Broadly speaking, three levels of intervention can be identified: - national campaigns directed to the general population; most of them are multi phase campaigns aimed at providing information about HIV transmission and protective behaviour; they use the various mass media channels and are mainly directed to sexual behaviour modifi cation; - community-based interventions, addressed to specific target popula tions; these populations have been typically selected according to both the high risk of infection (gay men and prostitutes) and the difficulty to reach the members of these communities (intravenous drug users); - individual testing and counselling, often supported by public funds or large non-governmental organizations. Major efforts have been devoted to the development and the implemen tation of these preventive programmes, both in terms of human re sources and financial support. On the other hand, in most countries, far less energy has been put into the evaluation of these campaigns. This gap is not explained by the fact that evaluation of AIDS/HIV cam paigns is a totally new challenge in terms of methodology: there are classical methods, developed over twenty years and used in other fields of prevention.

Book excerpt: The Ryan White Comprehensive AIDS Resources Emergency (CARE) Act gives funding to cities, states, and other public and private entities to provide care and support services to individuals with HIV and AIDS who have low-incomes and little or no insurance. The CARE Act is a discretionary program that relies on annual appropriations from Congress to provide care for low-income, uninsured, or underinsured individuals who have no other resources to pay for care. Despite its successes, funding has been insufficient to address all of the inequalities and gaps in coverage for people with HIV. In response to a congressional mandate, an Institute of Medicine committee was formed to reevaluate whether CARE allocation strategies are an equitable and efficient way of distributing resources to jurisdictions with the greatest needs and to assess whether quality of care can be refined and expanded. Measuring What Matters: Allocation, Planning, and Quality Assessment for the Ryan White CARE Act proposes several types of analyses that could be used to guide the evaluation and improvement of allocation formulas, as well as a framework for assessing quality of care provided to HIV-infected persons.

Book excerpt: If resources for HIV prevention efforts were truly unlimited, then this book would be en tirely unnecessary. In a world with limitless support for HIV prevention activities, one would simply implement all effective (or potentially effective) programs without regard to expense. We would do everything useful to prevent the further spread of the virus that has already claimed hundreds of thousands of lives in the United States and millions of lives worldwide. Unfortunately, funding for HIV prevention programs is limited. Even though the amount of available funding may seem quite large (especially in the United States), it is still fixed and not sufficient to meet all needs for such programs. This was very well illustrated in the summer of 1997 when over 500 community-based organizations applied for a combined total of $18 million of HIV prevention funding from the U.S. Centers for Disease Control and Prevention (CDC). Less than one-fifth ofthese organizations received support via this funding mechanism. Hence, although $18 million may seem like a large amount of money at first blush, it is not enough to meet all of the prevention needs that could be addressed by these community-based organizations.
