Part II: Price Regulation The second partofthis volume examines the role ofprice regulation in controlling health care costs. It contains three chapters. In chapter seven, I examine the alternatives for regulating pharmaceutical prices. In chapter eight, Jack Hadley examines the impactofvarious forms ofhospital price regulation; while in chapter nine,MarkPaulyexaminestheroleofpriceregulation incontrollingphysician fees. Chapter seven focuses on the issue of regulating pharmaceutical prices. There are two key issues examined in this paper. First, is there a clear need for price regulation, and second, can price regulation work in this industry? In response to the first question, I come to the conclusion that the proponents ofprice regulation have not really proven their case. Although the financial returns in the pharmaceu tical industry have been slightly higher than expected during the 1970s and 1980s, there is not overwhelming evidence of"price gouging" or excessive profits on the part of the industry. In response to the second question, the answer is clearly no. The traditional approaches to price regulation will not have the intended affect of eliminating excess profits from the industry while maintaining the incentives for research and development. First, rate-of-return regulation, the most natural approach, would result in many adverse incentives-includingexcessive investment in research and developmentinorderto inflatetheratebaseused tocalculatedtheallowablereturns.

Book excerpt: Social change has placed new demands on the practice of medicine, altering almost every aspect of patient care relationships. Just as medicine was encouraged to embrace the biological sciences some 100 years ago, recent directives indicate the importance of the social sciences in understanding biomedical practice. Humanistic challenges call for changes in curative and technological imperatives. In this book, social scientists contribute to such challenges by using social evidence to indicate appropriate new goals for health care in a changing environment. This book was designed to stimulate and challenge all those concerned with the human interactions that constitute medical practice. To encompass a wide range of topics, the authors include researchers; practicing physicians from the specialties of family, general, geriatric, pediatric, and oncological medicine; social and behavioral scientists; and public health representatives. Cutting across disciplinary boundaries, they explore the ethical, economic, and social aspects of patient care. These essays draw on past studies of the patient-doctor relationship and generate new and important questions. They address social behavior in patient care as a way to approach theoretical issues pertinent to the social and medical sciences. The authors also use social variables to study patient care and suggest new areas of sociomedical inquiry and new approaches to medical practice, education, and research. Its cross-disciplinary approach and jargon-free writing make this book an important and accessible tool for physician, scholar, and student.

Book excerpt: The Economics of Health and Health Care is the market-leading health economics textbook, providing comprehensive coverage of all the key topics, and balancing economic theory, empirical evidence, and public policy. The ninth edition offers updated material throughout, including two new chapters: Disparities in Health and Health Care (Chapter 7) examines issues of race, ethnicity, income, gender, and geography with respect to health care access, health inputs, and health outcomes; Pandemic Economics (Chapter 9) introduces a new and simplified economic treatment of epidemics and pandemics within the context of COVID-19. We also include applications from the growing literature on digital medicine. The book further highlights the impacts of the Affordable Care Act (ACA) and updates its path-breaking comparative analyses across countries to focus on the differences in access and costs. The book continues to provide a clear, step-by-step understanding of health economics, making economic principles accessible to students, supported by boxed examples, figures and tables. Each chapter contains concise summaries, discussion questions, and quantitative exercises to promote student learning. There is also a glossary of key terms and an extensive reference list. Instructors are supported by a range of digital supplements. It is the perfect textbook for students and practitioners taking undergraduate and postgraduate courses in health economics, health policy, and public health.
