Book excerpt: This Code of Practice, which has also been endorsed by WHO, PAHO and ESTRO, fulfils the need for a systematic and internationally unified approach to the calibration of ionization chambers in terms of absorbed dose to water and to the use of these detectors in determining the absorbed dose to water for the radiation beams used in radiotherapy. It provides a methodology for the determination of absorbed dose to water in the low, medium and high energy photon beams, electron beams, proton beams and heavy ion beams used for external radiation therapy.

Book excerpt: Treatment Planning and Dose Calculation in Radiation Oncology, Third Edition describes the treatment methods and technical guides as models of contemporary radiation therapy. These models should be modified for each individual patient to yield a best fit to the disease being treated and the radiation sources employed. This book is composed of seven chapters, and begins with an overview of the elements of clinical radiation oncology. The subsequent chapter deals with the production, interaction, and measurement of radiation. These topics are followed by intensive discussions of dose calculation for external beams and pretreatment procedures of radiation therapy. A chapter looks into the principles, apparatus, and dose calculation in brachytherapy. The final chapters describe the principles and practical applications of treatment planning. This book will be of value to radiation oncologists.

Book excerpt: Absorbed-dose-to-water calibrations are important to the medical community to facilitate the accurate determination of doses delivered to tumors during external-beam cancer therapy. The first version of this document offered an absorbed-dose-to-water calibration service based on a graphite calorimeter as the primary standard. However, the use of this calorimeter necessitated calculations to convert the measurement from graphite to water. In 1989, a water calorimeter was introduced at the National Institute of Standards and Technology (NIST), which was to replace the graphite calorimeter as the primary standard. Though the calculations necessary for conversion factors were eliminated with this new technology, a calibration service based on the water calorimeter was not developed at this time. Despite the fact that the service was available, the medical physics community did not take advantage of it and used chambers calibrated in terms of exposure (in units of roentgen) to calibrate their radiotherapy 60Co and high-energy electron accelerator x-ray. A protocol, commonly known as TG21, developed by the American Association of Physicists in Medicine (AAPM), involves many calculations to arrive at the quantity desired by the medical physicist in the practicing clinic, cGy/MU (centiGray/monitor unit). The AAPM has initiated a new protocol through Task Group 51 which involves absorbed-dose-to-water calibrations of ion chambers commonly used in the calibration of the clinical radiotherapy photon and electron beams.NIST has developed and is now prepared to offer the absorbed-dose-towater calibration service for ionization chambers based on a water calorimeter standard developed by Steve Domen at NIST. This document outlines the steps that have been taken to develop this service including a brief description of the Domen water calorimeter. The procedures that are involved in the calibration of an ionization chamber for this quantity are presented along with results from recent comparisons of the NIST with the Bureau International des Poids et Mesures (BIPM) in France and the National Radiation Council Canada (NRCC).

Book excerpt: This is the first international code of practice dedicated to the dosimetry of small static fields used in radiotherapy. It provides consistent reference dosimetry, traceable to metrological primary standards, and enables common procedures within a country to be followed for small field dosimetry. The publication presents an overview of the physics, followed by a general formalism for reference dosimetry in small fields. Guidelines for its practical implementation using suitable detectors and methods for the determination of field output factors are given for specific clinical machines that use small static fields. The development of this code of practice has been done through an international working group, established jointly with the American Association of Physicists in Medicine. Internationally harmonized guidelines in this field will ensure worldwide consistency in dose delivery to radiotherapy patients and will contribute to dose standardization in international clinical trial studies, comparing outcomes of various radiotherapy treatment modalities using small fields.

Book excerpt: Passenger screening at commercial airports in the United States has gone through significant changes since the events of September 11, 2001. In response to increased concern over terrorist attacks on aircrafts, the Transportation Security Administration (TSA) has deployed security systems of advanced imaging technology (AIT) to screen passengers at airports. To date (December 2014), TSA has deployed AITs in U.S. airports of two different technologies that use different types of radiation to detect threats: millimeter wave and X-ray backscatter AIT systems. X-ray backscatter AITs were deployed in U.S. airports in 2008 and subsequently removed from all airports by June 2013 due to privacy concerns. TSA is looking to deploy a second-generation X-ray backscatter AIT equipped with privacy software to eliminate production of an image of the person being screened in order to alleviate these concerns. This report reviews previous studies as well as current processes used by the Department of Homeland Security and equipment manufacturers to estimate radiation exposures resulting from backscatter X-ray advanced imaging technology system use in screening air travelers. Airport Passenger Screening Using Backscatter X-Ray Machines examines whether exposures comply with applicable health and safety standards for public and occupational exposures to ionizing radiation and whether system design, operating procedures, and maintenance procedures are appropriate to prevent over exposures of travelers and operators to ionizing radiation. This study aims to address concerns about exposure to radiation from X-ray backscatter AITs raised by Congress, individuals within the scientific community, and others.
