afford to ignore this crisis since it affects all levels of society and manifests itself in diverse health and social problems. In recent years, the World Health Organization has received numerous requests for training material for physicians in this field. This manual therefore meets an urgent need. The availability of new data and the dearth of suitable textbooks have made its preparation mandatory. The influence of sociocultural factors on drug dependence and alcohol related problems-on their cause, development, and consequences as well as on their treatment and prevention-has been taken into account in the preparation of this manual in order to ensure that its usefulness is not limited to one country or region. It has been prepared primarily for the teaching of physicians and medical students, although much of it is relevant to the training of nurses, midwives, health educators, primary-care workers, medical social workers, counselors, and psychologists. In fact, suggestions have been included for adapting the manual for use in the training of such varied groups of students.

Book excerpt: This book reports the findings of a study of the treatment of alcoholism in the out-patient clinics and the related in-patient facilities of state-supported alcoholism programmes in the United States. The authors compared a number of clinics simultaneously, and were thus able to investigate the influence of a variety of treatment programmes on a variety of patients. They show that clinics play a valuable role in assisting patients who have retained social stability despite their problem by maintaining contact with such patients, but that they are rarely useful for modifying either drinking habits or other aspects of malfunctioning in the case of patients whose social stability has crumbled. The study further shows that improvement in drinking habits (either by abstinence or by controlled drinking) is related to what the clinic does and to changes in the patient's social and interpersonal environment outside the clinic.

Book excerpt: The introduction of chlorpromazine in 1953, and haloperidol in 1958, into clinical practice dramatically altered the therapy of schizophrenic patients. Although representing by no means a cure for this severe psychiatric ill ness, it allowed, for the first time, to adequately control the severe hallu cinations and delusional beliefs which prevent these patients from leading a more or less independent life. Indeed these antipsychotics (and the many congeners that were to follow) significantly reduced the number ofchronic schizophrenic inpatients in psychiatric clinics all over the world. However soon after their introduction it became clear that, like all other available drugs, antipsychotics were by no means miracle drugs. In fact, two major problems appeared. First, the antipsychotics had very little effect on the so-called negative or defect symptoms, like social isolation, apathy and anhedonia, and secondly virtually all antipsychotics produced a number of side-effects, of which the neurological (often called extra pyramidal) side-effects were the most troublesome. Especially the tardive dyskinesia, which occurred in about 15 to 20% of the patients after pro longed treatment, represented a major problem in the treatment of schizo phrenic patients.
