Currently, there is no agreed solution among the stake-holder agencies regarding strategic health policy in support of this objective. Health care is a component of basic human needs and should be accessible, affordable, and effective. Following combat operations and phasing into stabilization operations, basic health care infrastructure and systems have often been either disrupted or degraded altogether. To address this situation, the U.S. Government requires a coordinated interagency approach to formulate a strategic health care plan. Incorporating all relevant players into this endeavor will promote sound organizational design, unity of effort, and a culture favorable to synchronization. This paper contains specific recommendations and advocates a renewed effort toward addressing them. The primary constructs under review are U.S. Government organization, leadership, and culture as they relate to a strategic healthcare policy. This approach will reduce redundant efforts, conserve resources and augment the legitimacy of the new Government of Iraq while supporting U.S. national strategic aims.

Despite the vast resources and attention expended since 9/11, terrorism has increased in many societies that have been caught up in the war on terror. Why have U.S. policies been unable to stem the tide of violence? Harrison Akins reveals how the war on terror has had the unintended consequence of increasing domestic terrorism in U.S. partner states. He examines the results of U.S.-backed counterterrorism operations that targeted al-Qaeda in peripheral regions of partner states, over which their central governments held little control. These operations often provoked a violent backlash from local terrorist groups, leading to a spike in retaliatory attacks against partner states. Senior U.S. officials frequently failed to grasp the implications of the historical conflict between central governments and the targeted peripheries. Instead, they exerted greater pressure on partner states to expand their counterterrorism efforts. This exacerbated the underlying conditions that drove the escalating attacks, trapping these governments in a deadly cycle of tit-for-tat violence with local terrorist groups. We are simultaneously engaged in a huge effort to learn how to conduct those missions for which we do not consistently prepare. Mr. Robert Ramsey's historical study examines three cases where the US Army has performed this same mission in the last half of the 20th century. In Korea during the 1950s, in Vietnam in the 1960s and 1970s, and in El Salvador in the 1980s the Army was tasked to build and advise host nation armies during a time of war. The author makes several key arguments about the lessons the Army thought it learned at the time.Among the key points Mr. Ramsey makes are the need for US advisors to have extensive language and cultural training, the lesser importance for them of technical and tactical skills training, and the need to adapt US organizational concepts, training techniques, and tactics to local conditions. Accordingly, he also notes the great importance of the host nation's leadership buying into and actively supporting the development of a performance-based selection, training, and promotion system. To its credit, the institutional Army learned these hard lessons, from successes and failures, during and after each of the cases examined in this study. However, they were often forgotten as the Army prepared for the next major conventional conflict.
