Research on glycoside kinetics has progressed at a rapid pace, requiring continuing reevaluation of the state of our understanding of this problem. The present article focuses on the effect of disease states (renal, gastrointestinal, thyroid, and cardiac) on the absorption, distribution, and clearance of a number of digitalis glycosides. Evidence is critically reviewed, and interpreted with respect to possible clinical implications. A. Renal Insufficiency I. Strophanthin Strophanthin disposition in renal failure has been evaluated in only two studies. KRAMER et ai. (1970) determined an elimination half-life of 14 h in normals as com pared to 60 h in anuric patients. Similar results were reported by BRASS and Pm LIPPS (1970) using tritiated strophanthin. They found a half-life value of 18 h in healthy individuals as compared to 68 h in anuric patients. The findings clearly in dicate that the elimination half-life of strophanthin is prolonged in renal failure.

The goal of this book is to update clinicians on recent data on the medical management of patients with CSA. Ischemic heart disease remains a major public health problem. Chronic stable angina is the initial manifestation of ischemic heart disease in approximately one half of patients. Stable coronary artery disease is generally characterized by episodes of reversible myocardial demand/supply mismatch, related to ischaemia or hypoxia, which are usually inducible by exercise, emotion or other stress and reproducible—but, which may also be occurring spontaneously. Such episodes of ischaemia/hypoxia are commonly associated with transient chest discomfort (angina pectoris). The aim of the management of CSA is to reduce symptoms and improve prognosis. The management of these patients encompasses lifestyle modification, control of coronary artery disease risk factors, evidence-based pharmacological therapy and patient education. All patients with stable angina should be offered optimal medical treatment, defined as one or two anti-anginal drugs as necessary, plus drugs for secondary prevention of cardiovascular disease. Regarding the role of revascularization, randomised trials provide compelling evidence that myocardial revascularisation by coronary artery bypass grafting or by percutaneous coronary intervention improves symptoms of angina relative to continued medical treatment.

SWEGs are established for exposures of 1, 10, 100, and 1,000 days. This report is the second volume in the series, Spacecraft Water Exposure Guidelines for Selected Chemicals. SWEG reports for acetone, alkylamines, ammonia, barium, cadmium, caprolactam, formate, formaldehyde, manganese, total organic carbon, and zinc are included in this report. The committee concludes that the SWEGs developed for these chemicals are scientifically valid based on the data reviewed by NASA and are consistent with the NRC (2000) report, Methods for Developing Spacecraft Water Exposure Guidelines. SWEG reports for additional chemicals will be presented in a subsequent volume.

Book excerpt: A major problem in the treatment of opioid dependence has been the persistence of relapse despite detoxification and enforced prolonged abstention from drug use, with or without conventional psychotherapy and other efforts at rehabilitation. Both initial addiction and subsequent relapses are usually ascribed to the quest for opioid-produced euphoria in persons with character disorders. This formulation is in accord with one-half of the common sense "pleasure-pain" principle, but it ignores the other half, namely, the long-lasting dysphoric consequences of re peated opioid use (distressing abstinence phenomena, sexual distur bances, disruption of marital status, unemployment, enmeshment in criminal activities, arrests, and imprisonment). In any case, the pleasure-pain principle is an empty tautology since it is incapable of refutation by any conceivable objective data that might seem contradic tory, inasmuch as it can be "saved" by invocation of untestable uncon scious intervening variables. Less tied to the pleasure-pain principle is the view that relapse is due to long-lasting sequelae of previous opioid addiction, resulting from complex conditioning processes, both operant and classical, involving pharmacological, environmental, social and personal variables. In this view, relapse is not simply a re-enactment of initial opioid use, but is a "disease, sui generis" a disease of its own kind. The factors contributing to this disease, sui generis are reviewed in this book.
