In these cases, the quality of the dental cast relies on the digitizing and machining processes provided by CAD-CAM systems. As geometric errors may be introduced during the step of scanning and manufacturing of casts, their accuracy is still questionable. The purpose of this study was to compare the accuracy of casts obtained using digital and conventional implant impressions. A maxillary model containing dentalimplants on the region of canine and pre-molar was used as master model. Dental casts (n=10) were fabricated through both conventional and digital implant impressions. The conventional casts (CC), stereolithographic casts (SC) and the master model (MM) were digitized and the accuracy was determined through a deviation analysis and linear measurements. Data were analyzed using Student t-test and ANOVA with p=0.05. SC showed higher deviation at vestibular (CC: 41 u00b1 28; SC: 117 u00b1 37 u03bcm) and lingual cusps (CC: 40 u00b1 19; SC: 80 u00b1 42 u03bcm) in comparison to CC. No statistically significant difference was found for linear measurements of conventional and digital casts. The entire-arch accuracy was comparable between casts. However, in located points, stereolithographic casts were less accurate in comparison with conventional casts.

With the ever-increasing number of dentists involved in implant dentistry, it is essential to ensure that their treatment methods follow the highest standard. The ITI Treatment Guide series, a compendium of evidence-based implant-therapy techniques in daily practice, is written by renowned clinicians and provides a comprehensive overview of various therapeutic options. Using an illustrated step-by-step approach, the ITI Treatment Guide shows practitioners how to manage different clinical situations, with the emphasis on sound diagnostics, evidence-based treatment concepts, and predictable treatment outcomes. The second volume of the ITI Treatment Guide is devoted to the restoration of partially dentate patients. Central to this volume of the ITI Treatment Guide are loading protocols available to the clinician and the patient and how they relate to various treatment indications, including both single and multiple missing teeth in the posterior and anterior regions of the mouth. Among potential topics for upcoming volumes are implant placement in extraction sockets, loading protocols in edentulous patients, implant therapy in the esthetic zone in extended edentulous spaces, and many more.

Book excerpt: TitleDigital intraoral scanners vs traditional impressions: Fitting accuracy of screwed mesostructuresBackgroundPassive fit and marginal precision of prosthetic structures on osteointegrated implants are a requirement to minimize tensions and to avoid loss of marginal bone and finally lack of osteointegration. The impression process is a primary step to obtain sufficient accuracy. We use daily intraoral scanners for impressions to made implant restorations from several years and we appreciate the clinical outcomes for passive fitting (cemented sovrastructures) and precision of screwed ones.AimThe aim of this job is to investigate the marginal accuracy of custom CoCr milled mesostructures on implants acquiring impressions by intraoral digital scanners and compare them to ones obtained bytraditional impressions systems and to evaluate if the misfit is within the optimal measure of 25-50u00b5m.Material and methodsTwo pair of implants were rigidly fixed in maxilla resin models. The first two implant were internal hexagonal flat connection implants made by Winsix implant system ( Biosaf In srl). The second two were internal octagonal conical connection implants made by Geass (Geass srl). Three impressions were took of each model. The first one was a traditional impression made by polyether material with the single impression technique. The other two impression was acquired using CS3500 Carestream Intraoral Scanner and Trios3 intraoral scanner. The digital impressions were made following factory recommendations for implant cases workflow.Milled CoCr screwed mesostructures were commissioned on the three impressions of each model. The aim was to analyze the misfit of the three mesostructures obtained using Sheffield test. The misfit was evaluated by a radiograph taken perpendicularly to the long axis of the implants. ImageJu00ae software was used to digitally process the radiographic images and perform the measurements. ResultsWe eliminate, using an artificial model some intraoral variables as blood, saliva, patient movements but we simulate the natural architecture of mouth maintaining crestal anatomy, gum, implant distance. We investigate the misfit radiographically performing the Scheffield test: in this way we screwed firmly only one screw of the mesostructure and we observe the matching of the other implant-structure connection. The bigger misfit was appreciate on the two mesostructures obtained by traditional impressions, one of them has failed to comply with the established gap limit of 50 u00b5m.Very good fitting, within the limit of 50 u00b5m was found on three mesostructures: The one made by impression with Carestream intraoral scanner CS3500 on winsix implantsand the two performed by Trios3 intraoral scanner 3Shapedental on Winsix implants and Geass Way implantsThe fitting accuracy of the last mesostructure made by impression performed on Geass Way implants with CS3500 scanner looks a middle way.ConclusionsThe misfit of all the mesostructures obtained by digital intraoral scanners seems to be in the safety range, avoiding structural tensions involving implant after screwing. This study confirm the fitting accuracy of milled mesostructures obtainable by digital intraoral impressions. We expected a more homogeneous results in the group of mesostructures obtained by digital impressions, poor number of mesostructures investigated may be the reason of the discrepancy.More data are indispensable to complete the results obtained and to investigate the real accuracy obtainable .

Book excerpt: Introduction: Studies demonstrated that the accuracy of intra-oral scanners can be compared with conventional impressions for most indications. However, little is known about their applicability to take impressions of multiple implants. Objective: compare the accuracy between digital impressions and different conventional printing techniques in implants in order to observe differences of the novel digital impression with the conventional techniques.Material and Methods: A stone cast of an edentulous mandible with 4 implants was fabricated to serve as master cast (control), two master models were made: one with parallel implants (Model 1)and another with uneven implants(Model 2). Model 1: 4 types of impressions were taken(Group A: tray impressions with reposition abutments; Group B: Open tray impression groups for dragging copings, without splinting; Group C: Open tray impressions for ferrules; Group D: Using 3M u2122 True Definition Scanner system).Model 2: 3 types of impressions were taken(Group E: tray impressions with reposition abutments; Group F: Open tray impression groups for dragging copings, without splinting; Group G: Using 3M u2122 True Definition Scanner system).Master casts and conventional impression casts were digitized with na extraoral high-resolution scanner. Standard tessellation language (STL)data sets from the seven groups of digital and conventional impressions were superimposed with the STL data set from the master cast to assess the 3D (global)deviations. To compare the master cast with digital and conventional impressions. Results: Statistical analysis was performed using the IBMu00ae SPSSu00ae Statistics Vs. 24, NY, USA.The normality of the distributions was analysed with an Kolmogorov-Smirnov test. The median comparison was performed using The differences between the median were analyzed using non-parametric Krustal-Wallis and Mann-Whitney tests with a significance level of p

Book excerpt: Aim: To compare early implant failure, template-related complications, and virtual planning accuracy of computer-assisted template-based implant placement using conventional impression and scan model or digital impression using conventional impression and scan model or digital impression to rehabilitate partial edentulous patients using flapless or mini-flap procedures and immediate loading.u2028Materials and methods: Any partially edentulous patients requiring at least 1 implant to be plan on three-dimensional (3D) cone beam computed tomography (CBCT) scans using a dedicated software were enrolled in the trial. Patients were randomised according to a parallel group design into two arms: intraoral digital impression (fully digital group) versus conventional impression and scan model (control group). Implants were to be placed flapless and loaded immediately, if inserted with a torque over 35 Ncm, with reinforced provisional prostheses. Three deviation parameters (horizontal, vertical, and angular) were defined and calculated between the planned and placed implant positions and analyzed statistically. Results were compared using a mixed-model repeated-measures analysis of variance (u03b1=0.05).Results: Twelve patients were randomised to the fully digital group (6 patients with 17 implants) and control group (6 patients with 20 implants). The mean error in angle was 2.56u00b11.52u00b0 (range 0.3u20135.0u00b0) in the fully digital group and 2.18u00b11.41u00b0 (range 0.3u20135.8u00b0) in the control group (P=0.519); in the horizontal plan (mesio-distal), the mean error was 0.57u00b10.32 mm (range 0.1u20131.1 mm) in the fully digital group and 0.43u00b10.26 mm (range 0.1u20130.9 mm) in the control group (P=0.249); in the vertical plan (apico-coronal), the mean error was 0.67u00b10.51 mm (range 0.0u20131.6 mm) in the fully digital group and 0.43u00b10.32 mm (range 0.0u20131.2 mm) in the control group (P=0.180).Conclusions: With the limitation of the present randomized controlled trial, intraoral digital impression may be a viable option to conventional impression and scan model for the rehabilitation of partial edentulous patients by using computer-guided template-assisted implant placement.
