Available in PDF, EPUB and Kindle. Book excerpt: Otitis media with effusion (OME) is one of the most common diseases of childhood, yet its long-term effects are only just beginning to be appreciated. Following a comprehensive review of the literature, David Chalmers and his co-authors detail a longitudinal, multidisciplinary study of over 1000 New Zealand children enrolled at three years and followed to age eleven. The changing prevalence of OME in the sample is described; evidence on a wide range of risk factors is examined; the problems of chronic perforation of the tympanic membrane, cholesteatoma, and ontological complications of surgical therapy are evaluated; and the developmental consequences of OME are traced over time. Early onset of bilateral OME is shown to have long-lasting consequences for language development, speech articulation, reading ability, and classroom behavior. The authors conclude from these results that there is an urgent need for the detection and management of early onset OME.

Because the prevalence, incidence, and management of this condition varies with age, this guideline focuses on the group at high risk for long-term effects: children ages 1-3 years. These recommendations are based on an extensive review of medical and health-related literature and expert opinion. They are given for diagnosis and hearing evaluation; control of environmental factors; and sequencing of interventions, including observation, use of antibiotics and other medications, and the appropriateness and timing of surgery.

This text details the importance of evidence-based data in interpreting the ever-enlarging body of literature on otitis media. The editors have assembled an impressive group of experts on all aspects of otitis media and addressed comprehensively many issues related to methodology, clinical management, and consequences of this disease. The eight chapters comprising the methodology section provide the necessary background and detail to allow physicians and other health professionals to understand and appreciate the value of evidence-based medicine. Updates include: the incorporation of new original research, systemic reviews, and evidence reports to existing chapters. New chapter topics include: evidence-based medicine, professional evidence reports, molecular and translational research, complementary and alternative medicine, bacteriologic efficacy of antimicrobials, vaccine prevention, international management perspectives, meta-analysis of speech and language sequelae, suppurative complications, host susceptibility to sequelae, and judicious use of systemic and topical antimicrobials. FEATURES: *Maturation of evidence-based medicine as a foundation for clinical care is reflected throughout the text. *Extensive evidence tables summarize study characteristics and quantitative outcomes for clinically relevant endpoints *Internationally distinguished contributors selected based on both their clinical expertise and their ability to write for an evidence-based text

This book was released on 2008 with total page 74 pages. Available in PDF, EPUB and Kindle. Book excerpt: Otitis media with effusion (OME) is a condition characterised by a collection of fluid within the middle ear without signs of acute inflammation. It is most common in young children, with a bimodal peak at 2 and 5 years of age. Eighty percent of children will have had a least one episode of OME by the age of 10 years. At age 7-8 years, about 8% of children will have middle ear effusions; this incidence increases in winter. The mean duration of effusions is 6-10 weeks but some cases are more persistent. OME is known to be a fluctuating condition with symptoms that vary with time and with age. The main symptom of OME is impaired hearing because the middle ear effusion causes a conductive hearing loss by reflection of the sound energy at the air-fluid interface. The diagnosis is based on suspicion of hearing loss, clinical history, clinical examination of the ears and appropriate audiometry and tympanometry. While most cases of OME will resolve spontaneously, some children will need intervention because of the effects of hearing loss. This intervention may take the form of educational and social action or the provision of a hearing aid to minimise the impact of the hearing loss. No non-surgical intervention has yet been shown conclusively to be of benefit. Surgical management usually takes the form of myringotomy and insertion of a ventilation tube (grommet), with or without adenoidectomy. Children with cleft palate are particularly susceptible to OME because of the impaired function of the Eustachian tube that results from the palatal anomaly, which in turn leads to a failure of middle ear ventilation. Similarly, children with Down's syndrome have a high incidence of OME, partly because of their impaired immunity and mucosal abnormality, with resulting susceptibility to ear infection. These groups of children need particular surveillance for OME so that proper action can be taken. This guideline attempts to bring together the evidence related to surgical management of OME in children younger than 12 years, and to advise on best and safest practice; treatment decisions need to balance potential benefits against potential risks, reflecting the fact that the degree of benefit usually depends upon the severity and persistence of the condition.

Available in PDF, EPUB and Kindle. Book excerpt: In preparing the material for this book, I took the advice of my students who generously shared their views and opinions with me. I was told that it would be preferable to have images of the various diseases with legends describing the disease. Students would be able to learn and retain the inf- mation more successfully if the material was accompanied by pictures and schematic drawings. Recent advances in technology have made it possible to photograph regions that are difcult to view with the naked eye, such as the ear, nose, throat, nasopharynx, and larynx – all the areas of otorhinolaryngology. Such an illustrated text in this feld is important and necessary for teaching purposes. In this book I tried to compile images of the basic conditions that are commonly seen in general practice and to give the reader a visual survey with a brief description of the condition. I added tables and schematic drawings in order to provide practical information. It is not the purpose of this book to be a comprehensive textbook, since many textbooks are already available with more detailed information of the conditions illustrated here. This book is primarily intended for medical students, family and general practitioners, and ENT trainees. It may also serve as basic reading material for those in allied specialties. I hope my colleagues fnd this book useful and it contributes toward their teaching purposes.

It is normal that some middle ear effusion persists after AOM, even for several weeks. This must not be interpreted as a new episode of AOM.

Written by an international authority on otitis media (middle ear problems), A Parent's Guide to Ear Tubes will help you decide if your child needs ear tubes and how to benefit most if ear tubes are placed. The book addresses, in a straightforward manner, the myriad of concerns that accompany ear problems. It is an easy read for parents offering practical information previously unavailable in one place. Most importantly, you will achieve peace of mind and a feeling of control over your child's ear problems. Need to know information is well-supported by accompanying colour illustrations. The aim of this guide is to: help your child feel great and sleep well; achieve peace of mind and a feeling of control over your child's ear problems; allow your child to progress as rapidly as possible with speech, language, and learning; reduce, or eliminate, the need for oral antibiotics by using antibiotic ear drops, when necessary; permit your child to bathe and swim without earplugs, headbands, or other water precautions; and keep the tubes functional and trouble-free for the longest time possible.
