Book excerpt: Introduction : La loi HPST en 2009 place l'éducation thérapeutique au coeur des missions des médecins généralistes. La pratique de l'ETP se développe en France depuis plusieurs années. Nous nous sommes demandé ce que cette loi avait pu changer dans la pratique des médecins généralistes quels étaient les réalités sur le terrain, leurs difficultés, leurs besoins face à cette mission devenue une priorité en matière de santé publique. Méthode : Une étude qualitative par entretiens semi-directifs a été réalisée entre juin 2013 et février 2014 auprès d'un échantillon de médecins généralistes Lensois dont au moins un patient avait participé à un proqramme d'ETP du réseau de santé PREVALENS. Ces entretiens ont été enregistrés, puis le Verbatim a été analysé grâce au logiciel d'encodage N'vivo L'encodage a été réalisé selon la méthode de la théorisation ancrée. Résultats : . 14 médecins généralistes ont participé à l'étude. Ils admettent que l'ETP a un rôle à jouer dans la prise en charge des maladies chroniques mais il existe encore trop d'obstacles pour l'intégrer à leurs pratiques. Le manque de temps, la surcharge de travail administratif, le manque de rémunération et le manque de formation sont des freins au développement de l'ETP en médecine de ville. Par ailleurs, l'éducation thérapeutique était souvent assimilée à du simple conseil ou à des actions de prévention Conclusion : Le manque de temps représente un frein majeur à la mise en oeuvre de l'ETP mais des solutions semblent exister. Un allégement des charges administratives, une rémunération adaptée, une formation accessible et une aide au développement d'outils pédagogiques pourraient favoriser le développement de l'ETP de premier recours dans la région Nord Pas de Calais. On peut se demander si l'intégration de l'ETP à la formation initiale des médecins généralistes ne permettrait pas de les sensibiliser à ces pratiques et développer ainsi sa pratique sur le territoire.
