Book excerpt: Research has consistently shown that therapy is effective and that clients experience positive outcomes from engaging in therapy (Orlinsky et al., 2004). However, past research has shown that many clients prematurely terminate therapy often with negative outcomes (Archer, Forbes, Metcalfe, & Winter, 2000; Carpenter, Morrow, del Gaudio, & Ritzler, 1981; Pekarick, 1985). The purpose of this study was to explore an intervention designed to reduce premature termination rates using a comprehensive, standardized role induction procedure and a more sensitive definition of premature termination. Further, the impact of the role induction as it relates to readiness for change was explored. This study also examined the impact the role induction would have on clients' commitment to therapy. A total of 106 adult clients seeking services at a community mental health center participated in the study. Participants were randomly assigned to either the experimental or control group. Prior to the intake session, each participant viewed a video. The experimental group viewed the comprehensive role induction video and the control group viewed a video explaining the services at the counseling center. Next, all participants completed the self-report measures of commitment to therapy (i.e. EAC-B; Tinsley et al., 1980) and readiness for change (i.e. SOC; McConnaughy, Prochaska, & Velicer, 1983). The present data failed to show that a role induction to counseling significantly impacts premature termination, commitment to counseling or readiness for change among clients seeking counseling at a rural community mental health center. Overall, the data failed to support previous research findings that seemed to indicate a role induction to counseling could reduce the occurrence of premature termination and increase a client's commitment to therapy (Reis & Brown, 2006; Walitzer, Dermen, & Connors, 1999).

Book excerpt: There may be several characteristics that impact client engagement in the counseling process. Studies have shown that, while characteristics exist that impact client follow through with attending scheduled counseling appointments, there is too much variability within these characteristics to grant any definitive findings. Most interesting to this study, research has shown that education level and experience level of the counselor does not impact client engagement (Nyman et al., 2010; Ogles, 2003). Also, one of the most reliable characteristics found to impact client engagement is congruence between client and counselor related to age, race, and sex (Bryan et al., 2004; Coleman, 2006; D'Andrea & Heckman, 2008; Hall et al., 2002; Malloy, 1981; Zhang & McCoy, 2009). Research finds that when clients and counselors have similar world views, the chances of creating a therapeutic relationship are heightened. This study attempted to determine if there are demographic or clinical characteristics specific to the client or counselor-in-training that impacts client engagement in the counseling process when services are delivered through a university- based community counseling clinic. This study also attempted to identify if congruence between client and counselor-in-training on characteristics of age, race, and sex impacted client engagement in the counseling process. Populations used within this study were clients who had received counseling services from a university-based community counseling clinic and counselors-in-training who had provided these services. Client characteristics were gathered using a comprehensive intake fotm and counselor-in-training characteristics were gathered from counselor education faculty who had worked with these students. Finding from this research suggest that client educational level, counselor-in-training age, and client and counselor-in-training age congruence were significant predictors of client engagement in the counseling process. This research found that those individuals with lower education levels were more likely to self-initiate termination in less than three sessions. Also, counselors-in-training between the ages of 30 to 39 were more successful in maintaining clients for more than nine sessions. Finally, when client and counselor-in-training were congruent on measures of age, client engagement was increased.

Book excerpt: With “groundbreaking research on the psychology of resilience” (Adam Grant), a top expert on human trauma argues that we vastly overestimate how common PTSD is in and fail to recognize how resilient people really are. After 9/11, mental health professionals flocked to New York to handle what everyone assumed would be a flood of trauma cases. Oddly, the flood never came. In The End of Trauma, pioneering psychologist George A. Bonanno argues that we failed to predict the psychological response to 9/11 because most of what we understand about trauma is wrong. For starters, it’s not nearly as common as we think. In fact, people are overwhelmingly resilient to adversity. What we often interpret as PTSD are signs of a natural process of learning how to deal with a specific situation. We can cope far more effectively if we understand how this process works. Drawing on four decades of research, Bonanno explains what makes us resilient, why we sometimes aren’t, and how we can better handle traumatic stress. Hopeful and humane, The End of Trauma overturns everything we thought we knew about how people respond to hardship.

Available in PDF, EPUB and Kindle. Book excerpt: Introduction to Counseling by Michael Scott Nystul provides an overview of counseling and the helping professions from the perspective of art and science—the science of counseling that generates a knowledge base proven to promote competency and efficacy in the practitioner, and the art of using this knowledge base to build skills that can be applied sensitively to clients in a multicultural society. The Fifth Edition has been organized into three sections: (1) an overview of counseling and the counseling process, (2) multicultural counseling and counseling theories, and (3) special approaches and settings. It continues to address key topics and issues, including gender, culture, and sexual orientation, and offers ways to integrate multiculturalism into all aspects of counseling, rather than view it as a separate entity. Highlighting emerging trends and changes in ethical codes, as well as reflecting the latest updates to the Diagnostic Statistical Manual (DSM-5), the book successfully illustrates the importance of art and science to modern-day counseling.
