Children with autism spectrum disorder (ASD) have difficulty making friends, even in elementary and middle school. However, very little is known about earlier friendships in children with autism such as preschool friendships. The current study examined friendships in preschool children with autism and explored how joint attention contributes to these friendships in mainstreamed settings. The participants were 31 mainstreamed preschool children (ages 2-5) with ASD. Two 15-minute school observations were conducted for each participant to capture participants' interactions with peers and adults during free play. Friendship was defined by three criteria: the children must have 1) at least 50% of their social initiation attempts responded to, 2) at least one interval of joint engagement, and 3) at least one positive affective exchange (adapted from Howes, 1983). Using this definition for friendship, the results indicated that 20% of the sample had friendships at school. Children with friends were more likely than children without friends to be jointly engaged with their peers during free play, and they used higher joint attention skills. Additionally, the study also found that adults were using far more behavioral management skills than any other strategies within the classrooms. Future studies may want to examine the effects of early interventions and/or teacher training on the development of friendships in preschool children with ASD within the school setting.

Fewer studies, however, have investigated the relation between parent and child interactions and peer relations in children with autism. The current study aimed to investigate the relation between parent-child joint attention skills, social competence and friendship quality in children with autism and in typically developing children. A matched sample of 20 preschool-aged children with autism and 20 preschool-aged typically developing children were observed interacting with their parents in a laboratory setting. Approximately one year later, parents filled out a questionnaire assessing their child's social competency and quality of friendships with peers. Results indicated significant group differences between children with autism and typically developing children in all study variables, with children with autism displaying less initiation of joint attention, lower social competence and low quality friendships. Additionally, child initiated joint attention was positively related to social competence for both groups; effects were not moderated by diagnosis status. It is concluded that parent and child interactions during the preschool years are important to the development of social competence with peers. Intervention and policy implications are discussed.

Two social skills, joint attention and imitation and their relationships with simple play and pretend play skills, were investigated. A longitudinal study with three phases of time was conducted to explore the sequence of the development of joint attention and imitation and play skills. Self-developed scales were used to measure the variables of interests and the reliabilities of the scales were established. Multiple regressions showed that imitation at Time 2 mediated the relation between joint attention at Time 1 and play skills at Time 3. The implication of the results for understanding the nature of play development and for design of early interventions for preschool children with ASD are also discussed. As an additional feature, the children and adults in this sample received Musical Intervention Therapy (MIT) in North Wales in the United Kingdom (UK). This allows comparison of interaction with music (hereafter referred to as 'Music') and without music ('Interaction') within an MIT setting. Timing plays a vital role in communication. It allows perception and coordination of communication tools such as speech, facial expressions, and body language. For example, neurotypical individuals naturally sense when it is their turn to speak, and use pauses to emphasise or alter the meaning of what they are trying to convey. When timing is optimal and communication flows evenly, people remain largely unaware of its importance. On the contrary, erratic timing is noticed immediately. For example, when the audio or video transmission during a Skype-conversation lags behind, one can still carry on with his or her conversation but it becomes harder to read social cues and communication becomes tedious. Scholars refer to this type of timing as 'social timing'. Its role is to structure and organise the timing of verbal and non-verbal communication tools. Social timing is further split into the temporal organisation of our own body language and speech ('intrapersonal') and that between self and other ('interpersonal'). Social timing in parent-infant interaction plays a key role for long-term development of communication and social skills. The pioneers of developmental social timing studies focused on describing rhythmicity of pre-verbal communication in the 1970s. A decade later, advances in statistical methods allowed examination of intra- and interpersonal timing, which can be cyclical and synchronous respectively.

Parents have also inquired about a handbook that they can share with family members and their children's teachers. So here it is, a reader friendly guidebook to The Friends Program. The program strives to replicate a preschool and elementary school setting to capture the challenges that these children experience at school, across social settings such as their home, and in after school activities. The program is unique in that the parents, experienced interpreters of their children's social world, attend every session. We learned early on that their observations and comments made during the parent's group provided the clinicians with insight that might otherwise take months to acquire. A parent immediately knew which movie or video their child was referring to during the table discussion. They understood why a child may look distressed at the start of a session or what they were eager to share in the group. The program runs on the academic calendar since we have learned that different issues arise at different times such as the opening of the school year, holidays, and tests. Our curriculum is not standardized but molds to fit the issues the children are currently raising, i.e., making new friends, keeping your cool, bullying, and collaboration. The goal of this program is to enhance a child's Social Communication, capacity for shared symbolic Play, understanding of Emotions and Emotional Regulation, verbal Communication and Sensory Regulation (SPECS). The book defines each area and elaborates on how these goals are addressed in different activity themes. Each activity has a theme, goal, and strategies that address the SPECS. The goal for the parents is to increase their understanding of their child, and provide a safe and supportive setting where they can openly share with the clinical facilitator and fellow parents the stages they travel as parents of children on the Autism Spectrum/ Asperger's Disorder. The journey involves stages of denial, anger and frustration, bargaining, sadness, advocacy, and acceptance. This journey does not travel a linear path but moves up and down in relationship to how their child is doing. The stages come to life with direct quotes from the parents along with a story written by a parent who was not aware of how we were going to present this process in our first book, Asperger's Disorder in Young Children (2004). Over time an increasing number of fathers are attending each session and that has provided a rich insight into the paths that couples travel. The group parents provide each other with ongoing support years after they leave the group, often sending us updates on the progress their child has made or their recent achievements as (for example) an Eagle Scout, in the martial arts, in the arts, or in their transition to college. The book contains the charts that we use in the group, a guide to children's books we use in our program, and a list of books for professionals and parents. Lastly we provide a review of our pilot outcome study, preliminary data, and a set of outcome measures. Nothing is more satisfying than watching a child who asked their parents and the staff at the start of the program, "who needs friends?", leave the program with a desire to maintain the friendships they made in the group and gradually expand their social world with a little nudge and lots of love from their parents.

Book excerpt: Early intervention is a critical component of efforts to optimize outcomes for children with autism spectrum disorders (ASD) and their families. One promising target for early intervention is joint attention, an early developing social-cognitive competency that is foundational to many other social, communicative, and cognitive skills; and a core deficit in children with ASD. While joint attention interventions are gaining interest among researchers, many are limited by their failure to situate joint attention development within the caregiver-child relationship and to adequately examine child and caregiver outcomes and transactional processes. The purpose of this study was to examine changes in child and caregiver joint attention actions and transactions across the course of a parent-mediated joint attention intervention. The Child-Caregiver Joint Attention coding system was developed and applied to videotaped caregiver-child interaction sessions from all phases of the Joint Attention Mediated Learning intervention. Participants included five mothers and their toddler aged sons. Joint attention actions examined included gaze alternations, pointing, showing, joint attention responding, and joint attention initiating for both children and caregivers. Four of five children demonstrated increases in gaze alternations, joint attention responding, and joint attention initiating by the end of the intervention. Three caregivers demonstrated increases in gaze alternations and joint attention responding, and four displayed increases in joint attention initiating. There was no clear pattern of change across children or caregivers in pointing or showing. All participants, with the exception of one caregiver, responded to a higher percentage of opportunities for joint attention in the final intervention phase than in Baseline, suggesting that most participants became more responsive to their social partners by the end of the intervention. The findings of this study suggest that parent-mediated joint attention interventions have the potential to promote changes in both child and caregiver joint attention actions and transactional relationships. Future research should continue to examine outcomes for both children and primary caregivers and changes in child-caregiver transactions over the course of different types of joint attention interventions in order to inform intervention development and selection, and explore mechanisms for change.
