These are some of the questions that the health care industry has been trying to understand. Communication among staff is extremely important. The purpose of the shift report is to transfer responsibility from one nursing shift to the other, providing precise details of the patients' state of health and current physical condition (Wildner and Ferri, 2012). Without accurate shift report nurses may miss valuable information leading to adverse effects. With the implementation of bedside shift report it has been shown to improve how patients perceive their quality of care. In one study patient and family members felt they were listened too, were taken into account, were better informed, concerns were dealt with immediately, and thus patients felt an increase in sense of safety (Wildner and Ferri, 2012). The implementation process for incorporating bedside shift report will be a conjoined effort. In order to ensure bedside shift report does indeed improve how patients perceive their quality of care an evaluation plan must occur. The method used in this study is a patient survey (Appendix A) and to view the improvement in patients' quality of care through hospital consumer assessment of healthcare providers and system (CAHPS) hospital survey scores (Appendix B). Once these methods are viewed at the three month mark and then again at the six month mark dissemination of results can take place with upper management and staff.

Some of these errors are accounted for through miscommunication during the change of the nurse shift report. One of the Joint Commission’s 2019 national patient safety goals for hospital programs was to improve communication effectiveness among caregivers since mistakes in a caregiver’s communication accounted for 37% of severe patient injuries. Safety is threatened by patients not being included. Therefore, adopting the bedside shift report allows patients to be actively engaged, reinforcing nurse-patient communication. The bedside shift report enables nurses to provide safe, high-quality, patient-centered care at the bedside. At the time of this project, a 24- cardiac bed unit at a community hospital in Louisiana lacked a bedside shift report. Surveillance of the nurse shift report at the nurses’ station unveiled countless communication gaps. The Hospital Consumer Assessment of Healthcare Providers and Systems survey measures patient perception of their hospital experience. The scores from April 2019 revealed a need for improvement in nurse-patient communication at the unit level. The data were compared for the communication with nurse dimension of the Hospital Consumer Assessment of Healthcare Providers and Systems scores preimplementation of the bedside shift report from April 2020 (July 2018–June 2019 discharges), with survey scores postimplementation of the bedside shift report from April 2021 (July 2019–June 2020 discharges), using the longitudinal data analysis. The four specific questions in the Hospital Consumer Assessment of Healthcare Providers and Systems survey related to communication with the nurse and comparing data preimplementation and postimplementation of the bedside shift report were examined. This quality improvement research project aimed to assess the bedside shift report on patient satisfaction in care delivery for the communication with nurse dimension in the Hospital Consumer Assessment of Healthcare Providers and Systems survey. Furthermore, this reporting method improves the patient's experience of care, promotes nurse and patient engagement, increases time spent providing direct care, improves communication, and enhances nursing practice. Furthermore, research shows that bedside shift report allows the oncoming nurse to visualize the patient, perform a quick assessment, answer patient questions, confirms the plan of care, and establishes patient goals for the shift. This new method of reporting will improve nursing practice, outcomes, and patient satisfaction.

Book excerpt: ABSTRACT Patient safety is a very broad and general term used to ensure that all aspects of patient care, from medical and nursing services to social and spiritual, are provided with the best interest of the patient. The aim statement for the project is improving patient safety by ensuring the implementation of bedside shift report by 80% of nurses by August 2016. Kurt Lewin's change theory is going to be used to effect the change. There were in-services given to nurses on the floor and the project was also discussed in staff meetings. Flyers were posted around the units to help build up and maintain enthusiasm for the implementation of BSR. Implementation was initiated behind schedule due to conflict in schedules and bureaucratic hoops. The implementation was started on April 28, 2016 and so there are no exact numbers at this time because there is a two week trial period. But I have had conversations with the some nurses that have started the project since initiation. So far they seem to be very receptive of the change. Most of the patients have been neutral at this time but we'll see if that's going to be the case in two weeks. There are numerous publications that support the efficacy of bedside shift report in relation to patient care and safety. Publications from the Agency for healthcare research & quality (AHRQ), MEDSURG Nursing, American nurse today, Journal of Clinical Nursing and the Nursing journal are some of the articles that were used to investigate and develop ideas and policies for the project. Patient experience with the bedside shift report (BSR) will be tracked and the data collected will be used to improve the process in order to enhance the patient experience. Patient experience would include if the patient felt more engaged in their care, if they felt safer and had more confidence in the personnel helping out with the care. Also nurse satisfaction is another benchmark that would be measured by sending out surveys periodically in an effort to gauge the opinions of the staff members. The data collected from the surveys will be used to develop improvements to the process and increase the comfort level of the nurses in giving BSR. We would also track if the nurses feel more accountable for the care they delivered during the shift and if the accountability translates to nurses being more responsible in the care that they deliver. Periodic collection of data will be used to monitor patient satisfaction and nursing compliance. The data would be monitored by the appointed BSR champions because they would be able to monitor and collect data on compliance of the floor nurses in real time and compare it with data collected from patient satisfaction surveys. There are going to be bi-weekly meetings to analyze and improve the process by applying PDSA cycle. Analysis of the data can also help to recognize fallouts earlier and provide opportunity to make corrections. In conclusion, sustainability is what is important. One of the ways I recommend to ensure continuity of the project by making sure that the new employees, regardless of experience, are trained during their eight to twelve week floor orientation. After conducting a failure mode effect analysis coupled with experience, I concluded that continuity is also dependent on proper training of new employees. I concluded this because nursing is a high turnover profession and if the current nurses don't teach the new nurses before they leave the facility then the whole process will have to be restarted.

In a study, there were increased nurses' satisfactions, accountability, patient involvement as well as decrease in 35% in falls and 50% decrease in medication errors. Pre/post implementation surveys are given to nurses in order to pinpoint barriers and revise the process. Nurse managers, administration and staff nurses are the recipients of disseminating evidence to enhance bedside reporting and continue ongoing evaluations.

The goal of this report is to provide information in a standardized format that allows accurate data to be transferred to the oncoming nurse in a clear, concise manner. Literature reveals this report occurring at the patients is the best nursing practice; however, it does come with obstacles, competing priorities, and distractions. A breakdown in communication can cause disruptions to care, inaccurate information, or even harm and death to the patient in certain circumstances. Identifying a standardized approach to bedside report is a priority for patient safety and the hospital’s safety culture. The Nurse Knowledge Exchange Plus (NKE plus) created by Kaiser Permanente was designed with a multifaceted team and front-line nursing staff to engage the team in helping break down the obstacles and distractions. NKE plus was chosen as the standardized tool for beside reporting for this project because of the engagement of staff, ability to work through barriers and distractions, and provide a comprehensive tool that uses the electronic health record, safety checks, and inclusion of the patient and family (Lin, Heisler, Fahey, McGinnis, & Whiffen, 2015). This evidence-based quality improvement project looked at the nurse’s perceptions of bedside reports and Press Ganey’s communication with nurse's scores for improvements.The results of this Doctorate of Nursing Practice (DNP) project did not support the evidence that NKE plus improved nurses’ perception of bedside reports or improved communication with nurses’ scores. However, several areas did show statistical significance. Those areas included: (a) the nurse I follow from a previous shift completes job responsibilities; (b) interpersonal relationships between shifts are good; (c) the patient’s condition matches what I get in the report; and (d) the current system of report fosters a partnership between the nurse, patient, and families. The results were skewed due to a lack of surveys for both the nursing questionnaire and patient surveys. Further dissemination of this project includes completing NKE plus throughout the other three units within the hospital because of the evidence supporting including patient and family during bedside report.

Medication error rates were increasing at the project site so an evidence-based solution was sought. The purpose of this quantitative quasi-experimental quality improvement project was to determine if the implementation of the Agency for Healthcare Research and Quality's (AHRQ) Nurse Bedside Shift Report Implementation Handbook would impact medication errors when compared to current practice among adult medical-surgical patients in an acute care hospital in rural Arkansas over four weeks. The theoretical foundation for the project utilized Peplau's theory of interpersonal relationships and Kurt Lewin's change model. The total sample size was 63, n = 29 in the comparative group and n = 34 in the implementation group. Data was obtained from the electronic medical record. A chi-squared test was used to analyze the data which indicated no statistically significant difference in reducing medication errors X2 (1, N = 63) = 2.42, p = 0.120. Despite the lack of statistical significance in reducing rates of medication errors, there was a clinical significance in reducing medication errors from 6.9% in the comparative group to 0% in the implementation group. Therefore, the implementation of the AHRQ's Nurse Bedside Shift Report Implementation Handbook could aide in reducing medication errors in this population and setting. Recommendations include sustaining the project and data reanalyzed over six months to determine if the small sample size and short project timeline impacted achieving both statistical and clinical significance.

The experience of engagement is a key qualifier of the exchange between the demand (i.e. citizens/patients) and the supply process of healthcare services. To understand and detect the strategic levers that sustain a good quality of patients’ engagement may thus allow not only to improve clinical outcomes, but also to increase patients’ satisfaction and to reduce the organizational costs of the delivery of services. By assuming a relational marketing perspective, the book offers practical insights about the developmental process of patients’ engagement, by suggesting concrete tools for assessing the levels of patients’ engagement and strategies to sustain it. Crucial resources to implement these strategies are also the new technologies that should be (1) implemented according to precise guidelines and (2) designed according to a user-centered design process.
