While extensive information and literature exist on motivation and communication, their specific applications in the realm of occupational safety and loss prevention are relatively scarce. I aspire to offer this book as a valuable resource to safety professionals, safety advisors and representatives, and industry line managers to improving plant facility design, training for operation and maintenance staff, and the development of safe operating procedures. Its purpose is to unravel what has gone wrong in the past and provide guidance on preventing similar incidents in the future. Regrettably, history has shown that many incidents tend to recur after a few months, often due to people moving on and lessons being forgotten.
