Book excerpt: Implementation of Total Quality Management (TQM) is a major challenge facing health care organizations in the United States today. Understanding the organization's culture or climate is an essential element in initiating any successful quality process. Cultural audits evaluate the current state of an organization, assessing a multitude of factors and can lead to fact-based, positive recommendations for improvement. The purpose of this research project was two-fold: identify the current cultural profile of Tripler Army Medical Center staff members to establish a baseline on the existing culture, and determine if any cultural differentiation exists among occupational categories. A quantitative and analytical methodology was used in evaluating the corporate culture. The data collecting instrument was a modified Litwin and Stringer Organization Climate Questionnaire which was distributed to a stratified random sample. Statistical analysis included both description and inferential methodologies. Tripler had a relatively positive cultural profile on seven of the ten construct variables. There was tremendous consensus between occupational categories on all variables. The lack of variation between groups indicates the climate described is strongly representative of the entire population. Benchmarking the current cultural blueprint against the desired cultural qualities is the basis for improvement programs.

Book excerpt: Total Quality Management (TQM) challenges health care organizations to provide high-quality clinical care that also meets or exceeds customer expectations. This requires the development of a reliable method for measuring the perceived quality of care and satisfaction from a patient's point of view. The management problem was that a comprehensive analysis of patient satisfaction had not been conducted at Tripler Army Medical Center (TAMC) to measure quality from the patient's perspective. The purpose of this study was to determine the current level of patient satisfaction and identify the various aspects of the health care delivery system at TAMC which contribute to patient satisfaction. A quantitative research approach was used to collect and analyze data using a patient satisfaction survey. The results indicated that the majority of patients surveyed were very satisfied and rated TAMC as above average in their overall evaluation. The direct care aspects of the health care system had the greatest influence on patient satisfaction, and accounted for 56.8% of the variance in the patient's overall evaluation and 55.4% of the variance in overall satisfaction. This study was also successful in identifying areas to be used in the TQM process to improve the quality of care at TAMC. Patient Satisfaction Survey, Total Quality Management.

Belvoir, Virginia is now one year into its quality transformation. Over the course of the past year, the hospital has invested heavily in adopting the TQM philosophy, spending over $100,000 to formally train 350 of 863 staff members in TQM. The DACH leadership desires to develop a cultural climate that reflects the TQM philosophy, i.e. a climate that fosters team work, innovation, risk taking, involvement, and concern for quality. The purpose of this project was to assess the organizational culture of DACH as it relates to TQM. Supporting objectives were to (1) validate or refute the DACH leadership's intuitive assessment of the current culture, (2) identify areas of the culture that need additional focused attention, and (3) serve as a benchmark against which to judge future assessments in the future. The results of the study suggest that many of the practices indicative of a TQM culture are present at DACH. The staff of DACH seem to have high morale and a good awareness of their mission. They look for an discuss ways to improve their work and are concerned about their customers.

At the direction of the medical center's chief of staff, a Business Process Reengineering Process Action Team (BPR-PAT) was established to evaluate Tripler's Utilization Management, Resource Management, Managed Care, Patient Administration, Information Management, and Clinical Support divisions to maximize billing, eliminate duplicative functions, simplify processes, improve data quality, and reduce costs. The purpose of this GMP is to provide a descriptive case study of the BPR-PAT's efforts and proposal to realign TAMC's organizational structure to reflect "best business practice" that will facilitate coordinated care and maximize the use of resources. The BPR-PAT proposal directs that TAMC's Managed Care Division be reorganized into Data Quality Management, Decision Support, and Customer Services Branches, and that the Managed Care Division be realigned to report to the Deputy Commander for Administration (DCA) instead of to the Deputy Commander for Clinical Services (DCCS). A fourth branch, Health Care Utilization, will be reorganized within TAMC's Quality Services Division.
