Book excerpt: Prostate segmentation is a required step in determining the volume of a prostate, which is very important in the diagnosis and the treatment of prostate cancer. In the past, radiologists manually segment the two-dimensional cross-sectional ultrasound images. Typically, it is necessary for them to outline at least a hundred of cross-sectional images in order to get an accurate estimate of the prostate℗s volume. This approach is very time-consuming. To be more efficient in accomplishing this task, an automated procedure has to be developed. However, because of the quality of the ultrasound image, it is very difficult to develop a computerized method for defining boundary of an object in an ultrasound image. The goal of this thesis is to find an automated segmentation algorithm for detecting the boundary of the prostate in ultrasound images. As the first step in this endeavour, a semi-automatic segmentation method is designed. This method is only semi-automatic because it requires the user to enter four initialization points, which are the data required in defining the initial contour. The discrete dynamic contour (DDC) algorithm is then used to automatically update the contour. The DDC model is made up of a set of connected vertices. When provided with an energy field that describes the features of the ultrasound image, the model automatically adjusts the vertices of the contour to attain a maximum energy. In the proposed algorithm, Mallat℗s dyadic wavelet transform is used to determine the energy field. Using the dyadic wavelet transform, approximate coefficients and detailed coefficients at different scales can be generated. In particular, the two sets of detailed coefficients represent the gradient of the smoothed ultrasound image. Since the gradient modulus is high at the locations where edge features appear, it is assigned to be the energy field used to drive the DDC model. The ultimate goal of this work is to develop a fully-automatic segmentation algorithm. Since only the initialization stage requires human supervision in the proposed semi-automatic initialization algorithm, the task of developing a fully-automatic segmentation algorithm is reduced to designing a fully-automatic initialization process. Such a process is introduced in this thesis. In this work, the contours defined by the semi-automatic and the fully-automatic segmentation algorithm are compared with the boundary outlined by an expert observer. Tested using 8 sample images, the mean absolute difference between the semi-automatically defined and the manually outlined boundary is less than 2.5 pixels, and that between the fully-automatically defined and the manually outlined boundary is less than 4 pixels. Automated segmentation tools that achieve this level of accuracy would be very useful in assisting radiologists to accomplish the task of segmenting prostate boundary much more efficiently.

Book excerpt: Outlining of the prostate boundary in ultrasound images is a very useful procedure performed and subsequently used by clinicians. The contribution of the resulting segmentation is twofold. First of all, the segmentation of the prostate glands can be used to analyze the size, geometry, and volume of the gland. Such analysis is useful as it is known that the former quantities used in conjunction with a PSA blood test can be used as an indicator of malignancy in the gland itself. The second purpose of accurate segmentation is for treatment planning purposes. In brachetherapy, commonly used to treat localized prostate cancer, the accurate location of the prostate must be found so that the radioactive seeds can be placed precisely in the malignant regions. Unfortunately, the current method of segmentation of ultrasound images is performed manually by expert radiologists. Due to the abundance of ultrasound data, the process of manual segmentation can be extremely time consuming and inefficient. A much more desirable way to perform the segmentation process is through automatic procedures, which should be able to accurately and efficiently extract the boundary of the prostate gland with minimal user intervention. This is the ultimate goal of the proposed approach. The proposed segmentation algorithm uses a probability distribution tracking framework to accurately and efficiently perform the task at hand. The basis for this methodology is to extract image and shape features from available manually segmented ultrasound images for which the actual prostate region is known. Then, the segmentation algorithm seeks a region in new ultrasound images whose features closely mirror the learned features of known prostate regions. Promising results were achieved using this method in a series of in silico and in vivo experiments.

The seed localization process is carried out efficiently using separable Gaussian filters in a probabilistic Gibbs random field framework. An approximation of the needle path through the prostate volume is obtained using a polynomial fit. The seeds are then detected and assigned to their corresponding needles by calculating local maxima of the voronoi region around the needle position. In our experiments, we were able to successfully localize over 85% of the implanted seeds. Furthermore, as a regular part of a brachytherapy cancer treatment, patient's prostate is scanned using a trans-rectal ultrasound probe, its boundary is manually outlined, and its volume is estimated for dosimetry purposes. In this thesis, we also propose a novel semi-automatic segmentation algorithm for prostate boundary detection that requires a reduced amount of radiologist's input, and thus speeds up the surgical procedure. Saved time can be used to re-scan the prostate during the operation and accordingly adjust the treatment plan. The proposed segmentation algorithm utilizes texture differences between ultrasound images of the prostate tissue and the surrounding tissues. It is carried out in the polar coordinate system and it uses three-dimensional data correlation to improve the smoothness and reliability of the segmentation. Test results show that the boundary segmentation obtained from the algorithm can reduce manual input by the factor of 3, without significantly affecting the accuracy of the segmentation (i.e. semi-automatically estimated prostate volume is within 90% of the original estimate)"--Abstract.
