Book excerpt: Nurse executives are challenged to provide adequate nurse staffing to prevent adverse patient outcomes, promote patient satisfaction and enhance nurses' job satisfaction. Historically, nurse staffing strategies have involved the use of patient classification systems, the use of time studies to predict staffing numbers and the best guesses of the nurse administrators. Collective bargaining units moved to include staffing ceilings or nurse-to-patient ratios to address staffing needs in hospitals. In 1999, the state of California signed AB394 into law requiring the implementation of mandated minimum nurse-to-patient ratios. The California Department of Health Services estimated that the mandated nurse-to-patient ratios would cost California hospitals $956 million annually after implementation given adequate recruiting sources (CHA, 2004). While this estimate was made prior to the known fiscal impact of the ratios, no study has been published addressing the actual costs differences to the hospitals of the mandated nurse-to-patient ratios that were implemented in 2004. As other states consider introducing legislation to implement the staffing ratios as California has done, the impact of these costs should be revealed. The aim of this study was to examine the impact on nursing costs expressed as a percentage of hospital operating costs from before the staffing ratios in California were implemented (2002) and after the staffing ratios were implemented (2006). Two hundred eighty-two hospitals were included in this study. Financial and productivity data were obtained from the Office of Statewide Health Planning and Development in California. Examination of the percentage of total hospital operating costs attributed to nursing costs shed light on the fiscal impact of the mandated nurse-to-patient ratios in California. Results demonstrated that there was significant difference in the percentage of total hospital operating costs attributed to Registered Nurse costs and to overall nursing costs. There was no significant difference in the percentage of total hospital operating costs attributed to the costs for the licensed vocational nurse. In addition, there were no significant differences noted in the level of unit support staff (aides/orderlies, technicians, clerical) after the implementation of the ratios.

AB 394 was introduced by Assembly person, Khuel, and signed into law by Governor Davis in the fall of 1999. This law is the first law mandating nurse staffing ratios for acute-care hospitals. This paper determines whether the nurse- to-patient ratio law has impacted acute care hospitals financially due to in-hospital mortality and/or unplanned readmission to intensive care units. The California Department of Health Services estimated that the mandated nurse-to-patient ratios would cost California hospitals $956 million annually after implementation given adequate recruiting sources (CHA, 2004). Not many study’s have been published addressing the fiscal impact to hospitals of the mandated nurse-to-patient ratios that were implemented in 2004. Previous results have shown that there was not a complex financial burden on hospitals, as the quality care of acute care patients was impeccable. From the research conducted also showed that in hospital mortality and/or unplanned readmission to intensive care units decreased due to AB 394.

Book excerpt: We use data from California to document and offer possible explanations for the sharp increase in hospital prices charged to private payers after 1999. We find a downward trend in price for private pay patients in the 1990s and a rapid upward trend beginning in 1999, amounting to an annual average increase of 10.6% per year over 1999-2005. Prices in 2006 were almost double prices in 1999. By contrast, there was little discernable trend in prices for Medicare and Medicaid patients, although these prices varied from year-to-year. Surprisingly, the increase in prices is not correlated, geographically, with the change in hospital market concentration. For example, the greatest price rises came from hospitals in monopoly and highly concentrated counties which experienced little or no change over our sample period. Two recent California state hospital regulations, the seismic retrofit mandate and the mandatory nurse staffing ratio affected hospital costs. However, the cost increases due to the nursing staffing regulations are not large enough to account for the price increase, and the price increase is not substantially correlated with the costs of compliance with the seismic retrofit mandate. Therefore, the source of the near-doubling of California hospital prices remains something of a mystery.
