Trotz der langjährigen wissenschaftlichen Arbeit ist es sehr schwierig, einheitliche Aussagen über die autistische Störung zu machen. Autistische Kinder zeigen oft völlig unterschiedliche Verhaltensweisen und unterscheiden sich voneinander wie alle anderen Menschen. Sie sind nicht einheitlich zu beschreiben. Das Erscheinungsbild des kindlichen Autismus zeigt Berührungspunkte zur geistigen Behinderung. Von den autistischen Kindern sind zwei Drittel bis drei Viertel der Betroffenen im Effekt geistig retardiert. Der übrige Teil der Autisten wird als intellektuell normal eingestuft. Einige wenige der Kinder zeigen überdurchschnittliche geistige Leistungen. Zu den grundsätzlichen Schwierigkeiten der erwachsenen wie auch kindlichen Autisten zählen die Probleme im Bereich des Sozialverhaltens. Aus dem Grund ist es wichtig, daß der Autist schon im Kindesalter so früh wie möglich Hilfe bekommen und betreut wird. Nur so können sie lernen, sich im sozialen Alltag zurechtzufinden. Ihre Integration in die Gesellschaft wird in jenem Maß gelingen, wie intensiv sie im Kindes- und Jugendalter gefördert wurden. Auf der Therapie der autistischen Kinder liegt aus dem Grund der Schwerpunkt der vorliegenden Arbeit. Der erste Teil der Arbeit befaßt sich einleitend mit den Begriffsdefinitionen der Erstbeschreiber des Autismus, der Symptomatik und der Ätiologie des kindlichen Autismus. Bei der Ätiologie werde ich auf die Theorien von BRUNO BETTELHEIM, MARGARET MAHLER und NIKO und ELISABETH TINBERGEN eingehen. The changing prevalence of OME in the sample is described; evidence on a wide range of risk factors is examined; the problems of chronic perforation of the tympanic membrane, cholesteatoma, and ontological complications of surgical therapy are evaluated; and the developmental consequences of OME are traced over time. Early onset of bilateral OME is shown to have long-lasting consequences for language development, speech articulation, reading ability, and classroom behavior. The authors conclude from these results that there is an urgent need for the detection and management of early onset OME.
