Book excerpt: L'accident vasculaire cérébral représente un bouleversement dans la vie des personnes atteintes et leur accompagnement en ambulatoire peut s'avérer complexe. Comment les patients sont-ils accompagnés et quelle place accordent-t-ils à leur médecin traitant dans ce suivi ? Il s'agit d'une étude qualitative par entretiens semi-dirigés réalisée auprès de dix-sept patients vivant au domicile dans la région Rhône-Alpes. Un réel défaut d'information est noté par la plupart des patients ainsi qu'une difficulté de compréhension de ces informations. Ils continuent à se poser de nombreuses questions même à distance de l'AVC. L'accompagnement n'est pas toujours optimal, concernant aussi bien les aides humaines, que le soutien psychologique ou encore le soutien de l'aidant. Les patients ne savent pas toujours vers qui se tourner pour chercher de l'aide. Le médecin traitant, par son approche centrée patient, joue un rôle important dans cet accompagnement. Il permet la pérennisation des connaissances autour de l'AVC, coordonne les soins nécessaires et guide les patients en fonction de leurs besoins. Malheureusement cette coordination n'est pas toujours parfaite et les patients peuvent se sentir démunis. Un annuaire informatif destiné aux patients et distribué par le médecin traitant serait un moyen d'améliorer l'accompagnement. En s'appuyant sur cet annuaire, le médecin traitant pourrait orienter ses patients vers les professionnels de proximité. Cette délivrance pourrait se faire lors d'une consultation dédiée au retour de l'hospitalisation.
