Oftmals werden diese Instrumente nicht alle mit Gründung des Unternehmens eingeführt, sondern nach und nach implementiert. Das hat zur Folge, dass diese Instrumente und ihr Einsatz mitunter nicht optimal aufeinander abgestimmt sind, es zu Doppelerhebungen von Daten kommt oder widersprüchliche Ergebnisse geliefert werden. Gerade Steuerungsinstrumente, die auf ganzheitlichen Ansätzen beruhen, die alle Bereiche bzw. Ebenen eines Unternehmens oder alle Schritte des Produktions- bzw. Leitungsprozesses umfassen, sind dafür prädestiniert, dass sie sich in ihrem Einsatz inhaltlich bzw. operativ überschneiden. Im Idealfall können sie sich ergänzen, problematisch ist, wenn sie einander und damit allenfalls auch das Unternehmen an sich behindern. Die Balanced Scorecard als Controlling-Instrument und das EFQM-Modell als Form des Qualitätsmanagements verfolgen beide einen ganzheitlichen Ansatz und sind daher genannten Ri-siken ausgesetzt. Die vorliegende Arbeit hat daher zum Ziel, einen Überblick über Methodik, Einsatzgebiete und Nutzen dieser beiden Instrumente zu geben, in der Folge etwaige Redundanzen und Widersprüchlichkeiten aufzuzeigen und letztendlich praxisrelevante Anregungen und Erfolgsfaktoren zur einander ergänzenden und harmonisierten Anwendung der beiden In-strumente zu skizzieren. Die folgenden Ausführungen sind zum Teil allgemeiner Natur und damit prinzipiell für alle Organisationen und Unternehmen gültig. Zum größten Teil, besonders ab Kapitel 4, beziehen sie sich jedoch speziell auf Gesundheitsunternehmen und hier vorrangig – auch im Sinne der Ausbildung, im Rahmen derer diese Arbeit verfasst wird – auf Krankenhäuser.

Die Funktionsweise des Balanced Scorecard Ansatzes wird in Kapitel 3 genauer erklärt, wobei dies unter Berücksichtigung des speziellen Einsatzgebiets im Krankenhaus erfolgt. Zur besseren Veranschaulichung der Theorie wird im empirischen Teil dieser Arbeit die Einführung einer Balanced Scorecard in einem deutschen Krankenhaus geschildert. Dabei wird die Vorgangsweise in der Theorie mit der Umsetzung in der Praxis verglichen und kommentiert. Für den Erfolg der Scorecard ist, wie in Kapitel 4 gezeigt wird, das unternehmensweite Herunterbrechen auf alle Hierarchieebenen von ausschlaggebender Bedeutung. Doch auch andere Voraussetzungen müssen unbedingt erfüllt werden, um dem Schlagwort „Vorsorgen ist besser als heilen“ (Kapitel 5) gerecht zu werden. Lieber vorher über mögliche Probleme, Gefahren, Stolperfallen und Hürden Bescheid wissen, als später über Leid klagen! Weiters interessant ist der Vergleich zu traditionellen Methoden der Krankenhausführung in Kapitel 7, um festzustellen, ob und wenn ja, welchen Fortschritt die Balanced Scorecard Methode bringt. Das Großprojekt Balanced Scorecard erfordert auch eine Auseinandersetzung mit dem Thema der Datenverarbeitung, in Kapitel 8 beleuchtet. Unabdingbar ist natürlich die Auseinandersetzung mit den Stärken und Schwächen dieses neuen Führungsinstruments, zu finden am Ende dieser Arbeit.

Dies erfordert im gesamten Krankenhaus die Umgestaltung der Prozesse, um die Verweildauern zu verkürzen. Im Krankenhaus gestaltet sich dies relativ schwierig, da im Gegensatz zu vielen anderen Unternehmen die Komplexität der Arbeitsabläufe vergleichsweise hoch ist. Die Behandlung eines Patienten erstreckt sich häufig über mehrere Bereiche, wobei unterschiedliche Berufsgruppen beteiligt sind. Vielfach fehlt an diesen Stellen das Schnittstellenmanagement, welches notwendig ist, um die Behandlung optimal zu koordinieren. Dies führt auch dazu, dass die einzelnen Berufgruppen und Abteilungen autonom arbeiten, um ihre Ziele zu erreichen. Dies wiederum erschwert die Erfüllung übergeordneter Ziele, die der Erhaltung des Unternehmens Krankenhaus dienen. Ein weiterer Faktor ist, dass Patienten zu Kunden geworden sind. Patienten erwarten eine auf sie abgestimmte individuelle Behandlung. Entspricht die Behandlung nicht den Wünschen der Patienten, können sie sich bei einer weiteren notwendigen stationären Behandlung in ein anderes Krankenhaus einweisen lassen. Krankenhäuser müssen sich also zunehmend durch Identifizieren der Patientenwünsche auf dem Markt positionieren um dem stärker werdenden Konkurrenzdruck standhalten zu können. Es gilt daher, dass Krankenhäuser ihre eigenen Potenziale erkennen, darauf aufbauend eine Strategie entwickeln und diese in konkrete Maßnahmen umsetzen. Weiterhin muss die Strategie sich auf alle relevanten Bereiche ausrichten und kommuniziert werden. Die Balanced Scorecard kann durch ihre Mehrperspektivität und ihrer Mischung aus monetären und nicht- monetären Messgrößen helfen, dies zu unterstützen. Im Folgenden soll deshalb aufgezeigt werden, warum die Einführung einer Balanced Scorecard im stationären Sektor der Krankenhäuser sinnvoll erscheint.

Durch die Einführung der Diagnosis Related Groups (DRG`s), den Regelungen des Krankenhausentgeltgesetzes (KHEntgG), dem wachsenden medizinischen Fortschritt sowie den demografischen Veränderungen der Bevölkerungsstruktur werden neue quantitative und qualitative Anforderungen an die Krankenhäuser gestellt. Der wirtschaftliche Erfolg hängt somit von einem effektiven und effizienten Prozessmanagement ab. Aus diesen Gründen werden strategische Ziele, strukturelle Veränderungen der Aufbau- und Ablauforganisation, operative Entscheidungen sowie die Erfolgskontrolle dessen mehr und mehr zu einer Existenzfrage. Es gilt herkömmliche Verwaltungsmethoden und zukunftsfähige Managementinstrumente mit Hilfe eines Führungskonzeptes zu vereinen. Das Konzept der Balanced Scorecard (BSC) wird von seinen Erfindern Robert S. Kaplan und David P. Norton als auf den Umsetzungsprozess fokussiertes, strategisches Führungs- und Steuerungsinstrument propagiert (vgl. Brinkmann 2003: 947), das Produktivität und Solidarität angleichen und das Management auf eine langfristige Wertschöpfung ausrichten kann. In führenden Wirtschaftsunternehmen findet diese Methode bereits seit ca. sieben Jahren Anwendung. So stellt sich die Frage, ob sie auch für Krankenhäuser geeignet ist, die bevorstehenden Veränderungen Erfolg versprechend anzugehen. Die vorliegende Arbeit beschäftigt sich deshalb mit der Praxistauglichkeit bzw. der Eignung der Balanced Scorecard für die Belange deutscher Krankenhäuser. Zunächst wird das „Unternehmen“ Krankenhaus anhand seiner spezifischen Strukturen dargestellt. Im weiteren Verlauf werden die theoretischen Grundlagen der BSC erläutert und der potentielle Einsatz im Krankenhaus näher betrachtet.
