Book excerpt: Since September 11, 2001, and the subsequent anthrax attacks, the use of tabletop exercises in public health for emergency preparedness assessment and emergency response training has increased significantly. The evidence base for these exercises, however, remains sparse and the quality of many of these exercises is poor due to insufficient beta testing. Most exercises focus on training and few provide public health agencies (PHAs) with tools to assess exercise performance. This narrow focus limits the ability of PHAs to use tabletop exercises as part of an overall continuous quality improvement effort. In 2003, the U.S. Department of Health and Human Services Office of the Assistant Secretary for Public Health Emergency Preparedness contracted the RAND Corporation to develop and beta test a suite of tabletop exercises that focus on the response of local PHAs (LPHAs) to outbreaks caused by bioterrorism in the first few hours to days of the response. RAND developed the tabletop exercises described in this manual as templates that LPHAs can customize and use to train public health workers in how to detect and response to bioterrorism events and to assess LPHAs' levels of preparedness over time. They were beta tested and refined in 13 LPHAs across the United States over 10 months.

Policymakers recognize the need to forge relationships and coordinate preparedness planning efforts at the local, state, national, and international levels. However, there is little consensus about the optimal level of localization or regionalization for each of the resources and services that must be operationalized during a bioterrorism response. We sought to evaluate the evidence regarding the effectiveness of existing regional systems that facilitate a response to bioterrorism. We sought evidence regarding the tasks that would need to be performed during a bioterrorism response (such as triage, provision of emergency medical care, transportation, and surveillance) and regionalized organizations that would likely contribute personnel, material, and information required to perform these bioterrorism response tasks. The Key Questions addressed in this Report are: 1. What are the key tasks of local responders—such as local public health officials, clinicians, and emergency medical personnel—during a bioterrorism event? 2. What resources do local responders require to perform the tasks identified in Key Question 1? 3. Which existing regional systems for delivery of goods and services could be relevant to supplying the resources identified in Key Question 2? 4. Can regionalization of bioterrorism preparedness planning facilitate supplying needed resources to local responders during a bioterrorism event? 5. How do geographic variations in the affected population (e.g., urban as opposed to rural), special populations, and the interplay of private and public sector players affect regionalized systems?

The nation's ability to respond to a bioterrorist attack, therefore, depends crucially on the state of preparedness of its medical care systems and public health infrastructure. Public health experts have for years complained about the deterioration of the public health system through neglect and lack of funding. They warn that the nation is ill equipped and insufficiently prepared to respond to a bioterrorist attack. For example, they point out that there are too few medical personnel trained to spot biological attacks, a shortage of sophisticated laboratories to identify the agents, and inadequate supplies of drugs and vaccines to counteract the threat.
