Book excerpt: Human trafficking has garnered a significant and growing amount of attention from the U.S. government since the 1990s, culminating in the passage of the federal Trafficking Victims Protection Act in 2000. There is also a growing body of research on human trafficking, but most of it has focused on trying to show that human trafficking is a problem. Wilson and Dalton explore the extent and characteristics of concrete cases of human trafficking in Columbus and Toledo, Ohio, as well as the awareness of and response to the problem by the justice systems and social service provider communities in the two cities. The authors summarize their content analysis of newspaper accounts as well as key respondent interviews that they conducted with criminal justice officials and social service providers in each site. These identified several cases of juvenile sex trafficking and forced prostitution in Toledo, as well as a smaller trafficking market centered on the forced labor of noncitizens in Columbus. Wilson and Dalton compare the two cities' considerably different responses to human trafficking, and conclude with suggestions on how to raise awareness about human trafficking and improve the responses of the criminal justice system, the juvenile justice system, and social services to the problem.

These are not only illegal activities, but also forms of violence and abuse that result in immediate and long-term physical, mental, and emotional harm to victims and survivors. In 2013, the Institute of Medicine/National Research Council released the report Confronting Commercial Sexual Exploitation and Sex Trafficking of Minors in the United States. The report found that the United States is in the very early stages of recognizing, understanding, and developing solutions for these crimes. Health care professionals need to be able to recognize past, ongoing, or potential victimization by commercial sexual exploitation and sex trafficking among the youth in their care. Failure to do so increases the possibility that those at risk may become victims, and victims may miss opportunities for assistance and remain vulnerable to further exploitation and abuse. This Guide for the Health Care Sector provides a summary of information from the original report that is most relevant to individuals who and settings that see children and adolescents for prevention and treatment of injury, illness, and disease. This includes physicians, nurses, advanced practice nurses, physician assistants, mental health professionals, and dentists who practice in settings such as emergency departments, urgent care, primary care clinics, adolescent medicine clinics, school clinics, shelters, community health centers, and dental clinics among others. This guide includes definitions of key terms and an overview of risk factors and consequences; barriers to identifying victims and survivors as well as opportunities for overcoming these barriers; examples of current practices in the health care sector; and recommendations aimed at identifying, preventing, and responding to these crimes.

Book excerpt: Research to evaluate the knowledge gaps and training needs of medical providers to meet sex trafficked victim’s specific needs is at the forefront of the human trafficking pandemic. Also, there are barriers to identifying and responding to victims of sex trafficking. Over the last ten years, there has been an increase in training of health care professionals on sex trafficking and its health implications settings (Bersamin, et al., 2014). However, methods and content of education on trafficking vary widely, and there is little evaluation of the impact of the training. Recent studies have demonstrated that 28 percent to 50 percent of human trafficked victims in the United States encountered health care professionals while in captivity but not identified or recognized (Bersamin, et al., 2014). The objective of this paper is to evaluate the knowledge gaps and training needs of medical providers and to demonstrate the importance of providing training to meet sex trafficked victim’s specific needs. Also, address the barriers to identify and respond to victims of sex trafficking. The process of researching healthcare provider training awareness in identifying sex trafficked victims may provide valuable insight for policy making to prevent, protect, and prosecute human traffickers and eradicate this crime.

According to this report, efforts to prevent, identify, and respond to these crimes require better collaborative approaches that build upon the capabilities of people and entities from a range of sectors. In addition, such efforts need to confront demand and the individuals who commit and benefit from these crimes. The report recommends increased awareness and understanding, strengthening of the law's response, strengthening of research to advance understanding and to support the development of prevention and intervention strategies, support for multi-sector and interagency collaboration, and creation of a digital information-sharing platform. A nation that is unaware of these problems or disengaged from solutions unwittingly contributes to the ongoing abuse of minors. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.
